Book excerpt: Since the private health sector is an important, and often dominant, provider of health services in Sub-Saharan Africa, it is the job of governments as the stewards of the health system to engage with it. Increasing the contributions that the existing private health sector is making to public health is an important, but often neglected, element of meeting the daunting health-related challenges facing African nations. This Report presents newly collected data on how and how effectively each country in the Africa region is engaging the respective private health sectors; and how the engagement compares across the region. While the approach taken by governments varies greatly between countries, there is much room for improvement in the Africa region overall to engage more effectively and room for exchange of ideas and good practices on how to do so. Improved solutions on the policy/regulatory side should be supported by effective organization of the private sector itself and by adjustments in donor programs that take the dynamics of the private health sector better into account.

Book excerpt: This book is a timely response to the current U.S. crisis in public funding. Offering a new philosophy of public service that defies the old categories of conservative and liberal, this practical book shows how the problem-solving abilities and profit-making discipline of the business community can make it a productive alternative for meeting public needs. Using education as an example of what should be a high domestic priority, the authors argue that business should recognize that it has a major stake in the quality of the product of our schools and should provide support. The book delineates other areas of national concern that merit the attention of American business. It concludes with an insightful discussion of how business involvement might be reinforced by incentive systems.

Book excerpt: The anthrax incidents following the 9/11 terrorist attacks put the spotlight on the nation's public health agencies, placing it under an unprecedented scrutiny that added new dimensions to the complex issues considered in this report. The Future of the Public's Health in the 21st Century reaffirms the vision of Healthy People 2010, and outlines a systems approach to assuring the nation's health in practice, research, and policy. This approach focuses on joining the unique resources and perspectives of diverse sectors and entities and challenges these groups to work in a concerted, strategic way to promote and protect the public's health. Focusing on diverse partnerships as the framework for public health, the book discusses: The need for a shift from an individual to a population-based approach in practice, research, policy, and community engagement. The status of the governmental public health infrastructure and what needs to be improved, including its interface with the health care delivery system. The roles nongovernment actors, such as academia, business, local communities and the media can play in creating a healthy nation. Providing an accessible analysis, this book will be important to public health policy-makers and practitioners, business and community leaders, health advocates, educators and journalists.

Book excerpt: "The Nation has lost sight of its public health goals and has allowed the system of public health to fall into 'disarray'," from The Future of Public Health. This startling book contains proposals for ensuring that public health service programs are efficient and effective enough to deal not only with the topics of today, but also with those of tomorrow. In addition, the authors make recommendations for core functions in public health assessment, policy development, and service assurances, and identify the level of government--federal, state, and local--at which these functions would best be handled.

Available in PDF, EPUB and Kindle. Book excerpt: When government services are turned over to profit-making corporations will a gentler, kinder America result? Significantly, when the issue of privatization of government arises, this question usually takes a back seat to the more narrowly focused one of: can the taxpayer pay less for the same service without diminishing existing quality? . . . This book compiles experiences of practitioners and corporations with generally positive experiences in contracting for services between public and private entities. The essays by professors raise serious questions that all societies will face in creating an appropriate mix so that their citizens enjoy gentler, kinder lives. Growth and Change For many cities and states faced with reductions in federal revenue sharing and little political support for increased taxes, privatization of the public sector seems the only viable alternative. In an effort to maintain existing service levels with decreased funds, many governments have turned to alternative service delivery approaches through such mechanisms as contracting, franchises, subsidies, and voucher plans. In this volume, Finley and a distinguished group of contributors from city governments, corporations, and universities, offer a comprehensive overview of privatization in practice. Their papers address privatization in a number of areas including transportation, fire protection, health care, and environmental services as well as the legal aspects of privatization. An especially stimulating chapter describes major European efforts at privatization. Divided into three major sections, the book begins with introductory chapters that examine the dimensions of public services, evaluate recent changes in the public-private mix, and explore alternative delivery methods. Part Two focuses on alternate services experiences of governments and companies, including topics on environmental infrastructure alternatives, alternative means of highway development, private fire contractor operations, and alternative health care delivery. The final section addresses both constraints to privatization and the opportunities presented by various alternative delivery mechanisms. Here the contributors address the legal liabilities of governments involved in contracting out, the financial responsibilities of the contracting entities, and government financing of facilities through bonds. A chapter by the editor recommends a process by which business persons can begin to successfully compete tith public deliverers, while the final chapter offers new insights into the ways in which various European countries have handled the issue of privatization. Policymakers and public sector executives will find these essays enlightening and provocative.

Available in PDF, EPUB and Kindle. Book excerpt: This book presents comparative analyses and accounts of the institutional changes that have occurred to the local level delivery of public utilities and personal social services in countries across Europe. Guided by a common conceptual frame and written by leading country experts, the book pursues a “developmental” approach to consider how the public/municipal sector-centred institutionalization of service delivery (climaxing in the 1970s) developed through its New Public Management-inspired and European Union market liberalization-driven restructuring of the 1980s and early 1990s. The book also discusses the most recent phase since the late 1990s, which has been marked by further marketization and privatization of service delivery on the one hand, and some return to public sector provision (“remunicipalization”) on the other. By comprising some 20 European countries, including Central East European “transformation” countries as well as the “sovereign debt”-stricken countries of Southern Europe, the chapters of this volume cover a much broader cross section of countries than other recent publications on the same subject.
