Book excerpt: The therapeutic alliance is a critical process variable found across psychotherapy protocols, as well as in other helping fields. Notably, the therapeutic alliance has been found to promote client symptomatic improvement in psychotherapy. While the literature suggesting an alliance-outcome link is fairly robust, we still know little about what predicts the strength of the therapeutic alliance. The present study sought to evaluate therapist Facilitative Interpersonal Skills as one such predictor. Therapist Facilitative Interpersonal Skills, assessed through an observer-rated performance task, have been found to predict both client symptomatic outcomes and client-rated alliance in past research. The present study hypothesized the replication of direct effects of FIS on outcome, FIS on alliance, and alliance on outcome, as well as an indirect effect of FIS on outcome through the alliance. The study’s sample consisted of 24 clinical psychology doctoral student therapists, who saw a total of 49 clients (approximately 2 each) during their first year of clinical training. Clients provided symptom and therapeutic alliance ratings at the beginning of each of their therapy sessions. No significant effects were found for FIS predicting outcome (controlling for symptoms at intake) or mean alliance (i.e., across sessions 2-9). No significant effects were found for mean alliance (i.e., across sessions 2-9) predicting outcome. As the independent variable (FIS) was not significantly related to the hypothesized intervening variable (mean alliance across sessions 2-9), and the hypothesized intervening variable was not significantly related to the dependent variable (outcome), the presence of an indirect effect could not be assessed. Further analyses found significant effects of therapist cohort on their FIS (i.e., therapists in the 2017 cohort had significantly higher FIS than those in the 2019 cohort, p = .02). Book excerpt: This study investigated temporal patterns of therapeutic alliance development across treatment and their relationship to outcome, and the relationship between client attachment styles and interpersonal problems and alliance strength and patterns across treatment. Sixty-six clients diagnosed with major depression participated in one of two 16-week treatment conditions: Process-experiential therapy and Cognitive-behavioural therapy. Results revealed two alliance "shape" patterns; Stable Linear and Steep Linear-Quadratic, as well as two alliance "shape and level" patterns; High Strength Linear-Quadratic and Low-to-Mid Strength Stable . Results indicated that the two alliance "shape" patterns did not differentially predict treatment outcome. With respect to "shape and level" patterns, results revealed that clients with a High Strength Linear-Quadratic alliance pattern had better treatment outcomes than clients with a Low-to-Mid Strength Stable alliance pattern. In terms of client attachment style, results indicated that clients with a Steep Linear-Quadratic "shape" pattern report more discomfort with closeness at pre-treatment. Further, clients with a High-Strength Linear-Quadratic "shape and level" pattern report more confidence and less need for approval at pre-treatment. With respect to client interpersonal problems, results revealed that clients with a Steep Linear-Quadratic "shape" pattern report being more cold and distant at pre-treatment and clients with a Low-Mid Strength Stable "shape and level" pattern report being more socially inhibited, less assertive, and more overly-accommodating at pre-treatment. Lastly, results revealed that specific client attachment characteristics and interpersonal problems differentially predict total average alliance strength as well as the average strength of the client and therapist bond across treatment.

However, there was a significant difference on both dimensions of love and dominance between the mean of the therapists' and observers' appraisals of the therapists' interpersonal impact and the clients' appraisals of the therapists' interpersonal impact. Level of congruence between clients' and therapists' appraisals was significantly related to clients' self-reported symptom severity, but opposite of the predicted direction. Higher congruence between client and therapist appraisals was related to higher client self-reported symptom severity. Clients' symptom severity was also significantly related to the working alliance. Higher self-reported symptom severity was associated with a lower total working alliance score. Level of congruence between clients' and therapists' appraisals was not significantly related to the working alliance.
