This guideline provides evidence-based recommendations in the following areas: prevalence thresholds, target age groups and frequency of PC, establishment of WASH and snail control activities to support control and elimination of schistosomiasis, diagnostic tests for the assessment of schistosomiasis infection in animal reservoirs, in snail hosts, and in humans. The guideline will provide support to Member States, programme managers, health workers and other stakeholders on the implementation of national schistosomiasis control and elimination programme.

These infections were known to the ancient Egyptians and their transmission shows little evidence of slowing down, globally. The good news is that field applicable, and increasingly affordable, chemotherapy has been available for almost 25 years. Using chemotherapy and other means of control, some countries have decreased transmission and made excellent headway against morbidity. The bad news is that the public health problems caused by schistosomiasis are still with us, with the estimated number of cases of schistosomiasis, while shifting geographically, remaining approximately 200 million for the last 30 years. In fact, with the development of field usable ultrasound technology and meta-analyses performed on existing data, there is a new appreciation for the extent of non-lethal morbidity associated with these infections. While the percentage of individuals with severe hepatosplenic disease remains below 10%, recent reassessments of morbidity associated with schistosomiasis indicate that the prevelance of symptoms and the cost in diability-adjusted life years is much greater than was previously, commonly appreciated (Van der Werf, M. J. , et al. 2003, Acta Tropica 86:125-139; Charles H. King, personnel communication). Strong impetus for addressing these issues is provided by the World Health Assembly’s recently passed Resolution 54. 19, which calls for efforts to reduce morbidity caused by schistosomiasis and soil-transmitted helminths in school-aged children, largely through chemotherapy campaigns.

Book excerpt: Current efforts to limit the ravages of schistosomiasis are pushing the world closer to eliminating a chronic infection that has been associated with human life in the tropics since time immemorial. This notwithstanding, the disease remains a scourge for large populations in sub-Saharan Africa, Latin America, and Southeast Asia, and the main part of this book is made up by papers dealing with its current distribution, discussing ways and means to establish and implement improved control approaches. While chemotherapy limits the symptoms caused by schistosomiasis, the number of infected people will not decrease until the parasite's life cycle is interrupted. To that end, some papers focus on the intermediate snail host, which is notoriously difficult to control, while others discuss human hygiene and sanitation. The latter approach not only prevents infection through avoiding people being infected from the snail, but more importantly, also stops people infecting the snail by leaving contagious feces and urine in nature. With morbidity reduced by chemotherapy, the immediate target now is the interruption of transmission to be achieved by new tools, such as the novel chemotherapies, improved diagnostics (for humans, animals, and snails), and vaccines discussed in several of the papers. As made clear in this book, a complex infection requires new tools as well as work on many fronts, above all; however, a clear idea is needed as to how to skillfully combine the tools available and sustain implemented control activities.

Book excerpt: The soil-transmitted nematode parasites, or geohelminths, are - called because they have a direct life cycle, which involves no intermediate hosts or vectors, and are transmitted by faecal contamination of soil, foodstuffs and water supplies. They all inhabit the intestine in their adult stages but most species also have tissue-migratoryjuvenile stages, so the disease manifestations they cause can therefore be both local and systemic. The geohelminths together present an enormous infection burden on humanity. Those which cause the most disease in humans are divided into three main groupings, Ascaris lumbricoides (the large roundworm), Trichuris trichiura (whipworm), and the blood-feeding hookworms (Ancylostoma duodenale and Necator americanus ), and this book concentrates on these. These intestinal parasites are highly prevalent worldwide, A. lumbricoides is estimated to infect 1471 million (over a quarter ofthe world’s population), hookworms 1277 million, and T. trichiura 1049 million. The highly pathogenic Strongyloides species might also be classified as geohelminths, but they are not dealt with here because the understanding of their epidemiology, immunology and genetics has not advanced as rapidly as for the others. This is primarily because of the often covert nature of the infections, with consequent difficulties for analysis. If there is ever a second edition of this book, then there will hopefully be much to say about this infection.

It leads to the loss of about 4.5 million disability-adjusted life years (DALYs).

This report presents an evaluation of current research and challenges in controlling the helminthiases of public health importance, including onchocerciasis, lymphatic filariasis, soil-transmitted helminthiases, schistosomiasis, food-borne trematodiases and taeniasis/cysticercosis. The evaluation covers five major themes--intervention, epidemiology and surveillance, environmental and social ecology, data and modelling, and fundamental biology. Despite the recent demonstrated successes and expansion of tools for the helminthiases outlined here, and the development of some research capacity, the evaluation found major deficiencies in our current control tools, in diagnostics, and in our fundamental knowledge of helminth biology and transmission dynamics, as well as in capacity and policy for health research. Thus the current research issues are summarized here, and opportunities for improving disease control and reducing poverty are identified. Recommendations are presented to inform public health policy, guide implementation programs, and focus the research community on the needs of disease control and the opportunities for bettering human welfare. This is one of ten disease and thematic reference group reports that have come out of the TDR Think Tank, all of which have contributed to the development of the Global Report for Research on Infectious Diseases of Poverty.
