Additionally, they have fine long-term clinical outcomes when used in implant-supported overdentures. However, the treatment concept of telescoping implant fixed dentures for edentulous patients has been described only in few case reports so far. Our team believes telescopes can be flexibly used in retaining full-mouth implant-supported fixed protheses. Purpose: The aim of this investigation was to figure out the implant and prosthetic survival rates, the healthy condition of peri-implant soft and hard tissues, patient satisfaction and the incidence rates of prosthetic maintenance for an average follow-up period of 3 years. Material and Methods: From March 2012 to June 2017, 17 edentulous patients (11 male and 6 female; mean age 56) were selected to have implant-supported fixed telescopic prostheses. A total of 18 protheses were fabricated(8 maxillary and 10 mandibular). Fixed dentures retained by telescopic crowns were used as the final prosthesis, with milling titanium or all-ceramic abutments serving as primary crowns and gold-electroforming crowns serving as secondary crowns. Health status of peri-implant soft tissue, marginal bone loss, patient satisfaction and prosthetic complications were evaluated at the time of follow-up visits (3 months, 1 year, 2 years and 3 years after definitive rehabilitation). Data was counted and analyzed applying SPSS v23 software package. Quantitative data about health condition of soft-tissue and bone loss during three-yearu2019s observation time was evaluated by the method of multilevel analysis. Multilevel analysis was performed by means of R version 3.1.0. Results: All 17 patients (56 u00b1 6.1 years/range: 42.7-68.5) were treated with one-piece implant-supported fixed telescopic dentures. There were 58 implants in the maxilla and 53 implants in the mandible. 109 implants out of 111 implants were retained, yielding a 98.2% cumulative implant success rate. Radiography results indicated stable marginal bone loss in survival implants. There was no significant differences in modified plaque index and modified sulcus bleeding index over this time (P>0.05). The probing attachment level deteriorated by 1.5 mm during the first 3 years (P0.05). Among the restorations, 2 porcelain fractures were detected but neither had notable impact on the restoration outcomes. All 18 restorations provided sufficient fixation and stability and all patients were satisfied with the outcomes. Totally 2 porcelain fractures occurred but had no influence on the function of restoration. The frequency of prosthetic maintenance per patient per year was 0.11. Conclusion: One-piece implant-supported fixed detachable telescopic restoration is an effective method with short-term esthetic and stable outcomes in edentulous patients. One of the key benefits is the access of both retrievability and retentive force. Besides, it provides the possibility of restoring an edentulous mouth with fixed dentures using implants even situated in undesirable positions or direction. Of these, bone volume at implant insertion is a key factor for predictable longterm results. In particular, atrophies within class III present a medium horizontal defect, which complicates the correct placement ofregular implants without thread exposure, and implant survival. AimTo present 3-year follow-up results from a prospective singlecenter study evaluating the clinical outcome of taperedimplants with conical connection when used in one-stageguided bone regeneration (GBR) procedures at mediumhorizontal bone defects (u22654 to u22646 mm of residual horizontalbone width).Material and MethodsThis study was designed as a single-cohort prospectivesingle center clinical trial. Partially edentulous patients,having u22654 to u22646 mm of residual horizontal bone width, wereselected and treated with simultaneous implant installationand GBR. Implants (NobelReplace Tapered CC PMC, NobelBiocare) were inserted using a GBR sandwich approach: 2mm of autologous bone, and 2 mm of anorganic bovinebone were placed in layers around the implant and coveredusing a resorbable collagen membrane fixed with titaniumpins. Follow-up visits were performed at 15 days, 1-, 3- and6-months after implant insertion, with definitive prosthesisdelivery at 12 months after implant installation. Outcomeswere implant and prosthesis survival rate, report of anybiological and prosthetic complications, peri-implantmarginal bone loss, bleeding on probing (BOP), and probingpocket depth (PPD).ResultsForty-five consecutive patients (20 male, 25 female; meanage 52.1 years) received 63 implants with at least one GBRprocedure simultaneously. All patients returned for the followupexamination 3 years after implant insertion-GBR surgery.No implant or prosthesis failed, giving cumulative implant andprosthetic survival rates of 100%. No major prosthetic orbiological complications were recorded. Mean marginal bonelevelapical to the reference point at implant placement(baseline) was 0.12u00b10.14 mm, and 0.61u00b10.21 mm and0.77u00b10.22 mm at the definitive restoration delivery and 2years after final loading, respectively. The total meanmarginal bone loss by the 3-year visit was 0.65u00b10.18 mm.Mean BOP values did not alter significantly betweendefinitive restoration and 2-years after loading (1.23u00b10.93 vs1.17u00b10.78; P=0.569), and neither did mean PPD values(2.62u00b10.59 vs 2.60u00b10.54; P=0.765).ConclusionsTapered implants with conical connection and partial machinedcollar had all survived with no major complications at 3-yearsafter implant insertion, and mean marginal bone level changes atimplant sites were low. Thus, within the limitations of the presentstudy, simultaneous placement of tapered implants with conicalconnection and GBR seems a viable treatment option for oralrehabilitation at sites with medium horizontal bone defects.

