Many successful outreach programs have been designed and implemented at the local level to reach the 726,000 uninsured children who may qualify for Medi-Cal and the additional 535,000 uninsured children who may qualify for Healthy Families. In the past six months, under the state's leadership, these outreach efforts have been extended to ensure successful enrollment, encourage the appropriate use of health services, and support the retention of coverage over time. This background paper examines the practices of a variety of local programs involved in Medi-Cal and Healthy Families outreach to develop a clearer picture of effective strategies. It is difficult to generalize about local outreach efforts because each project has a unique set of characteristics that has developed in response to the needs of the local community, the history of the agency, and funding sources. Broadly speaking, however, outreach projects can be grouped into three categories based on the primary site of their activities: (1) health facilities, including public and private hospitals and clinics; (2) school-based programs; and (3) community-based organizations (CBOs) such as churches, job-training sites, and cultural organizations.

Book excerpt: Objective: To describe California’s Proposition 10 revenue, First 5’s expenditures on health and dental care, the number of children enrolled in Medi-Cal or Healthy Families programs, and the ACA’s influence on health and dental enrollment in a rural, a suburban, and an urban county between 2005 and 2015. Methods: A descriptive analysis for a rural (Amador), a suburban (Placer), and an urban (Sacramento) county from 2005 to 2015 which included the aggregate data from First 5’s Prop 10 revenue and health and dental expenditures along with the aggregate data from the California Department of Health Care Services on the number of children aged zero to five who were enrolled in Medi-Cal or Healthy Families. Results: The amount of Prop 10 revenue received by Amador, Placer, and Sacramento counties between 2005 and 2015 have declined by approximately 2-28%. First 5’s expenditures on health and dental care varied from county to county and year to year. All three counties spent less than 25% of their Prop 10 revenue on health and 0-40% on dental health. On average, Amador County spent $21 per child, Placer County $4, and Sacramento County $33 for health and dental care. The number of children enrolled in Medi-Cal or Healthy Families gradually increased for all three counties between 2005 and 2015 despite the diminishing Prop 10 revenue or First 5 spending. All three counties were proactive in enrolling children into Medi-Cal or Healthy Families well before the ACA was enacted. Conclusions: Even with declining Prop 10 revenue, California’s First 5 program may have been a buffer for young California children who otherwise would not have had health and dental insurance. Programs such as Medi-Cal, Healthy Families, Heathy Kids, and the ACA have made a difference for California’s low-income, uninsured, and ethnic children’s health and dental care.

Care Without Coverage examines the real consequences for adults who lack health insurance. The study presents findings in the areas of prevention and screening, cancer, chronic illness, hospital-based care, and general health status. The committee looked at the consequences of being uninsured for people suffering from cancer, diabetes, HIV infection and AIDS, heart and kidney disease, mental illness, traumatic injuries, and heart attacks. It focused on the roughly 30 million-one in seven-working-age Americans without health insurance. This group does not include the population over 65 that is covered by Medicare or the nearly 10 million children who are uninsured in this country. The main findings of the report are that working-age Americans without health insurance are more likely to receive too little medical care and receive it too late; be sicker and die sooner; and receive poorer care when they are in the hospital, even for acute situations like a motor vehicle crash.
