Clinical Practice Guidelines We Can Trust examines the current state of clinical practice guidelines and how they can be improved to enhance healthcare quality and patient outcomes. Clinical practice guidelines now are ubiquitous in our healthcare system. The Guidelines International Network (GIN) database currently lists more than 3,700 guidelines from 39 countries. Developing guidelines presents a number of challenges including lack of transparent methodological practices, difficulty reconciling conflicting guidelines, and conflicts of interest. Clinical Practice Guidelines We Can Trust explores questions surrounding the quality of CPG development processes and the establishment of standards. It proposes eight standards for developing trustworthy clinical practice guidelines emphasizing transparency; management of conflict of interest ; systematic review-guideline development intersection; establishing evidence foundations for and rating strength of guideline recommendations; articulation of recommendations; external review; and updating. Clinical Practice Guidelines We Can Trust shows how clinical practice guidelines can enhance clinician and patient decision-making by translating complex scientific research findings into recommendations for clinical practice that are relevant to the individual patient encounter, instead of implementing a one size fits all approach to patient care. The recommendations in this guideline can be used by policy-makers and programme managers to assess current intake levels of free sugars in their countries relative to a benchmark. They can also be used to develop measures to decrease intake of free sugars, where necessary, through a range of public health interventions. Examples of such interventions and measures that are already being implemented by countries include food and nutrition labelling, consumer education, regulation of marketing of food and non-alcoholic beverages that are high in free sugars, and fiscal policies targeting foods and beverages that are high in free sugars. This guideline should be used in conjunction with other nutrient guidelines and dietary goals, in particular those related to fats and fatty acids (including saturated fatty acids and trans-fatty acids), to guide development of effective public health nutrition policies and programmes to promote a healthy diet.

Module 2: screening – systematic screening for tuberculosis disease is an updated and consolidated summary of WHO recommendations on systematic screening for tuberculosis (TB) disease, containing 17 recommendations for populations in which TB screening should be conducted and tools to be used for TB screening. TB screening is strongly recommendations for household and close contacts of individuals with TB, people living with HIV, miners exposed to silica dust, and prisoners. In addition, screening is conditionally recommended for people with risk factors for TB attending health care, and for communities with risk factors for TB and limited access to care (e.g. homeless, urban poor, refugees, migrants). General population screening is recommended in high-burden settings (0.5% prevalence or higher). Symptoms, chest radiography (CXR), and molecular WHO-recommended rapid diagnostic tests for TB are recommended as screening tools for all adults eligible for screening. Computer-aided detection programmes are recommended as alternatives to human interpretation of CXR in settings where trained personnel are scarce. For people living with HIV, C-reactive protein is also a good screening tool. This guideline document is accompanied by an operational handbook, the WHO operational handbook on tuberculosis. Module 2: screening – systematic screening for tuberculosis disease, that presents principles of screening, steps in planning and implementing a screening programme, and algorithm options for screening different populations.

It provides an outline for developing and managing a PHEOC to achieve a goal-oriented response to public health emergencies and unity of effort among response agencies. A public health emergency is here defined as an occurrence, or imminent threat, of an illness or health condition that poses a substantial risk of a significant number of human fatalities, injuries or permanent or long-term disability. Public health emergencies can result from a wide range of hazards and complex emergencies. Experience has shown that timely implementation of an EOC provides an essential platform for the effective management of public health emergencies. Public health emergencies involve increased incidence of illness, injury and/or death and require special measures to address increased morbidity, mortality and interruption of essential health services. For such emergencies, a multi-agency, multi-jurisdictional response is often required, working with the national disaster management organization. When normal resources and capacities are exceeded, support from outside the affected areas will also be required. External assistance could include national, cross-border, regional or international resources.

Book excerpt: In this publication, recommendations for the Use of dual-stain cytology to triage women after a positive test for human papillomavirus (HPV) are presented. Dual-stain cytology can be used as a triage test in cervical “screen, triage and treat" algorithms for cancer prevention. It is performed on liquid-based cytology (LBC) slides (not on conventional Pap smears) to detect the presence of two proteins: p16 and Ki-67. Recommendation for the general population of women: In a screen, triage and treat approach using HPV Nucleic Acid Tests (NATs) as the primary screening test among the general population of women, WHO suggests using partial genotyping, colposcopy, VIA, cytology or dual-stain cytology to triage women after a positive HPV NATs result. When providing dual-stain cytology to triage women after a positive HPV NAT, WHO suggests: using samples collected by the health worker; and retesting with HPV NATs 24 months after a negative dual-stain cytology result. These are conditional recommendations based on low-certainty evidence for dual-stain cytology as a triage test. No recommendation was made for using dual-stain cytology to triage women living with HIV after a positive HPV DNA test, because evidence on the outcomes of using dual-stain cytology applicable to this population was minimal.
