Book excerpt: The brain is an extremely energy consuming part of the body, which makes it dangerously vulnerable to metabolic stress. It’s no wonder then that abnormalities of brain energy metabolism are becoming the usual suspects and a hallmark of many neurodegenerative diseases. The socioeconomic burden of these alone begs for urgent measures to be taken for better understanding both fundamental and applied problems of neuroenergetics and neuroprotection. For instance, brain imaging reveals that the diseased brains of Alzheimer’s patients cannot efficiently utilize the vital brain fuel, glucose. The resulting energy deficit causes neuronal hyperactivity, seizures and cognitive impairments. Administration of native energy substrates complementary to glucose is a logical (and attractive in its simplicity) approach in fighting the energy crisis in the brain*. The two closely related aspects of brain activity -- neuronal and metabolic – are currently considered to be of utmost importance in both fundamental and applied neuroscience. Although recently the studies of both brain activity and metabolism in normal conditions, under metabolic stress, and in neurodegenerative diseases have experienced significant progress, their overlapping areas deserve further clarification by joint efforts from experts in such fields as (1) energy demands, supplies, and efficiency at the cellular level: in neurons, glial elements, micro-vessels and in the process of their coordinated interactions; (2) specific roles of energy substrates in fine-tuning of the demand-supply mechanism in the condition of metabolic stress; and (3) the macro-level of energy homeostasis and dietary manipulations possible beneficial for neurodegenerative diseases. The result of combining into a coherent whole the recent findings in these fields will hopefully bring forward a broader view and better understanding of the knowledge continuum, which is under the threat of further fragmentation due to the unavoidable process of specialization in neuroscience. Current issue covers the three major groups of topics: 1. The Pros and Cons of studies of neuronal activity using brain slice preparations 2. The role of particular energy substrates in metabolic support of neuronal activity 3. The macro-level of energy homeostasis and the dietary manipulations that seem promising in prevention and correction of the diseases of brain energy metabolism.

Autoregulation is pronounced in the brain, with myogenic, metabolic and neurogenic mechanisms contributing to maintain relatively constant blood flow during both increases and decreases in pressure. In addition, unlike peripheral organs where the majority of vascular resistance resides in small arteries and arterioles, large extracranial and intracranial arteries contribute significantly to vascular resistance in the brain. The prominent role of large arteries in cerebrovascular resistance helps maintain blood flow and protect downstream vessels during changes in perfusion pressure. The cerebral endothelium is also unique in that its barrier properties are in some way more like epithelium than endothelium in the periphery. The cerebral endothelium, known as the blood-brain barrier, has specialized tight junctions that do not allow ions to pass freely and has very low hydraulic conductivity and transcellular transport. This special configuration modifies Starling's forces in the brain microcirculation such that ions retained in the vascular lumen oppose water movement due to hydrostatic pressure. Tight water regulation is necessary in the brain because it has limited capacity for expansion within the skull. Increased intracranial pressure due to vasogenic edema can cause severe neurologic complications and death.

A concluding chapter, devoted to the human, includes an anatomical atlas of the human lower brainstem. Emphasis is placed on anatomically-established neural circuitry rather than on poorly characterized functional centers (e.g., vasomotor centers). Similarly, conventionally recognized neural systems, including the limbic system, the reticular formation and the autonomic nervous system, are shown to be unhelpful as explanatory concepts. The autonomic nervous system, for example, comprises afferent and efferent neurons which connect the brain with the various cardiovascular and visceral structures.It does not function in its own right. Patterns of autonomic discharge originate in the brain. The book stresses the role of the visceral information that reaches the brainstem via afferent components of the lower cranial nerves, including the vagus. Homeostatic functions involve changes in behavior as well as changes in internal physiological functions, an integration achieved by the nervous system functioning as a whole.

Book excerpt: First published in 1994, this book explores the paradigm of muscles as molecular and metabolic machines in which all structures and functions are exquisitely integrated and matched to each other. The analysis begins with a standard reductionist approach-reviewing the integrated machine parts. The key working components of the complete muscle machine are proteins (soluble, organelle, or membrane localized), and a conservative count indicates that today more than 100 such machine parts are known, essentially all occurring as cell specific isoforms. Random assortment of these machine parts or protein isoforms could generate an astronomical number of "muscle machines" and an equally enormous number of muscle fiber types. The question is, why aren't such large numbers ever seen? To attack this problem, the reductionist approach is complemented with an integrationist/adaptational one. Evidence is presented that the more highly specialized the muscle type, the further one moves from the above extreme; in the most highly specialized muscles, typically only one fiber type is found. It is argued that instead of random assortment of isoforms or machine parts, only specific and often unique combinations can work in appropriate fashion. A few established examples of this fundamental principle are reviewed, but emphasis is placed on the fact that we know dreadfully little about why this is so and what kinds of further studies are needed. The issue of why the very large numbers of fiber types theoretically possible are never even approximately realized has never before been addressed. Indeed, it is rarely recognized. Muscles as Molecular and Metabolic Machines is the first work of its kind on the subject.
