L'analyse biomécanique montre que l'utilisation d'implants inclinés permet de diminuer la longueur des cantilevers distaux et donc de réduire les contraintes mécaniques. Les résultats cliniques montrent que ce protocole peut être considéré comme une technique prédictible, avec un excellent pronostic à court, moyen et long terme.

Deux de ces propositions thérapeutiques s'articulent autour d'un nombre d'implants minimum, avec d'une part, une solution fixe avec une prothèse implanto-portée sur 6 implants et d'autre part, une solution amovible avec une prothèse implanto-retenue. La littérature, ne permet pas à l'heure actuelle d'établir un fil conducteur dirigeant systématiquement vers une de ces deux situations. De cette étude de la littérature, il ressort, avec un niveau de preuve moyen, qu'au moins 4 implants maxillaires sont nécessaires afin de réaliser une prothèse amovible implanto-retenue. Concernant le concept d'all-on-six, cette solution implanto-portée permet de pallier l'aspect amovible de la prothèse complète mais nécessite un nombre d'étapes laboratoires et cliniques supplémentaires ainsi qu'une dextérité accrue avec le placement des implants distaux le long de la paroie sinusienne. Le choix se fait de manière plurifactorielle, dépendant de critères tels que la résorption osseuse, le décalage des bases osseuses, les moyens financiers, le contexte biologique du patient et ses attentes. L'alliance thérapeutique entre le praticien et le patient conditionne le choix final. Ces deux types de restaurations prothétiques nécessitent cependant une maintenance accrue.

Even the best method can be made better by optimization of its single components. This can happen many years later, and now that the general principle of hernia repair is broadly understood all over the world, these sequelae are being noticed more and more. The 5th Suvretta meeting was held in order to define these sequelae, to evaluate the absolute and relative risks they pose, and to discuss the methods of their prevention, diagnosis and treatment. We discussed whether the principal risk was related to technique, material or both. This discussion and the future approach to and evaluation of this problem were the aims of the meeting, working on the premise that even the best method can be made better by optimizing its individual components.

This dramatic change has set the stage for the third Suvretta meeting. All aspects of the mesh world have been discussed in detail by more than 50 international experts during an intense week resulting in an assessment of success and failures. After posing the question, whether meshes have defeated recurrences in the groin, epidemiological clinical data on recurrences were presented and show that this problem still exists. In particular, novel molecular biology-based research results stress the pathophysiological importance of a defective scarring process in these patients with inherent conclusions for future therapies. Regarding the variety existing meshes, there are already more than 100 different mesh devices, a comprehensive review of their chemical and textile properties was presented, with emphasis on their impact on biological responses. However, more than 90% of the participants articulated the need for improved mesh prosthesis, because their characteristic inflammatory and fibrotic foreign body reaction cause minor and major complications, e.g. pain, infections, adhesions, damage of the spermatic cord. The differentiated use of meshes in various procedures was discussed, including groin, incisional, parastomal, diaphragmal and hiatal hernias as well as their use in extended abdominal wall defects or in paediatric or plastic surgery. In summary this book summarizes the most up-to-date knowledge about meshes and hopefully serve as manual for both practical surgeons and scientists involved in the growing world of mesh.
