Book excerpt: This first-ever Surgeon General's Report on bone health and osteoporosis illustrates the large burden that bone disease places on our Nation and its citizens. Like other chronic diseases that disproportionately affect the elderly, the prevalence of bone disease and fractures is projected to increase markedly as the population ages. If these predictions come true, bone disease and fractures will have a tremendous negative impact on the future well-being of Americans. But as this report makes clear, they need not come true: by working together we can change the picture of aging in America. Osteoporosis, fractures, and other chronic diseases no longer should be thought of as an inevitable part of growing old. By focusing on prevention and lifestyle changes, including physical activity and nutrition, as well as early diagnosis and appropriate treatment, Americans can avoid much of the damaging impact of bone disease and other chronic diseases. This Surgeon General's Report brings together for the first time the scientific evidence related to the prevention, assessment, diagnosis, and treatment of bone disease. More importantly, it provides a framework for moving forward. The report will be another effective tool in educating Americans about how they can promote bone health throughout their lives. This first-ever Surgeon General's Report on bone health and osteoporosis provides much needed information on bone health, an often overlooked aspect of physical health. This report follows in the tradition of previous Surgeon Generals' reports by identifying the relevant scientific data, rigorously evaluating and summarizing the evidence, and determining conclusions.

Book excerpt: There are a number of therapies and treatments available for the prevention of fragility fractures in people thought to be at risk, or to prevent further fractures in those who have already had one or more fragility fractures. However, identifying who will benefit from preventative treatment is imprecise. A number of risk assessment tools are available to predict fracture incidence over a period of time, and these may be used to aid decision making. These tools are limited in that they may not include all risk factors, or may lack details of some risk factors. Tools are dependent on the accuracy of the epidemiological data used to derive them and tools validated in other populations may not apply to the UK. Two tools, FRAX and QFracture, are available for use in the UK. It is not clear whether these tools are equally accurate and whether choice of tool should depend on circumstances. This short clinical guideline aims to provide guidance on the selection and use of risk assessment tools in the care of people who may be at risk of fragility fractures in all settings in which NHS care is received.

Book excerpt: Resistance training (RT) has been shown to improve muscular strength and endurance, body composition, and other health/fitness factors in middle-aged to older adults. It is also associated with maintenance of bone mineral density (BMD) - critical to minimizing fracture risk and associated healthcare costs that accompany advancing age. bioDensity(TM) training is a low-volume high-intensity RT approach that involves four exercises performed for 5-seconds each once per week. The four exercises engage most major muscle groups, are performed at voluntary-maximal intensity over a limited-range and are intended to elicit multiple body weight loading of the skeleton. PURPOSE: To determine whether a 24 week bioDensity(TM) intervention improves BMD and health/fitness factors in osteopenic/osteoporotic adults. METHODS: 15 participants are currently training with bioDensity(TM) 1x/week; nine females (59.8±5.0 yrs; 24.0±4.4 kg/m2) have completed 24-week testing. Baseline and 24-week testing includes BMD and body composition (DEXA), Y-balance test, Senior Fitness Test, floor-to-stand test, and muscular force production measurements. RESULTS: Findings at 24-weeks (N=9) show no change in BMD or body composition. Chest press, leg press and vertical lift strength increased 58%, 73%, and 54%, respectively (P

Available in PDF, EPUB and Kindle. Book excerpt: This edited work presents the most current evidence on osteosarcopenia from bench to bedside, which is expected to facilitate the understanding of this syndrome and to develop preventive and therapeutic strategies. With our aging population, chronic diseases such as osteoporosis and sarcopenia are becoming highly prevalent. Fortunately, our understanding of the bone and muscle interactions has increased in recent years. This has allowed to the coining of the term osteosarcopenia to describe a syndrome in which these two diseases overlap. This overlap between osteoporosis and sarcopenia has major negative effects not only on our older adults but also on health systems worldwide. Readers will find a highly translational approach that starts with a summary of recent discoveries on stem cells biology, muscle and bone interactions – including the role of local bone and muscle fat – followed by comprehensive reviews on myokines (i.e. myostatin), osteokines (i.e. osteocalcin) and adipokines (i.e. interleukins) as major players and determinants of bone and muscle loss with aging. In addition, the role of sex steroids (i.e. estrogens, androgens), and calciotropic hormones (i.e. parathyroid hormone, vitamin D) in the pathogenesis of this syndrome is also reviewed. Moreover, using practical diagnostic and therapeutic tips, this book summarizes the clinical characteristics of osteosarcopenic patients thus facilitating the diagnosis and treatment of this syndrome in clinical practice. Finally, the book presents the case for the Falls and Fractures Clinic as the optimal model of care for this syndrome, aimed to avoid fragmentation and optimize osteosarcopenia care, and simultaneously prevent falls and fractures in older persons. This book offers relevant information on the mechanisms of osteosarcopenia, and a practical guide on how to identify and treat this geriatric syndrome and its adverse outcomes, which are dramatically affecting our aging population. The work is written by leaders in the field and is especially suited not only to any researcher in the musculoskeletal arena but also to medical specialists and allied health professionals involved in the care of older persons.

