Book excerpt: Le niveau de risque des femmes enceintes est défini à partir des recommandations de l'HAS, et il en est de même pour les différents types de suivi. L'objectif de cette étude était de déterminer la conformité des suivis selon les facteurs de risques des patientes au sein du RPAI. Il s'agit d'un audit de pratique multicentrique sur la description du suivi de 100 femmes au sein des maternités du réseau, entre le 14 septembre 2012 et le 14 décembre 2012. 100 dossiers ont été analysés : 38 à l'HCE, 20 à Voiron, 25 à la Clinique Mutualiste, 17 à la Clinique des Cèdres, la Clinique Belledonne n'ayant pas accepté de participer à l'étude. 89% des suivis sont conformes. Les suivis A, pouvant être menés par un médecin généraliste ou spécialiste, ou une sage-femme, représentent quasiment 75% des suivis en début de grossesse. Cette prévalence diminue au cours de la grossesse avec l'apparition de pathologies mais reste néanmoins largement majoritaire au 9ème mois. Les gynécologues obstétriciens sont les plus consultés dans le suivi prénatal en assurant 54% des suivis dès la première consultation. Cette étude a permis d'évoquer le manque probable d'information des patientes sur les compétences des autres acteurs de soins dans le suivi prénatal et donc la nécessité d'une campagne d'information sur ce sujet. Elle a aussi mis en évidence l'importance d'une réorientation des patientes selon leurs facteurs de risque pour un suivi adapté de toutes les femmes également en vue de l'évolution de la démographie des professionnels.

C’est pourquoi l’HAS a émis en 2007 des recommandations sur l’orientation des femmes enceintes en fonction de situations à risque identifiées. Par les formations médicales et le développement d’un dossier périnatal partagé (DPP), le Réseau Périnatal de Basse-Normandie a mis en pratique ces recommandations. Afin de savoir si celles-ci ont modifié l’organisation du suivi de grossesse et amélioré sa sécurité, une double enquête a été réalisée sur le bassin de population d’Avranches-Granville. D'abord une enquête prospective, par questionnaire, a sollicité 119 professionnels de la périnatalité entre le 15 juin 2012 et le 31 juillet 2013. En complément, une enquête rétrospective de 75 dossiers de grossesse a été réalisée sur les années 2006, 2010 et 2012. L'étude a comparé la prise en charge des grossesses en fonction des risques avant et après les recommandations puis après la mise en place du DPP. 56% des professionnels connaissent ces recommandations mais seulement la moitié les utilise et en majorité des Médecins Généralistes. 30% des réponses concernant des cas concrets sont en adéquation avec l’HAS et un suivi moins strict est choisi dans 55% des cas. On ne retrouve pas d’amélioration de la prise en charge des risques depuis 2006. L’utilisation du DPP n’a pas augmenté l’adhésion des professionnels à cette classification malgré son utilisation majeure à hauteur de 78%. Les sages-femmes et médecins généralistes sont de plus en plus sollicités et leur implication croissante sera une alternative à proposer pour un suivi adapté à chaque patiente face à la diminution des obstétriciens.

Notre travail consistera donc à mettre en place un outil d'aide à l'orientation des patientes, lors d'un entretien dédié au moment de leur inscription à la maternité, afin de les guider dans le choix du professionnel responsable du suivi des six premiers mois de leur grossesse. L'outil a été développé selon le guide méthodologique édité par la HAS en 2008 concernant l'élaboration d'un document écrit d'information à l'intention des patients et des usagers du système de santé. Une première version a été établie à partir du rapport de la HAS de 2007 sur le suivi et l'orientation des femmes enceintes en fonction des situations à risque identifiées. Deux groupes de travail pluridisciplinaire constitués de sages-femmes, médecins généralistes et gynécologues obstétriciens ont ensuite permis de l'améliorer. Deux types de suivi ont été définis. Le suivi par une sage-femme ou, un médecin généraliste ou, un gynécologue médical ou obstétricien dans le cas des situations à risque modéré et le suivi par un gynécologue-obstétricien de l'équipe responsable de l'accouchement dans le cas des situations à risque élevé. Chacun de ces suivis pouvant être complété d'un avis spécialisé. Ces avis complémentaires ont ainsi été étayés et il a notamment été précisé à quel moment ceux-ci devaient intervenir. La surveillance des grossesses, essentiellement assurée jusqu'ici par les gynécologues-obstétriciens, devrait évoluer, notamment vers une responsabilité croissante des sages-femmes et des médecins généralistes dans le suivi des grossesses dites physiologiques. Le défi à relever pour tous les professionnels de la périnatalité est donc d'apprendre à travailler en réseau pour un accompagnement à la fois médical, psychologique et social des futurs parents

