Book excerpt: The purpose of this study was to use music to distract and relax pediatric patients during medical procedures. Procedural support via live music therapy interventions included a variety of activities such as music paired with breathing exercises, counting to music, developing sequence in song, and manipulation of musical and play objects. Because invasive and non-invasive are two radically different types of procedures, two separate experimental designs were implemented and administered to a total of 40 pediatric patients between the ages of 5 and 12, in the main area and the emergency department of a southeastern regional hospital in North Florida. Invasive procedures included intravenous starts, finger pricks, incision and draining, suturing, and removal of foreign material in the skin. Non-invasive procedures included X-rays, computed tomography scans, and breathing treatments. All subjects were randomly assigned to a control or experimental group. Demographics were collected on all pediatric patients; this information included gender, age, procedure, and length of procedure. For self-report, The Pain Intensity and Assessment Tool, modeled after the Wong-Baker FACES Pain Rating Scale, was administered pre and post treatment to measure each child's level of pain, and modified to measure each child's level of anxiety. Results indicated no significant difference in control versus experimental groups receiving either invasive or non-invasive procedures in the main hospital or emergency centers. Mean and standard deviation scores, however, showed a decrease in anxiety and pain levels when comparing pre to post procedures. A behavioral checklist was used post-intervention to record the number of times an anxiety-related behavior occurred. Regardless of procedure, groups receiving music intervention displayed fewer aversive behaviors than groups that did not. In addition, patient satisfaction with MT assistance during the medial procedures was 100%.

However, this procedure can cause more anxiety and pain for individuals involved in the Phase 1 clinical trial "to Assess the Feasibility, Safety, and Tolerability of Autologous Mesenchymal Stem Cell Transplantation in Patients With Relapsing Forms of Multiple Sclerosis" (NIH, 2013). Generally a bone marrow aspiration or biopsy requires a bone marrow needle to be inserted only once into the posterior superior iliac crest. However for this clinical trial, the physician obtains six separate specimens by inserting the bone marrow needle into the same insertion site three times to each hip area, but at a different angle. In multiple sclerosis patients scheduled for bone marrow aspiration, does pre-medication with an anti-anxiety drug and music therapy during the procedure, compared with Morphine Sulfate IV, with Lidocaine at the procedure site for both groups provide better relief with anxiety and pain over a period of one year? Music therapy has been shown to have a significant effect on patients experiencing pain and anxiety, especially during a bone marrow aspiration (Shabanloei and others, 2010). Prescribing oral lorazepam 90 to 120 minutes before the bone marrow exam has proven to be an effective treatment (Milligan and others, 1987). These methods would be simple and inexpensive for health care to use.

Book excerpt: The purpose of this study was to examine the effect of music therapy on patient anxiety and perception during radiation oncology procedures. The design of this study included experimental and control groups with pre-test and multiple post-test data collection. Groups were randomly assigned to experimental (music) and control (standard treatment) groups and included patients with head/neck, abdomen and breast treatment area (N = 26). The dependent variables included a Visual Analog Scale (VAS) anxiety measure, items from the Hospital Consumer Assessment of Healthcare and Provider Services/Picket and the facility Music Therapy satisfaction questionnaires. Results of a Two-Way Analysis of Variance with repeated measures revealed significantly lower post-test procedural anxiety between the experimental and control group across procedures after the simulation, the first treatment and the fifth. Patients' overall rating of the experience during each visit was significantly different between groups and across conditions. Overall, music therapy decreased anxiety, especially at the time of the first treatment.
