Il apparait que le critère le plus pertinent pour distinguer ces deux pathologies se situe dans le domaine de la communication non-verbale. Nous avons basé notre travail sur l'étude de deux cas d'enfants pour lesquels un diagnostic différentiel entre dysphasie et troubles envahissants du développement s'avère ambigu. Nous avons instauré une rééducation afin d'évaluer l'intérêt d'une aide mimogestuelle pour développer le langage et la communication de ces enfants et fournir des éléments en vue de préciser leur diagnostic. Notre étude nous a avant tout permis d'appréhender l'apport d'un moyen de communication gestuel et tant qu'aide augmentative pour les enfants dysphasique. Elle a également montré un intérêt dans l'élaboration et la précision d'un diagnostic différentiel de par l'investissement des enfants vis-à-vis de ce mode de communication et leur évolution au cours de la prise en charge. Nos résultats font cependant apparaître des nuances dans l'efficacité de cet outil selon la gravité des troubles présentés par ces enfants.

Les facteurs d'environnement, y compris l'impact des actions thérapeutiques, sont mal connus. Un rappel sur la clinique des troubles envahissants du développement, les données épidémiologiques et les hypothèses étiologiques introduit notre propos. Puis, en s'appuyant sur une revue de la littérature, ce travail de thèse met en évidence la diversité clinique des tableaux de troubles envahissants du développement, l'hétérogénéité de leurs devenirs et propose de discuter des facteurs permettant de prédire l'évolution. Enfin, trois situations cliniques illustrent les données théoriques.
