It describes structural and functional properties of the cerebral circulation that are unique to the brain, an organ with high metabolic demands and the need for tight water and ion homeostasis. Autoregulation is pronounced in the brain, with myogenic, metabolic and neurogenic mechanisms contributing to maintain relatively constant blood flow during both increases and decreases in pressure. In addition, unlike peripheral organs where the majority of vascular resistance resides in small arteries and arterioles, large extracranial and intracranial arteries contribute significantly to vascular resistance in the brain. The prominent role of large arteries in cerebrovascular resistance helps maintain blood flow and protect downstream vessels during changes in perfusion pressure. The cerebral endothelium is also unique in that its barrier properties are in some way more like epithelium than endothelium in the periphery. The cerebral endothelium, known as the blood-brain barrier, has specialized tight junctions that do not allow ions to pass freely and has very low hydraulic conductivity and transcellular transport. This special configuration modifies Starling's forces in the brain microcirculation such that ions retained in the vascular lumen oppose water movement due to hydrostatic pressure. Tight water regulation is necessary in the brain because it has limited capacity for expansion within the skull. Increased intracranial pressure due to vasogenic edema can cause severe neurologic complications and death.

Book excerpt: A vascular system consists of a supplying arterial and a draining venous part which are connected by a terminal vascular network. The arterial segment can be characterized according to the structural features of the vessel wall. However, it is sometimes diffi cult to distinguish the capillary from the postcapillary vessels on the basis of structural features alone. On the other hand, physiologic qualities such as permeability can hard ly be associated with an equivalent histologic pattern of the vessel wall (lllig 1961; Rhodin 1967, 1968; Hauck 1971; Westergaard 1974). A defmition of a vascular seg ment based on biologic significance should combine morphological and functional qualities of the vessel walls. During the ontogeny of the mammalian organism a variety of vascular patterns (e. g., distribution of arteries and veins, arrangement of the capillaries) has been formed typical of each organ (Wolff et al. 1975; Baez 1977). The capillaries connect the feed ing arterioles and the collecting venules in two different ways according to the branch ing pattern of the terminal vessels (Hauck 1975, Wolff et al., 1975). The arterioles and venules are directly connected by capillary segments. Consequently a terminal vessel called arteriovenous (a-v) capillary results, or a closely meshed capillary network is de veloped which connects arterioles and venules by a variable number of small capillary branches arranged parallel to the preexisting a-v capillary.

It is a central feature of numerous post-embryonic processes, including tissue and organ growth and regeneration. It is also part of the pathology of tumour formation and certain inflammatory conditions. In recent years, comprehension of the molecular genetics of blood vessel formation has progressed enormously and studies in vertebrate model systems, especially the mouse and the zebrafish, have identified a common set of molecules and processes that are conserved throughout vertebrate embryogenesis while, in addition, highlighting aspects that may differ between different animal groups. The discovery in the past decade of the crucial role of new blood vessel formation for the development of cancers has generated great interest in angiogenesis (the formation of new blood vessels from pre-existing ones), with its major implications for potential cancer-control strategies. In addition, there are numerous situations where therapeutic treatments either require or would be assisted by vasculogenesis (the de novo formation of blood vessels). In particular, post-stroke therapies could include treatments that stimulate neovascularization of the affected tissues. The development of such treatments, however, requires thoroughly understanding the developmental properties of endothelial cells and the basic biology of blood vessel formation. While there are many books on angiogenesis, this unique book focuses on exactly this basic biology and explores blood vessel formation in connection with tissue development in a range of animal models. It includes detailed discussions of relevant cell biology, genetics and embryogenesis of blood vessel formation and presents insights into the cross-talk between developing blood vessels and other tissues. Most of us now are too young to remember the unpleasant ring of a polemic between those who produced "hair-splitting" parcellations of the cortex (to paraphrase one of O. Vogt's favourite expressions) and those who saw the cortex as a homogeneous matrix sus taining the reverberations of EEG waves (to paraphrase Bailey and von Bonin). One camp accused the other of producing bogus preparations with a paint brush, and the other way around the accusation was that of poor eye-sight. Artefacts of various sorts were invoked to explain the opponent's error, ranging from perceptual effects (Mach bands crispening the areal borders) to poor fixation supposedly due to perfusion too soon (!) after death. I have heard most of this directly from the protagonists' mouths. The polemic was not resolved but it has mellowed with age and ultimately faded out. I was relieved to see that Professor Braak elegantly avoids dis cussion of an extrememist tenet, that of "hair-sharp" areal boundaries, which makes little sense in developmental biology and is irrelevant to neurophysiology. It was actually detrimental to cortical neuroanatomy, since its negation led to the idea that structurally distinct areas are not at all existent. Yet, nobody would deny the reality of five fingers on one hand even if the detailed assignment of every epidermal cell to one finger or another is obviously impossible.
