Many of the animal experiments however were done on open-chested dogs with cut cardiac nerves and under the influence of pharmaca. It is doubtful, therefore whether these results can be transferred without reser vation to the human situation. The introduction of electrical stimulation of the heart in clinical cardiology has opened new ways to study some aspects of cardiac arrhythmias directly in the unaesthesized patient. This study reports observations on patients who were admitted to the University Department of Cardiology, Wilhelmina Gasthuis, Amsterdam, for the evaluation and treatment of tachycardias. Electrically induced premature beats were used in an effort to elucidate the origin and mechanism of these tachycardias. The first chapter is on classification and diagnosis of tachy cardias with special emphasis on our current knowledge of the differential diagnosis between supraventricular tachycardias with aberrant conduction and ventricular tachycardias. This is followed by theoretical considerations on tachycardias especially in relation to the methods used in this study. After an outline of these methods the results of our studies in patients with atrial flutter, A-V junc tional tachycardias and tachycardias related to the pre-excitation syndrome are reported. A discussion on the value of electrical stimulation for the treatment of tachycardias is followed by a summary of our results.

Book excerpt: The molecular basis for atrial fibrillation continues to be largely unknown, and therapy remains unchanged, aimed at controlling the heart rate and preventing systemic emboli with anticoagulation. Familial atrial fibrillation is more common than previously suspected. While atrial fibrillation is commonly associated with acquired heart disease, a significant proportion of individuals have early onset without other forms of heart disease, referred to as "lone" atrial fibrillators. It is also well recognized that atrial fibrillation occurs on a reversible or functional basis, without associated structural heart disease, such as with hyperthyroidism or of atrial fibrillation following surgery. It remains to be determined what percentage in these individuals is familial or due to a genetic predisposition. Mapping the locus for familial atrial fibrillation is the first step towards the identification of the gene. Isolation of the gene and subsequent identification of the responsible molecular genetic defect should provide a point of entry into the mechanism responsible for the familial form and the common acquired forms of the disease and eventually provide more effective therapy. We know that the ionic currents responsible for the action potential of the atrium is due to multiple channel proteins as is electrical conduction throughout the atria. Analogous to the ongoing genetic studies in patients with familial long QT syndrome, it is highly likely that defects in each of these channel proteins will be manifested in familial atrial fibrillation.

Technological advancements have included the recording of intracellular potentials from cardiac cells, the recording of intracardiac extra cellular potentials generated by specific cardiac structures, simultaneous record ings from numerous sites with computer processing for spatial mapping of activa tion or potential variations with time, high gain, high resolution recordings with signal averaging for detecting potentials of low amplitudes, complex stimulation protocols, various high energy stimulation modes, intracellular voltage control of multicellular preparations and single cardiac cells, and the isolation of single cardiac cells for electrophysiological study. The interest and technology have produced an increasing bounty of information and understanding, acceptable solutions to some clinical problems, and definite progress toward solutions to other problems. Progress in research in electrophysiology and arrhythmias has been reviewed and highlighted in various meetings and books in recent years. Because the body of information has become so large, general overviews of the field have neces sarily been superficial in certain aspects or have contained gaps.
