Book excerpt: Our ability to treat common bacterial infections with antibiotics goes back only 65 years. However, the authors of this report make it clear that sustaining a supply of effective and affordable antibiotics cannot be without changes to the incentives facing patients, physicians, hospitals, insurers, and pharmaceutical manufacturers. In fact, increasing resistance to these drugs is already exacting a terrible price. Every day in the United States, approximately 172 men, women, and children die from infections caused by antibiotic-resistant bacteria in hospitals alone. Beyond those deaths, antibiotic resistance is costing billions of dollars through prolonged hospital stays and the need for doctors to resort to ever more costly drugs to use as substitute treatments. Extending the Cure presents the problem of antibiotic resistance as a conflict between individual decision makers and their short-term interest and the interest of society as a whole, in both present and future: The effort that doctors make to please each patient by prescribing a drug when it might not be properly indicated, poor monitoring of discharged patients to ensure that they do not transmit drug-resistant pathogens to other persons, excesses in the marketing of new antibiotics, and the broad overuse of antibiotics all contribute to the development and spread of antibiotic-resistant bacteria. The book explores a range of policy options that would encourage patients, health care providers, and managed care organizations to serve as more responsible stewards of existing antibiotics as well as proposals that would give pharmaceutical firms greater incentives to develop new antibiotics and avoid overselling. If the problem continues unaddressed, antibiotic resistance has the potential to derail the health care system and return us to a world where people of all ages routinely die from simple infections. As a basis for future research and a spur to a critically important dialogue, Extending the Cure is a fundamental first step in addressing this public health crisis. Book excerpt: The development of micro- and nanodevices for blood analysis is an interdisciplinary subject that demands the integration of several research fields, such as biotechnology, medicine, chemistry, informatics, optics, electronics, mechanics, and micro/nanotechnologies. Over the last few decades, there has been a notably fast development in the miniaturization of mechanical microdevices, later known as microelectromechanical systems (MEMS), which combine electrical and mechanical components at a microscale level. The integration of microflow and optical components in MEMS microdevices, as well as the development of micropumps and microvalves, have promoted the interest of several research fields dealing with fluid flow and transport phenomena happening in microscale devices. Microfluidic systems have many advantages over their macroscale counterparts, offering the ability to work with small sample volumes, providing good manipulation and control of samples, decreasing reaction times, and allowing parallel operations in one single step. As a consequence, microdevices offer great potential for the development of portable and point-of-care diagnostic devices, particularly for blood analysis. Moreover, the recent progress in nanotechnology has contributed to its increasing popularity, and has expanded the areas of application of microfluidic devices, including in the manipulation and analysis of flows on the scale of DNA, proteins, and nanoparticles (nanoflows). In this Special Issue, we invited contributions (original research papers, review articles, and brief communications) that focus on the latest advances and challenges in micro- and nanodevices for diagnostics and blood analysis, micro- and nanofluidics, technologies for flow visualization, MEMS, biochips, and lab-on-a-chip devices and their application to research and industry. We hope to provide an opportunity to the engineering and biomedical community to exchange knowledge and information and to bring together researchers who are interested in the general field of MEMS and micro/nanofluidics and, especially, in its applications to biomedical areas.

Book excerpt: As part of its scientific activities, the German Research Council on Smoking and Health regularly provides opportunities for scientists to discuss progress in the field of nicotine research. In this context, the Research Council sponsored a Satellite Symposium in Hamburg, June 28-30, 1990 entitled "Effects of Nicotine on Biological Systems". This meeting was held in conjunction with the XIth International Congress of Pharmacology in Amsterdam and follows the first Satellite Symposium on Nicotine which was convened in Brisbane, Australia in 1987. The aim of these conferences has been to discuss state of the art research on the pharmacology and toxicology of nicotine and its metabolites and to integrate this information to help define nicotinic actions on the central and peripheral nervous system as well as to evaluate health or behavioral effects associated with use of this alkaloid. Furthermore, at this conference, potential therapeutic benefits of nicotine for certain disease states were discussed. Smoking and the health effects of smoking were dealt with only as far as they could not be separated from the effects of nicotine. This volume contains the lectures presented at the symposium and illustrates that knowledge of nicotine has advanced considerably in recent years with regard to mechanisms of its actions. Despite such progress however, it is apparent that a' large number of questions remain unanswered, especially in the light of new insight into cellular and molecular mechanisms which can be affected by nicotine.
