Book excerpt: The primary physiological role of estrogens is the development of secondary sexual characteristics including development and function of the normal breast, reproductive system, bone homeostasis, cognitive functions and cardiovascular system. Estrogen has also been implicated as a major player in the progression of normal breast epithelial tissue to a carcinoma. The role of estrogens in breast cancer has been studied extensively and mainly attributed to the transcriptional activation of growth genes through the estrogen receptor. Along with activation of genes, estrogen is also responsible for repression of many genes. Anti-estrogens antagonize both gene activation and gene repression by estrogen. However, the significance and physiological relevance of gene repression by estrogen is poorly understood. mRNA profiling of MCF-7 breast cancer cells combined with a detailed gene ontology analysis revealed that the genes repressed by estrogen are mostly involved in tumor progression including invasion, drug resistance, angiogenesis and immune evasion. This study looks at the mechanism and impact of estrogen in repressing these tumor progression genes in breast cancer cells; we found that estrogen suppresses the invasiveness of breast cancer cells in a manner that is antagonized by anti-estrogens and is independent of HER2 status. These studies have implications in understanding the incidence of invasive breast cancer following hormone replacement therapy and long term anti-estrogen treatment and may also aid in the development of superior drugs for adjuvant treatment in breast cancer. Variability in response to chemotherapeutic drugs among patients is a longstanding issue in the treatment of several cancers. Likewise, in advanced lung cancer, patient response to the recently introduced drug, pemetrexed is variable. Pemetrexed is an antifolate approved for the first-line treatment of advanced non-squamous non-small cell lung cancer. Dexamethasone was added to the treatment regimen with pemetrexed to alleviate the serious side effect of severe skin rash observed in many patients. Dexamethasone is administered to patients the day before, the day of and the day after pemetrexed chemotherapy. We show that treatment of non-squamous non-small cell lung cancer cell line models with dexamethasone causes a reduction in the S-phase fraction of cells along with a decrease in the expression of thymidylate synthase and dihydrofolate reductase, which are primary and secondary targets of pemetrexed, respectively. As a consequence of these effects of dexamethasone pemetrexed cytotoxicity is attenuated. The response to dexamethasone is variable among different cell line models. Our correlative and functional studies demonstrate that the variability in pemetrexed sensitivity is causally related to variability in the expression of the glucocorticoid receptor isoform alpha, i.e. cells with relatively lower expression of the receptor fail to respond to dexamethasone and hence are sensitive to pemetrexed. These results could help to predict response to pemetrexed therapy leading to the development of individualized treatment strategies.

Book excerpt: Women with estrogen receptor positive breast tumors respond to adjuvant hormonal therapy with an enhancement of disease free survival and overall survival. A substantial fraction of tumors are dependent upon estrogen for growth, and antiestrogen has been shown to be effective for the treatment of breast cancer. The precise mechanism of estrogen and anti-estrogen action in breast tumors still remain unclear. We postulated that estrogen and anti-estrogen act by activating the function of the endogenous estrogen receptor. Using rat mammary gland as the model, we examined the function of ER in response to estrogen and antiestrogen. Our data suggested that estrogen and anti-estrogen are able to activate and regulate the function of estrogen receptor in a cell context dependent fashion. We conclude that estrogen and anti-estrogen may exert their effects on the growth of target tissues, at least in part, by increasing the functional activity of the endogenous estrogen receptor. Abstracts for the research done in this funding period were submitted to the Annual Endocrine Meeting in June, 1997 and Era of Hope DOD BCRP Meeting in October, 1997 for presentation.

The response to this volume was superb, indicating a tremendous interest in basic growth regulatory mechanisms governing breast cancer and controlling its malignant progres sion. In the two years since its publication, much new and exciting in formation has been published and the full interplay of regulatory mechanisms is now beginning to emerge. We have divided this book into four sections that we hope will unify important concepts and help to crystallize areas of consensus and/or disagreement among a diverse group of basic and clinical scientists working on the disease. The first section is devoted to studies on oncogenes, antioncogenes, proliferation, and tumor prognosis. The first chapter, by Sunderland and McGuire, introduces the characteristics of breast cancer as studied by patho logists to establish prognostic outcome. Of particular interest is a new proto oncogene called HER-2 (or neu), which is rapidly becoming accepted as a valuable new tumor marker of poor prognosis. The second chapter, by Lee Bookstein and Lee, introduces the best known antioncogene, the retinoblas toma antioncogene, whose expression is sometimes lost in breast cancer. Malignant progression appears to be influenced by the balance of proto oncogene and antioncogene expression.

One of the more recent initiatives of ESO has been the institution of permanent study groups, also called task forces, where a limited number of leading experts are invited to meet once a year with the aim of defining the state of the art and possibly reaching a consensus on future developments in specific fields of oncology.

It now appears that the mechan sim of action of the four major classes of steroid hormones is qualitatively similar. Research during the past decade has demonstrated steroid hormone receptors in a variety of normal and neoplastic tissues. Receptor-containing neoplasms have been shown to be hormone-dependent and undergo regression when treated with hormone antagonists. Natural and synthetic steroids also have been employed for many years to successfully treat various types of cancer, for example: estrogens, androgens and progestagens for breast cancer; estrogens and progestagens for prostatic carcinoma; progestagens for endometrial carcinoma; and corticoids for leu kemias. All of these neoplasms have now been found to possess receptors for the steroids empirically used in their treatment.

Book excerpt: The role of steroid receptor coactivator-1 (SRC-1) in breast cancer is primarily a function of (i) the expression patterns of nuclear receptors (NRs) and SRC family members in the tumor, and (ii) their inherent interaction affinities. An outstanding question is the extent to which SRC/NR pathways in breast cancer are uncoupled by administration of selective estrogen receptor modulator (SERM) regimens such as 4-hydroxytamoxifen. An appreciation of the role of SRC-1 and other SRCs in the selective recruitment of NRs during the development of breast cancer is a prerequisite for the design of novel SRMs for treatment of breast cancer. We used BlAcore analysis to monitor interactions between SRCs and His-tagged NRs. Distinct affinities were noted for different NR-coactivator interactions, indicating an interaction preference spectrum for estrogen receptor-alpha (ERalpha) of SRC-3>SRC-1>SRC-2. The interaction of ERalpha with SRC family members was differentially influenced by different SERMs. In all cases, the interaction between ERalpha and SRCs was promoted by 17beta-estradiol and inhibited by 4-hydroxytamoxifen, raloxifene and ICI 182, 780. The interaction kinetics of SRC family members with liganded NRs are consistent with a bipartite model involving a transitional intermediate. By providing accurate measurements of the modulation by SRMs of the interaction between NRs and SRC family members, we have created a model system within which rational design of SERMs for breast cancer treatment can be evaluated in a functional context.