L'objectif de l'étude est de recueillir le ressenti des femmes enceintes sur le vécu de la consultation et du suivi ambulatoire afin d'améliorer notre pratique. Méthode : Une étude qualitative par entretiens semi dirigés auprès de femmes enceintes à bas risque majeur ayant bénéficié d'une consultation d'orientation à l'hôpital Jeanne de Flandre. Résultat : 15 entretiens ont été réalisés. La consultation d'orientation a été bien vécue par les patientes. Les femmes enceintes déclarent un manque d'information sur le suivi de grossesse. Cette consultation permet de s'informer sur le suivi de grossesse, de se sentir en sécurité et d'être rassurée. Au final, cette consultation permet de se préparer à la suite du suivi. Les critiques sur la consultation sont le caractère redondant en cas de grossesse précédente, le trop plein d'information, la survenue tardive dans le suivi, le faible choix dans les professionnels libéraux. Suite à cette consultation, elles acceptent le principe d'un suivi ambulatoire. Elles reconnaissent ne pas avoir besoin d'un suivi plus important. Le suivi ambulatoire a été globalement bien vécu par les patientes. Elles en apprécient la proximité, l'intimité, la flexibilité. Au final ce suivi ambulatoire apparaît équivalent au suivi hospitalier et leur parait également plus adapté avec le recul. Enfin, les patientes ont relevé peu d'échanges entre professionnels de la périnatalité mais notent que ces échanges se font correctement avec le dossier obstétrical. Conclusion : La consultation d'orientation permet de redonner un sens au caractère physiologique de la grossesse. Le suivi ambulatoire est accepté et apprécié des patientes. La création de cette consultation devrait ainsi permettre un retour du suivi de femmes enceintes vers la ville et la médecine ambulatoire.

The 2018 TZG updates and expands on the guidance in the one previous jointly-developed, zoonoses-specific guidance document: the 2008 Tripartite “Zoonotic Diseases: A Guide to Establishing Collaboration between Animal and Human Health Sectors at the Country Level”, developed in WHO South-East Asia Region and Western Pacific Region. The 2018 TZG supports building by countries of the resilience and capacity to address emerging and endemic zoonotic diseases such as avian influenza, rabies, Ebola, and Rift Valley fever, as well as food-borne diseases and antimicrobial resistance, and to minimize their impacts on health, livelihoods, and economies. It additionally supports country efforts to implement WHO International Health Regulations (2005) and OIE international standards, to address gaps identified through external and internal health system evaluations, and to achieve targets of the Sustainable Development Goals. The 2018 TZG provides relevant country ministries and agencies with lessons learned and good practices identified from country-level experiences in taking OH approaches for preparedness, prevention, detection and response to zoonotic disease threats, and provides guidance on multisectoral communication, coordination, and collaboration. It informs on regional and country-level OH activities and relevant unisectoral and multisectoral tools available for countries to use.

Book excerpt: Mitochondrial replacement techniques (MRTs) are designed to prevent the transmission of mitochondrial DNA (mtDNA) diseases from mother to child. While MRTs, if effective, could satisfy a desire of women seeking to have a genetically related child without the risk of passing on mtDNA disease, the technique raises significant ethical and social issues. It would create offspring who have genetic material from two women, something never sanctioned in humans, and would create mitochondrial changes that could be heritable (in female offspring), and therefore passed on in perpetuity. The manipulation would be performed on eggs or embryos, would affect every cell of the resulting individual, and once carried out this genetic manipulation is not reversible. Mitochondrial Replacement Techniques considers the implications of manipulating mitochondrial content both in children born to women as a result of participating in these studies and in descendants of any female offspring. This study examines the ethical and social issues related to MRTs, outlines principles that would provide a framework and foundation for oversight of MRTs, and develops recommendations to inform the Food and Drug Administration's consideration of investigational new drug applications.

The social sciences have been accumulating knowledge since the early fifties as to how we are regulated by others. S. Milgram, S. Ashe, P. Zimbardo and J. Murray Bowen, M.D., the first researcher to observe several live-in families, for up to three years, at the National Institute of Mental Health. Describing how family members overly influence one another and distribute stress unevenly, Bowen described both how symptoms and family leaders emerge in highly stressed families. Our brain is not organized to automatically perceive that each family has an emotional system, fine-tuned by evolution and "valuing" its survival as a whole, as much as the survival of any individual. It is easier to see this emotional system function in ants or mice but not in humans. The emotional system is organized to snooker us humans: encouraging us to take sides, run away from others, to pressure others, to get sick, to blame others, and to have great difficulty in seeing our part in problems. It is hard to see that we become anxious, stressed out and even that we are difficult to deal with. But "thinking systems" can open the doors of perception, allowing us to experience the world in a different way. This book offers both coaching ideas and stories from leaders as to strategies to break out from social control by de-triangling, using paradoxes, reversals and other types of interruptions of highly linked emotional processes. Time is needed to think clearly about the automatic nature of the two against one triangle. Time and experience is required as we learn strategies to put two people together and get self outside the control of the system. In addition, it takes time to clarify and define one's principles, to know what "I" will or will not do and to be able to take a stand with others with whom we are very involved. The good news is that systems' thinking is possible for anyone. It is always possible for an individual to understand feelings and to integrate them with their more rational brains. In so doing, an individual increases his or her ability to communicate despite misunderstandings or even rejection from important others. The effort involved in creating your Mindful Compass enables us to perceive the relationship system without experiencing it's threats. The four points on the Mindful Compass are: 1) Action for Self, 2) Resistance to Forward Progress, 3) Knowledge of Social Systems and the 4) The Ability to Stand Alone. Each gives us a view of the process one enters when making an effort to define a self and build an emotional backbone. It is not easy to find our way through the social jungle. The ability to know emotional systems well enough to take a position for self and to become more differentiated is part of the natural way humans cope with pressure. Now people can use available knowledge to build an emotional backbone, by thoughtfully altering their part in the relationship system. No one knows how far one can go by making an effort to be more of a self-defined individual in relationships to others. Through increasing emotional maturity, we can find greater individual freedom at the same time that we increase our ability to cooperate and to be close to others.
