Download or read book Medicaid and SCHIP recent HHS approvals of demonstration waiver projects raise concerns. written by and published by DIANE Publishing. This book was released on 2002 with total page 71 pages. Available in PDF, EPUB and Kindle. Book excerpt: States provide health care coverage to about 40 million low-income uninsured adults and children largely through two federal-state programs-Medicaid and the State Children's Health Insurance Program (SCHIP). Medicaid generally covers low-income families and elderly and disabled individuals, while SCHIP provides health coverage to children in families whose incomes, while low, are above Medicaid's eligibility requirements. To receive federal funding, which covered on average about 57 percent of Medicaid expenditures and 72 percent of SCHIP expenditures in 2001, states must meet certain statutory requirements including providing a certain level of benefits to specified populations. Under section 1115 of the Social Security Act, the Secretary of Health and Human Services (HHS) can waive many of the statutory requirements in the case of experimental, pilot, or demonstration projects that are likely to promote program objectives. As part of their responsibility to protect the fiscal integrity of the programs, traditionally, HHS and the Office of Management and Budget (OMB) have had a policy that all approved waiver projects be "budget neutral" for the federal government-that is, the proposed project cannot result in federal expenditures that are higher than they would have been without the project.

Book excerpt: As part of an eventual statewide set of health insurance reform measures, the State of Oregon has proposed implementing a demonstration program, with Federal cofunding, that would change the State's existing Medicaid program in three fundamental ways. It would: 1) expand coverage to include all persons with incomes up to 100 percent of the Federal poverty level; 2) enroll all covered persons in some form of managed care, such as with a health maintenance organization or a "gatekeeper"primary care physician; and 3) determine acute and primary health care benefits according to a ranked list of services, with actual benefits dependent on the level of program funding. Available in PDF, EPUB and Kindle. Book excerpt: Section 1115 of the Social Security Act provides the Secretary of Health and Human Services (HHS) with broad authority to conduct research and demonstration projects under several programs authorized by the Social Security Act. Two such programs are Medicaid and the State Children's Health Insurance Program (SCHIP). Specifically, Section 1115 allows the Secretary to waive certain statutory requirements to modify virtually all aspects of the programs as long as the changes further the goals of Titles XIX (Medicaid) and/or XXI (SCHIP). In recent years, there has been increased interest among states and the federal government in the Section 1115 waiver authority as a means to restructure Medicaid coverage, control costs, and increase state flexibility. States have used the Section 1115 waiver authority to cover nonMedicaid and SCHIP services, limit benefit packages for certain groups, cap program enrollment, among other purposes. As of fall 2003, there were 68 operational (i.e., approved and implemented) Medicaid and SCHIP Section 1115 waiver programs. In FY2003 (the most recent data available), Section 1115 waiver federal expenditures (for Medicaid and SCHIP) totaled $22.4 billion. Section 1115 waiver programs represented nearly14% of all federal Medicaid spending in the 50 states and the District of Columbia for FY2003 (8% for SCHIP), and provided coverage to approximately 9.5 million enrollees. Of the 9.5 million total Medicaid and SCHIP waiver enrollees, 2.4 million were eligible only for a targeted benefit package such as family planning or pharmacy benefits. This report provides background information on the waiver authority, and will be updated when new data are available.