Book excerpt: INTRODUCTION: Weight-bearing exercise may positively affect bone via muscle contractions, impact forces, and hormonal changes; however, the importance of these factors remains controversial. PURPOSE: To determine if 12-months of resistance training (RT) or plyometric (PLY) exercise beneficially impacts bone mineral density (BMD) in active, osteopenic men, and to examine the potential osteogenic mechanisms. METHODS: Participants were randomized to 12-months of RT or PLY. LBM and BMD of the whole body, weight-bearing, and non-weight-bearing sites were measured at 0- and 12-months. Testosterone, free testosterone, and estradiol concentrations were assessed using ELISA kits. RESULTS: Whole body and leg BMD increased with no differences between RT and PLY. Hip BMD increased in the RT group. The relationship between the percent change in whole body LBM and the percent change in leg BMD was positive in the PLY group and negative in the RT group. The percent change in whole body LBM was positively correlated with the percent change in hip BMD in the PLY group. The percent change in arm LBM was positively correlated with the percent change in arm BMD in the RT group. The percent change in testosterone and estradiol were negatively correlated with the percent change in hip BMD. DISCUSSION: In conclusion, muscle contraction forces and impact forces may be potential mechanisms for osteogenesis in osteopenic men.

Female soldiers sustain twice the number of stress fractures compared to males. Exercise interventions for women are needed to promote military readiness in ways that enhance bone strength and reduce stress fractures. This study, currently in progress, is investigating the effects of 30 weeks of concentric Vs eccentric isokinetic resistance training on bone stiffness (mechanical impedance), bone quality (mineral density and content), and bone cell activity (biomarkers of turnover). Female volunteers (N = 120), 18-26 years of age have been categorized into normal bone density (NBD, N = 60) and low bone density (LBD, N = 60) subgroups, based on whole body bone density measurements. Subjects subsequently have been randomly assigned within NBD and LBD subgroups for either concentric (n = 30 LBD, n = 30 NBD) and eccentric (N = 30 LBD, N = 30 NBD) isokinetic exercise training. Exercises are performed using non-dominant arms and legs only, leaving dominant limbs as self-controls Results of this study will enhance understandings of effects of specialized isokinetic resistance exercise training on bone adaptations among young adult females. This work has important implications for reducing stress fractures arising from combat- readiness activities in women and their risks for future osteoporosis.

Book excerpt: Since the publication of the first edition, the U.S. Surgeon General released the first-ever report on bone health and osteoporosis in October 2004. This report focuses even more attention on the devastating impact osteoporosis has on millions of lives. According to the National Osteoporosis Foundation, 2 million American men have osteoporosis, and another 12 million are at risk for this disease. Yet despite the large number of men affected, the lack of awareness by doctors and their patients puts men at a higher risk that the condition may go undiagnosed and untreated. It is estimated that one-fifth to one-third of all hip fractures occur in men. This second edition brings on board John Bilezikian and Dirk Vanderschueren as editors with Eric Orwoll. The table of contents is more than doubling with 58 planned chapters. The format is larger – 8.5 x 11. This edition of Osteoporosis in Men brings together even more eminent investigators and clinicians to interpret developments in this growing field, and describe state-of-the-art research as well as practical approaches to diagnosis, prevention and therapy. - Brings together more eminent investigators and clinicians to interpret developments in this growing field - Describes state-of-the-art research as well as practical approaches to diagnosis, prevention and therapy - There is no book on the market that covers osteoporosis in men as comprehensively as this book

Available in PDF, EPUB and Kindle. Book excerpt: A meta-analysis was done to determine what effect exercise has on the bone mineral density (BMD) of the lumbar spine, proximal femur, and distal radius in postmenopausal women, utilizing data from 18 studies. Treatment effects (TE) were calculated for each of the skeletal sites, aerobic and strength training, as well as methods of BMD measurement (dual energy x-ray absorptiometry and dual photon absorptiometry). The resulting TEs found exercise, in general, to significantly increase (p c 0.05) the BMD of the lumbar spine (0.73%), and the proximal femur (0.35%); however, the distal radius showed a decrease ( -0.91%) in BMD. When looking for the effect of aerobic versus strength training, significant (p c 0.05) increases in BMD were found in the spine and femur. Since positive changes were not seen in all sites, the benefits of exercise may be specific to the skeletal sites which the working muscles are attached. When the % change for exercise and control groups were analyzed separately, the exercise group increased 2.31% at the lumbar spine and 0.61% at the proximal femur. However, the exercise group decreased -2.21% at the radius. The control group decreased at the lumbar spine -1.09%, and at the femur -1.1 I%, however, increased at the radius 0.36%. The BMD % changes were significantly larger for the spine and femur, therefore, vigorous exercise along with resistance training may help slow the decrease in BMD as one ages.
