Book excerpt: On Friday, the 8th of October 1976, a one day Symposium on Cardiac Pacing, supported by an educational grant from Vitatron Medical, was held at the Sheraton Boston Hotel in Boston, Massachusetts. The intent of the Organizing Committee was to provide a concentrated overview of various aspects of cardiac pacemaker technology ranging from historical aspects, indications, electrode design, energy sources, engineering concepts, detection of malfunction and fol lowup of implanted systems. A look into future trends was also emphasized. The audience consisted of physicians from all parts of the United States with a special interest in pacemaker systems. Their comments, questions, and description of their own experiences, which arose during the discussion period at the end of the program, extended the breadth of the information provided. J . In that year, I visited the University of Minnesota to discuss collaborative opportunities in cardiac anatomy, physiology, and medical device testing. The meeting was with a faculty member of the Department of Anesthesiology, Professor Paul Iaizzo. I didn’t know what to expect but, as always, I remained open minded and optimistic. Little did I know that my life would never be the same. . . . During the mid to late 1990s, Paul Iaizzo and his team were performing anesthesia research on isolated guinea pig hearts. We found the work appealing, but it was unclear how this research might apply to our interest in tools to aid in the design of implantable devices for the cardiovascular system. As discussions progressed, we noted that we would be far more interested in reanimation of large mammalian hearts, in particular, human hearts. Paul was confident this could be accomplished on large hearts, but thought that it would be unlikely that we would ever have access to human hearts for this application. We shook hands and the collaboration was born in 1997. In the same year, Paul and the research team at the University of Minnesota (including Bill Gallagher and Charles Soule) reanimated several swine hearts. Unlike the previous work on guinea pig hearts which were reanimated in Langendorff mode, the intention of this research was to produce a fully functional working heart model for device testing and cardiac research.

The formal recognition of this highly specialized and technical field of medicine represents the culmination of thirty years of remarkable scientific and intellectual discovery. Beginning in the 1950s, cardiologists realized that cardiac arrhythmias were the cause of significant morbidity and the sudden death of at least 350,000 patients every year in the United States alone. At that time the only tools available for analyzing abnormal heart rhythms were the standard EKG machine and careful deductive reasoning. During the early 1960s, cardiac pacemakers reflected the first foray in the electrical therapy of cardiac arrhythmias. Pacemakers were first implanted in order to control syncopal episodes related to bradycardic heart rhythms. Although crude and bulky devices, their utility was immediately obvious to physicians and patients alike. The recognition that electrical signals could be recorded from inside the heart and that the heart's rhythm could be controlled by the application of electrical energy began the era of clinical cardiac electrophysiology which was to follow. In the late 1960s and early 1970s and at the peak of the Vietnam conflict. a group of cardiologists with special training in cardiac electrophysiology were sequestered at the US Public Health Service Hospital at Staten Island.
