U.S. law prohibits the use of international family planning funds for abortions and requires that all family planning programs supported by AID be voluntary. This report discusses: (1) how much family planning assistance AID provided in fiscal years 1996-1999 and plans to provide in fiscal year 2000, and (2) what procedures have been established to ensure that funds are not being used for prohibited activities. GAO found that AID provided $432 million in assistance in fiscal year 1996 and $385 million in each of fiscal years 1997-1999. AID has established multiple procedures to ensure that family planning funds are not used for prohibited activities, including specifying the restrictions in grants, contracts, and cooperative agreements.

Book excerpt: In January 2006, to better align foreign assistance programs with U.S. foreign policy goals, the Secretary of State appointed the Administrator of the U.S. Agency for International Development (USAID) to serve concurrently as Director of Foreign Assistance (DFA) and gave the DFA authority over all Department of State and USAID foreign assistance funding and programs. The Office of the Director of Foreign Assistance (State/F) was given responsibility for reforming foreign assistance by, among other things, consolidating State and USAID foreign assistance processes. GAO was asked to (1) examine State/F's key efforts to consolidate State and USAID foreign assistance processes and (2) identify any key challenges that affect State/F's reform of foreign assistance. GAO evaluated budget, planning, and other documents and interviewed agency officials in Washington, D.C.; Ethiopia; Haiti; Jordan; Kenya; Peru; and Ukraine. GAO is making seven recommendations to enhance State-USAID organizational transformation, interagency coordination and collaboration, planning processes, and workforce management. State and USAID generally acknowledged or agreed to consider six of the seven recommendations but asserted they had met the one related to regional planning. GAO maintains this recommendation is valid; it is important that an agency clearly capture all relevant programs and activities in its planning processes.

Congress provided about $8.7 billion for the Global Health Programs (GHP) account in fiscal year 2018. In 2017, the President reinstated and expanded a policy, which now requires foreign NGOs to agree that, as a condition of receiving global health assistance, they will not perform or actively promote abortion as a method of family planning or provide financial support during the award term to other foreign NGOs that conduct such activities. The Reagan administration first implemented this policy, known as the Mexico City Policy, in 1984, and subsequent administrations have rescinded and reinstated it. The Mexico City Policy initially applied only to family planning and reproductive health assistance, which received about $560 million of GHP funds in fiscal year 2018. Upon reinstating the policy, the Trump Administration renamed it PLGHA and applied it to all global health assistance to the extent allowable by law. This report identifies (1) global health assistance awards that U.S. agencies determined to be subject to the U.S. government's PLGHA policy requiring foreign NGOs to agree that they would not perform or actively promote abortion as a method of family planning, and (2) planned funding for awards involving NGOs that declined to accept the terms and conditions of this policy.
