Book excerpt: Excerpt from The Office Treatment of Rectal Diseases; Explained and Simplified: Being an Exposition of the Treatment of All Those Disease, Both Medical and Surgical, of the Rectum, Anus, and Sigmoid Flexure, the Cure Which May Be Accomplished Without Surgical Anaesthesia It is not intended that this book shall take the place of larger works upon the same subject, but that it may be an addition to them, and cover many points that have been omitted. It is the result of many years practical work as a specialist and teacher along this line, and is a description of methods tried and found valuable. Diagnosis is discussed very briefly, merely to bring the malady prominently before the reader. Etiology, pathology, anatomy, vague theories, and major operations are omitted, and the work devoted merely to treatment, which is discussed in the most terse and practical manner possible. I have consulted freely the literature of the subject, and have made use of such portions as seemed suitable for a work of this kind. As surgical procedures are an evolution, and the result of many minds working along the same lines, it is impossible to give proper credit to any one person; for this reason names of different men who have written upon this subject are not mentioned except in rare instances. Book excerpt: Excerpt from Observations on Some of the Principal Diseases of the Rectum and Anus: Particularly Stricture of the Rectum, the Hemorrhoidal Excrescence, and the Fistula in Ano On the subject of the removal of the he morrhoidal excrescence, the result of my ex perience has been somewhat different from what I had been led to expect by the writers who have treated of this affection. I have not always found it a secure and safe operation. Other surgeons also have met with instances of great danger, and even death, produced by the free extirpation of these tumors. I have endeavoured to suggest some other modes of treating the disease, or so to modify the operation, as to render it less serious and less objectionable. Book excerpt: Anorectal Disorders: Diagnosis and Nonsurgical Treatments incorporates the most recent major advances in the understanding of anal and rectal anatomy to provide clinical evidence of care for those suffering from anorectal disorders. By condensing basic and practical aspects in the knowledge of anorectal disorders, this book provides a comprehensive review of the anatomy and physiology of the anorectal region. It includes new technology that expands and modifies current perceptions about the functional anatomy of the anorectal region and ultimately impacts how we conceive muscle and nerve structures functioning together to maintain fecal continence and properly execute the process of defecation. Perfect for medical researchers, gastroenterologists, general practitioners, internal medicine doctors and general surgeons, this title will provide diagnosis and treatment options for all patients suffering from anorectal disorders. Identifies frequent, but not well understood, anorectal disorders, including ulcers, constipation, dyssynergic defecation, tumors and malformations Condenses the basic and practical aspects of anorectal disorders for better treatment of patients Includes clinical cases that summarize chapter content in a clinical context

It is also a paradigm for multimodal management, as the combination of surgery, chemotherapy and radiotherapy is often necessary to achieve the optimal outcome. Recently, international experts met in Heidelberg, Germany to discuss the latest developments in the management of rectal cancer, including the anatomic and pathologic basis, staging tools, surgical concepts including fast-track surgery and laparoscopic resection, functional outcome after surgery and the role of radio- and chemotherapy. This monograph summarizes this meeting and gives an extensive overview of the current concepts in management of rectal cancer.

Book excerpt: Three distinct types of contractions perform colonic motility functions. Rhythmic phasic contractions (RPCs) cause slow net distal propulsion with extensive mixing/turning over. Infrequently occurring giant migrating contractions (GMCs) produce mass movements. Tonic contractions aid RPCs in their motor function. The spatiotemporal patterns of these contractions differ markedly. The amplitude and distance of propagation of a GMC are several-fold larger than those of an RPC. The enteric neurons and smooth muscle cells are the core regulators of all three types of contractions. The regulation of contractions by these mechanisms is modifiable by extrinsic factors: CNS, autonomic neurons, hormones, inflammatory mediators, and stress mediators. Only the GMCs produce descending inhibition, which accommodates the large bolus being propelled without increasing muscle tone. The strong compression of the colon wall generates afferent signals that are below nociceptive threshold in healthy subjects. However, these signals become nociceptive; if the amplitudes of GMCs increase, afferent nerves become hypersensitive, or descending inhibition is impaired. The GMCs also provide the force for rapid propulsion of feces and descending inhibition to relax the internal anal sphincter during defecation. The dysregulation of GMCs is a major factor in colonic motility disorders: irritable bowel syndrome (IBS), inflammatory bowel disease (IBD), and diverticular disease (DD). Frequent mass movements by GMCs cause diarrhea in diarrhea predominant IBS, IBD, and DD, while a decrease in the frequency of GMCs causes constipation. The GMCs generate the afferent signals for intermittent short-lived episodes of abdominal cramping in these disorders. Epigenetic dysregulation due to adverse events in early life is one of the major factors in generating the symptoms of IBS in adulthood.
