Book excerpt: The main aim of this study is to define the clinical criteria which must be considered in order to come to an adequate decision whether a patient with intracranial arteriovenous malformation (AVM) should be operated upon or treated conservatively. A special method was used to reduce the effects of selection. This method made it possible to evaluate the therapeutic efficacy of conservative treatment versus surgery. The method implies that patients with equal combinations of variables (risk profiles) were compared in the two treatment lines. The variables building up the risk-profile pattern were chosen by analysing the decision process, as it was originally practiced by the surgeon who selected and treated the patients of this study. The risk profiles thus described relevant characteristics of the patient and his malformation. The variables were assigned numerical values accord ing to their prognostic value. Summation of the variables making up the risk profile then gives each risk profile a certain value. A low value symbolizes a bad prognosis and a high value a good prognosis in both treatment groups. There were many risk profiles with the same value in both groups and a comparison could be made over a large part of the risk-profile scale. This comparison showed that surgical treatment of A VM can always be justified, although the indications for surgery are less strong in patients with low risk profile values.

Book excerpt: Focusing on both the patient's perspective and the neurosurgeon's concerns, Intracranial Arteriovenous Malformations: Essentials for Patients and Practitioners, edited by leading experts Drs. Philip E. Stieg, Alexander A. Khalessi, and Michael L. J. Apuzzo, starts with an up-to-date approach to the matter of doctor-patient communication and moves on to the highly technical details of AVM treatment options. The first section covers communication with patients (who may well want to read it themselves); the second section is directed to neurosurgeons and other specialists caring for patients with intracranial AVMs, including those in emergency medicine, obstetrics, anesthesia, and intensive care. It offers a highly sophisticated but readable approach to the contemporary treatment of these challenging lesions. - Provides expert guidance on diagnosis, histopathology, natural history, anatomy, imaging, and treatment options and their risks and benefits—all with the goal of helping patients make informed decisions about the optimal management choices for their own individual cases. - Facilitates articulate, data-driven discussion and regarding the clinical diagnosis and surgical procedures involved in treating AVMs. - Addresses specific, difficult issues that arise during the treatment of AVMs, offering real-world advice to neurosurgeons and other care providers. - Includes key pearls in every chapter, as well as stunning anatomical illustrations throughout.

The book is divided in to three parts; the first part discusses common approaches in neurovascular surgery, describing the steps, indications for and limitations of the approach, as well as the associated complications and how to avoid them. The second part addresses surgical treatment based on pathology, taking the different locations of lesions into consideration. The third part focuses on the technological developments that support neurovascular surgery, which may not be available everywhere, but have been included to help vascular surgeon understand the principles. This book is a guide for young neurosurgeons, neurosurgery residents and neurosurgery fellows, as well as for medical students and nurses who are interested in neurosurgery or are associated with this field in any way. It is also a useful teaching aid for senior neurosurgeons.

Book excerpt: Every so often a gathering of minds and experience occurs that results in an all encompassing overview in depth of such a vast subject as Cerebro-Vascular Malformations, as occurred in Verona in June 1992 and which warrants publication. Professors Da Pian and Pasqualin deserve high compliment and it is a measure of the respect in which they are held that virtually all those most knowledgeable around the world attended, presented their work and thoughts and contributed to intense discussion. Ljunggren's opening historical survey set the stage and must be the most comprehensive yet published. Subarachnoid hemorrhage from aneurysm rupture still constitutes, I dare say, the most difficult problem for neurosurgeons, in relief of the brain injury and arterial reaction and the technical perfection of aneurysm obliteration, even for small, as well as large and giant sacs. Very large high flow A VMs can be as demanding too. The bulk of the conference was devoted to subarachnoid hemorrhage, aneurysms and A VMs which were discussed under about 14 headings each. But vein of Galen malformations, dural A VMs, cavernous angiomas and venous angiomas (renamed developmental venous anomalies) came under scrutiny, not always with consensus. Trends are perceptible such as fibrinolysis of subarachnoid clot, non surgery for Galenic and dural malforma tions, the benignity of venous angioma, but there is still much variation in approach, pharmacologically, technically and with such as the evolving endovascular and radiosurgical stories, used alone or in conjunction.

Available in PDF, EPUB and Kindle. Book excerpt: The book provides an excellent review of all the clinical aspects of neuroanesthesia in children, including neurosurgeries during fetal state to neonatal, infancy, toddler, and school-going age groups. To provide optimal anesthetic care in children undergoing neurosurgery, the care provider must have adequate knowledge on the developing brain and spinal cord, and the effect of anesthetics on the neuronal tissue, and the inherent issues pertaining to neurologic lesions. This book covers the diagnostic, imaging, surgical as well as anesthetic managements of all the neurosurgical problems in children. The chapters include a wide range of topics from basic neurophysiology to general concerns for pediatric neuroanesthesia, including fluid management, blood transfusion, temperature regulation, and surgical positioning, as well as specific issues such as anesthesia for brain tumor surgery, hydrocephalus, neural tube defects, cerebrovascular surgeries such as aneurysmal surgery, arteriovenous malformations (AVMs), Moyamoya disease, and vein of Galen malformation, functional neurosurgery, epilepsy surgery, neuroendoscopy, craniovertebral junction anomalies, spinal surgeries, neurotrauma, and brain abscess with congenital heart diseases. Interesting topics like neuroanesthesia in remote locations, regional anesthesia during neurosurgery, and anesthesia for children with neuromuscular disease are also discussed. Moreover, the book elaborates on advanced neuroanesthesia techniques during fetal neurosurgery and craniopagus separation surgery; and the postoperative intensive care management aspects in each chapter. It is supplemented with figures depicting surgical procedures and positioning, neuroimages, tables and illustrations for easy understanding. This book caters to neuroanesthesiologists, pediatric anesthesiologists, residents, and fellows of anesthesia or neuroanesthesia, practicing anesthesiologists, pediatric neurointensivists, nurse anesthetists, neurosurgeons, and pediatric neurosurgeons. It also serves as a reference book for the DM (neuroanesthesiology and neurocritical care), DNB-SS (neuroanesthesiology), and MD (anesthesiology) curriculums apart from anesthesia residency and pediatric anesthesia/ neurosurgery fellowship programs offered at various Institutions worldwide.

Book excerpt: Arteriovenous malformations (AVM) and arteriovenous fistulas (AVF) differ from all other pathology affecting the central nervous system by their high-flow arteriovenous shunts. Permanent occlusion of these shunts is the essence and the challenge of therapy. Endovascular therapy and radiosurgery became accepted alternatives or adjuncts to surgery. In many instances the choice of the primary therapeutic modality is not clear and arguments can be found for several options. However, microsurgery, endovascular therapy and radiosurgery differ very much with regard to invasiveness, length of stay at the hospital but also residual risk after therapy. The emerging treatment concepts are the object of this book. The result is a unique structured presentation of AVM and AVF therapy.
