Available in PDF, EPUB and Kindle. Book excerpt: Foundations of Supply-Side Economics: Theory and Evidence is composed of a series of papers containing both theoretical and empirical analyses of a set of issues in government fiscal policy. The type of analysis employed in the book is standard neoclassical economics, and this analysis is used to study the macroeconomic incentive effects of taxation. The book contains contributions that cover the analysis of the effects of taxes imposed purely for generating revenues; the process of capital formation; and an attempt to integrate supply-side analysis into a traditional macroeconomic framework. Reports on the empirical evidence on taxation and economic activity and the estimation of a small macroeconomic model of the United States for the postwar period; description of a method of calculating effective marginal tax rates on factor incomes using available U.S. data; and the estimation of the effect of fiscal policy on private investment in plant and equipment are presented as well. Economists will find the book highly insightful.

Available in PDF, EPUB and Kindle. Book excerpt: The authors explore a new framework for evaluating economic development projects. This framework is based on a job-chain approach. Each new job created by an economic development incentive is filled by an employee who leaves behind another job. In turn, that job may be filled by someone who leaves behind their old job, etc. Such job chains end when an unemployedworker, someone not previously in the labor force, or an in-migrant to the labor market takes a vacancy. Job chains are the mechanism for observing and measuring "trickle down". The job trains model developed in this book presents new insights into local economic development evaluation and strategy.

Available in PDF, EPUB and Kindle. Book excerpt: For the past several decades, politicians and economists thought that high levels of inequality were good for the economy. But because America’s middle class is now so weak, the US economy suffers from the kinds of problems that plague less-developed countries. As Hollowed Out explains, to have strong, sustainable growth, the economy needs to work for everyone and expand from the middle out. This new thinking has the potential to supplant trickle-down economics—the theory that was so wrong about inequality and our economy—and shape economic policymaking for generations.

Book excerpt: From inside the book: “Since 1980, the economy has been growing, and productivity has been growing, but trickle-down values—that we, the American people promote, pursuant to the Republican Party’s conservative ideology—have rigged the economy to continuously upwardly redistribute those revenues attributable to our increased productivity, yielding a productivity/wage disconnect, resulting in increased concentration of income and wealth at the top, in corporations and among older Americans (beneficiaries of income from Social Security, pensions and investments and continuing income due to delaying retirement), and the lowest percentage of GDP attributable to wages and highest attributable to profits since World War II. But trickle-down has not only distorted our economic thought; it has also distorted our political thought, our sociology and our concept of the rule of law. The result has been that the trickle-down policies promoted by the Republican Party are undermining our economy, democracy, institutions and health.” For further discussion contact author at [email protected].

Available in PDF, EPUB and Kindle. Book excerpt: Pediatric headache is the most common neurological complaint that presents to the general pediatrician, urgent care and emergency rooms, and to pediatric neurology. Despite how common headaches are in the pediatric population and how many children continue to suffer from their headaches on a daily basis, there remains a tremendous disconnect for those tasked with being the first-line providers (general pediatric providers). This stems from the fact that headache patients can be so variable and challenging, with numerous comorbid concerns, all the while requiring an individualized treatment approach without a cure. The purpose of this proposed book is to help foster a better understanding of how to approach, assess, and ultimately treat pediatric headache for the general pediatric provider in an attempt to make what once was a daunting, overwhelming complaint from patients and their families into something each general pediatric provider can conquer with confidence. With the aid of experts in the field of pediatric headache, this book will begin by providing the foundation as to what types of headaches, primary and secondary as well as their precursors, are commonly seen in the pediatric population. From there, the focus will turn to when should the provider worry about a patient who presents with headaches. The clinic visit -- from the history to the “red flags” to the pertinent exam -- will be reviewed in depth to help distinguish between primary and secondary headaches as well as who warrants an evaluation. Additionally, comorbid concerns will be covered as these may have direct impact on the evaluation and subsequent treatments offered. The next section will delve into the treatment of pediatric headache with a comprehensive approach covering everything from lifestyle to complementary and alternative therapies/treatments, to medications, both acute and prophylactic. Other more specialized treatment options such as procedures and devices will also be discussed albeit these options are more likely to be offered by a specialist rather than a general provider. A subsequent section will highlight the unique challenges for the older adolescent patients as they transition into young adulthood. The closing section will highlight the future of pediatric headache.
