Malnutrition manifests itself in many different ways: as poor child growth and development; as individuals who are skin and bone or prone to infection; as those who are carrying too much weight or whose blood contains too much sugar, salt, fat, or cholesterol; or those who are deficient in important vitamins or minerals. Malnutrition and diet are by far the biggest risk factors for the global burden of disease: every country is facing a serious public health challenge from malnutrition. The economic consequences represent losses of 11 percent of gross domestic product (GDP) every year in Africa and Asia, whereas preventing malnutrion delivers $16 in returns on investment for every $1 spent. The world’s countries have agreed on targets for nutrition, but despite some progress in recent years the world is off track to reach those targets. This third stocktaking of the state of the world’s nutrition points to ways to reverse this trend and end all forms of malnutrition by 2030.

Good nutrition provides a vital foundation for human development, central to meeting our full potential. When nutrition status improves, it leads to a host of positive outcomes for individuals and families. Many more children will live to see their fifth birthdays, their growth will be less disrupted, and they will gain in height and weight. They will learn more in school because their brain function is not impaired. As a result of this positive early environment, as adults they will have better jobs and get ill less often. Older adults will age more healthily and live longer.

These developments reverse years of relative neglect for nutrition. Undernutrition is a global challenge with huge social and economic costs. It kills millions of young children annually, stunts growth, erodes child development, reduces the amount of schooling children attain, and increases the likelihood of their being poor as adults, if they survive. Stunting persists through a lifetime and beyond—underweight mothers are more likely to give birth to underweight children, perpetuating undernutrition across generations. Book excerpt: This book’s main hypothesis is that Egypt’s large food subsidy system has been ineffective in reducing undernutrition; in fact, it may have contributed to sustaining and even aggravating both nutrition challenges. For a long time, the subsidy system provided only calorie-rich foods, at very low and constant prices and with quotas much above dietary recommendations. This system has created incentives to consume calorie-overladen and unbalanced diets, increasing the risks of child and maternal overnutrition and, at high subsidy levels, the risk of inadequate child nutrition. Moreover, the large public budget allocated to the food subsidies is unavailable for possibly more nutrition-beneficial spending, such as for child and maternal nutrition-specific interventions. The authors’ findings consistently suggest that—in addition to the well-known economic rationale for reforming the Egyptian food subsidy system—there are strong reasons to reform food subsidies due to nutrition and public health concerns. A fundamental food subsidy reform process has been under way since June 2014. The already-implemented changes can be expected to have reduced some incentives for overconsumption and may have positive dietary effects. However, further major reform efforts are needed to transform the current subsidy system into a key policy instrument in the fight against malnutrition. The findings of this book should be valuable to policy makers, analysts, development partners, and others concerned with improving food security and promoting healthy nutrition in Egypt and other developing countries with large social protection programs.

Book excerpt: Updates for many countries have made it possible to estimate hunger in the world with greater accuracy this year. In particular, newly accessible data enabled the revision of the entire series of undernourishment estimates for China back to 2000, resulting in a substantial downward shift of the series of the number of undernourished in the world. Nevertheless, the revision confirms the trend reported in past editions: the number of people affected by hunger globally has been slowly on the rise since 2014. The report also shows that the burden of malnutrition in all its forms continues to be a challenge. There has been some progress for child stunting, low birthweight and exclusive breastfeeding, but at a pace that is still too slow. Childhood overweight is not improving and adult obesity is on the rise in all regions. The report complements the usual assessment of food security and nutrition with projections of what the world may look like in 2030, if trends of the last decade continue. Projections show that the world is not on track to achieve Zero Hunger by 2030 and, despite some progress, most indicators are also not on track to meet global nutrition targets. The food security and nutritional status of the most vulnerable population groups is likely to deteriorate further due to the health and socio economic impacts of the COVID-19 pandemic. The report puts a spotlight on diet quality as a critical link between food security and nutrition. Meeting SDG 2 targets will only be possible if people have enough food to eat and if what they are eating is nutritious and affordable. The report also introduces new analysis of the cost and affordability of healthy diets around the world, by region and in different development contexts. It presents valuations of the health and climate-change costs associated with current food consumption patterns, as well as the potential cost savings if food consumption patterns were to shift towards healthy diets that include sustainability considerations. The report then concludes with a discussion of the policies and strategies to transform food systems to ensure affordable healthy diets, as part of the required efforts to end both hunger and all forms of malnutrition.

Book excerpt: The 2016 Global Hunger Index (GHI) presents a multidimensional measure of national, regional, and global hunger, focusing on how the world can get to Zero Hunger by 2030. The developing world has made substantial progress in reducing hunger since 2000. The 2016 GHI shows that the level of hunger in developing countries as a group has fallen by 29 percent. Yet this progress has been uneven, and great disparities in hunger continue to exist at the regional, national, and subnational levels. Levels of hunger are still serious or alarming in 50 countries. The highest hunger levels are still found in Africa south of the Sahara and South Asia. Although GHI scores for these two regions have declined over time, the current levels remain close to the alarming category. Africa south of the Sahara has achieved the largest absolute improvement since 2000 and South Asia has also seen a sizable reduction—but the decline in hunger must accelerate in these regions if the world is to achieve Zero Hunger by 2030. The 2016 report, with an essay from United Nations Special Adviser David Nabarro, hails the new paradigm of international development proposed in the United Nations’ 2030 Agenda for Sustainable Development, which envisages Zero Hunger by 2030, as one goal among 17, in a holistic, integrated, and transformative plan for the world. To get to Zero Hunger while leaving no one behind, the 2016 GHI highlights the importance of identifying the regions, countries, and populations that are most vulnerable to hunger and undernutrition so progress can be accelerated there.
