Book excerpt: Human schistosomes (blood flukes) are digenetic trematodes that spend the adult part of their life cycle in humans and a further part in aquatic snails. Despite advances in chemotherapy, schistosomiasis is still a significant infection in the populations of several countries in the tropics. This book replaces a previous volume Schistosomiasis: Epidemiology, Treatment and Control (Heinemann, 1982) by Jordan and Webbe. All chapters have been rewritten by internationally renowned workers. Ultrasound, expected to aid identification of early disease in the field and increase our understanding of its evolution, is discussed in a new chapter. Others, each with an extensive bibliography, review the parasites and their snail intermediate hosts, epidemiology, clinical manifestations and pathology, diagnosis, immunology, drugs and patient management and control. Limitations of the role of chemotherapy in morbidity control are discussed and the need for flexibility in control interventions in the varied epidemiological situations is stressed. An interdisciplinary approach may be necessary to reduce transmission by appropriate measures against the snail intermediate host, and to implement public health measures, including the provision of safe water (with many other medical and social benefits) and health education. This has led to a growing international interest in trematode infections, although factors such as the difficulties entailed in the diagnosis, the complexity of human and agricultural practices, the lack of assessments of the economic costs or the limited number of effective drugs are preventing the development of control measures of these diseases in humans and livestock. In-depth studies are needed to clarify the current epidemiology of these helminth infections and to identify new and specific targets for both effective diagnosis and treatments. The main goal of this book is to present the major trematodes and their corresponding diseases in the framework of modern parasitology, considering matters such as the application of novel techniques and analysis of data in the context of host-parasite interactions and to show applications of new techniques and concepts for the studies on digenetic trematodes. Book excerpt: Evaluates the carcinogenic risk to humans posed by infection with three schistosoma trematodes, three liver flukes and the bacterium Helicobacter pylori. These biological agents were selected for evaluation on the basis of evidence suggesting a causal association between infection and the development of human cancers. The first monograph assesses data on Schistosoma haematobium S. mansoniand S. japonicum, the three species which account for the vast majority of schistosomal disease in humans. The most extensive sections evaluate the results of studies of cancer in humans and experimental animals, concentrating on the strength of evidence linking infection with S. haematobium to squamous-cell carcinoma of the urinary bladder, and infection with S. mansoni or S. japonicum to an increased risk for cancers of the gastrointestinal tract. The monograph concludes that infection with S. haematobium is carcinogenic to humans and that infection with S. japonicum is possibly carcinogenic to humans; infection with S. mansoni could not be classified. The second monograph on liver flukes evaluates data on Opisthorchis viverriniO. felineus and Clonorchis sinensis. These foodborne trematodes establish a chronic infection of the bile ducts and more rarely the pancreatic duct and gall-bladder of humans and other mammals. Most carcinogenicity data centre on the possibility that these trematodes are involved in the development of cholangiocarcinoma or liver cancer. The monograph concludes that infection with O. viverrini is carcinogenic to humans and that infection with C. sinensis is probably carcinogenic to humans; infection with O. felineus could not be classified. The final monograph evaluates data on Helicobacter pylori. More than half of the world's population may be infected with this bacterium, which is responsible for most cases of chronic gastritis and duodenal ulcer. Citing several studies linking infection to gastric cancer in humans, the monograph concludes that infection with H. pylori is carcinogenic to humans.

In many parts of Africa parasitic helminths are responsible for enormous economic losses, hampering rural development programmes and reducing the pace of economic growth. Many parasitic helminths are either zoonoses (diseases naturally transmitted between vertebrate animals and man) or have evolved from animal parasites. The modification of the environment through wars, famine and the ever expanding and increasingly mobile human population brings people into close contact with new environments and wildlife species which makes the study and control of zoonoses of special interest and complexity. In Africa, the transmission of helminth parasites is highly influenced by the ever changing social and cultural differences between diverse groups of peoples and their interaction with wild and domestic animals. It is not surprising, therefore, that approaches to the study and control of parasitic zoonoses require intersectoral cooperation between physicians, veterinarians, parasitologists, zoologists, demographers, anthropologists, engineers and economists to provide the breadth of knowledge and expertise required to develop our understanding of these diseases and to devise methods for their control. Around the turn of this century, parasitology enjoyed a certain vogue, inspired by colonial responsibilities of the technically ad vanced countries, and by the exciting etiological and therapeutic discoveries of Ross, Manson, Ehrlich, and others. For some decades, the Western hemi sphere's interest in animal parasites has been eclipsed by concern for bacteria and viruses as agents of transmissible disease. Only very recently, initiatives like the Tropical Disease Research programs of WHO-World Bank-UNDP, and the Great Neglected Disease networks of the Rockefeller and MacArthur Foundations have begun to compensate for the neglect of these problems by United States federal health research agencies. Throughout that period, how ever, the Rockefeller Institute (later University) has given high priority to the challenges of parasitism, corresponding during a formidable period with Dr. Trager's own career. The present work then, is a distillation of the insight collected by our principal doyen of parasite biology, informed but by no means confined to his own research. It is addressed to the reader of broad biological interest and training, not to the specialist. The disarmingly unpretentious style makes the work readily accessible to college undergraduates or even to gifted high school students; but do not be deceived thereby, as it has an enormous range of factual information and theoretical insight, familiar to few, but potentially important to most biologists.
