Book excerpt: The majority of the offending toxicants to be reviewed in this volume were devel oped to help mankind, and it is only with prolonged or widespread application that their adverse effects have been recognized. Conversely, in the case of pre scrip tion drugs, there has been an attempt to identify the adverse effects in advance and incorporate these risks into the decision of approval for human consumption. Unfortunately, for those drugs in which recognized injury occurs only after prolonged use, such appraisals are made in retrospect. Despite this, most renal injury induced by drugs or toxicants can be either prevented by excluding drugs with unacceptable side effects or interrupted by eliminating the offending agent once damage is manifested. The fact that prevention, reversibility, or arrest of renal injury is possible provided a major impetus for this publication. Since no international registry for nephrotoxic injury exists, estimates of incidence must rely on less than ideal sources. Recently I, together with Dr. William Bennett, summarized a survey of the frequency of various categories of nephrologic disease (Porter and Bennett, 1981). Based on this survey, we projected that in nearly one of ten patients seeking nephrologic consultation a nephrotoxic etiology may be involved. Of cases of end-stage renal disease, between 3 and 4% are due to drug nephrotoxicity, according to recent published results(European Dialysis and Transplant Association, 1979). For acute renal failure, antibiotics and contrast agents persist as major offending agents, while for chronic renal failure, analgesics remain a worldwide problem.

Such books have presented extensive tables of interactions and their management. Although of considerable value to clinicians, such publica tions have not, however, been so expressive about the individual mechanisms that underlie these interactions. It is within this sphere of "mechanisms" that this present volume specialises. It deals with mechanisms of in vitro and in vivo, drug-drug, drug food and drug-herbals interactions and those that cause drugs to interfere with diagnostic laboratory tests. We believe that an explanation of the mechanisms of such interactions will enable practitioners to understand more fully the nature of the interactions and thus enable them to manage better their clinical outcome. If mechanisms of interactions are better understood, then it may be pos sible for the researcher to develop meaningful animal/biochemical/tissue cul ture or physicochemical models to which new molecules could be exposed during their development stages. The present position, which largely relies on patients experiencing adverse interactions before they can be established or documented, can hardly be regarded as satisfactory. Book excerpt: Interest and research in urinary enzymology were incited about three decades ago by reports that urinary enzymes are elevated in diseases of the kidney and urinary tract. Of the more than 40 hydrolases, oxidoreductases, transferases, and lyases identified in human and animal urine, only ten or so are being used as diagnostic indicators. Recognition of the quantitative distribution of enzymes in the various anatomical and functional parts of the nephron and advances in our understanding of the handling of proteins by the kidney have made it possible to associate urinary enzyme activity patterns with physiological and pathophysio logical functions of the nephron. Confidence in the diagnostic value of urinary enzymes is not unanimous among clinicians and among scientists. The main reason for the difference in opinion may well be that the variability in data exceeds the variability one is accustomed to in the diagnostic enzymology of blood plasma enzymes. In contrast to plasma enzymes, which are protected by an "enzyme friendly" milieu, enzymes released into the urine encounter an "enzyme hostile" environ ment: no or little protective protein, variable pH, variable volume, variable metabolite and salt concentrations, variable concentrations of enzyme in hibitors. Through advances in methodology some of these factors can now be controlled; standardization of urine collection periods and preanalytical treat ment are as important as optimization of assay methods.