Success factors such asthe absence of mobility, pain, infection, and the amount of crestal bone loss around the implants were determined in this case series,as well as the stability of the prosthesis and patient satisfaction, one year after prosthetic rehabilitation. Results No patient dropped out, no implant failures or complications occurred. With regards to the radiographic bone loss, a 1mm bone losswas observed on 2 implants over the 1-year follow-up period. No screw fracture was observed, nor any unscrewing of prosthesis. Conclusion and Clinical implicationWithin the limits of this case study it can be concluded that Axiom implants have an excellent bone crest stability, and the new in Linkconnection is reliable and defines a new standard in plural restorations.

Book excerpt: Since their introduction, the use of endosseous implant for replacements of missing teeth has increased dramatically. To date, however, there is no evidence-based answer to the important clinical question: What implant-supported prosthesis designs provide the best outcomes? Cochrane systematic reviews have been used to critically assess scientific literature, to answer a wide array of questions. This approach was used to focus on implant-related treatment outcomes, including patient-based outcomes. Data showed that in most cases clinical and patient-mediated outcomes did not differ significantly from one another regardless of the prosthesis used. However, long-term prognosis of implant-supported overdentures using magnet attachments may produce inferior outcomes when compared to other attachments.In this dissertation it was shown that long-term results following immediate loading of endosseous implants with overdentures were successful based on biological and clinical parameters, cost to patient, and patient-based outcomes. Moreover, it was noted that well-designed clinical trials evaluating efficacy of immediate loading with fixed prostheses are uncommon. Furthermore, a parallel randomized controlled clinical trial focusing on this issue was carried out. In this trial, the author investigated the effects of immediate and delayed loading of implants with mandibular fixed prostheses. The implant-related outcomes underscore that even when implants are loaded during the healing period (i.e. immediate loading), successful osseointegration can be achieved and maintained. Patients. satisfaction and oral health-related quality of life indicated that the two loading schemes addressed patient's need equally.

Book excerpt: The rehabilitation of edentulous arches with implant-supported fixed complete dentures (IFCDs) may solve immediate problems of stability and retention associated with conventional removable dentures.Studies have shown an improvement in quality of life in patients with implant-supported fixed total dentures. The objective of this study was to analyze the improvement of this condition over time in implant-supported fixed total prostheses. The study includes even those patients whose prosthetics suffered failure.A sample of 95 patients with IFCDs answered the Oral Health Impact Profile questionnaire (OHIP-14) up to 5 years after the use of prostheses and the data was statistically analyzed.There was a statistically significant difference in the improvement of patients' quality of life immediately after prosthesis installation and at individually evaluated times throughout the study, even with the incidence of mechanical failure. It was concluded that even with the need for repair, the use of IFCDs is satisfactory in improving the quality of life in edentulous patients over time.
