It highlights continued progress made towards meeting international targets for malaria control to be achieved by 2010 and by 2015. International funds disbursed for malaria control increased from $200 million in 2004 to $1.5 billion in 2009. Since 2008, more than 289 million insecticide-treated mosquito nets have been delivered to sub-Saharan Africa, enough to protect three quarters of the 765 million persons at risk of the disease. Over the last decade, 11 countries in the Africa Region and 32 countries in other regions showed reductions of 50% or greater in either confirmed malaria cases or malaria admissions and deaths. Malaria control is making a major contribution to reducing mortality in children less than 5 years old and to attaining the health-related Millennium Development Goals.

Book excerpt: Contexte : La carie dentaire reste une pathologie à forte prévalence, avec pour conséquences de nombreuses complications médicales locales et générales, à l'origine d'un important coût de santé. Des thérapeutiques fluorées topiques et systémiques existent, mais semblent ne pas être utilisées à bon escient par les médecins généralistes, malgré des recommandations récentes et complètes. Objectif : Mettre en évidence les déterminants de la prise en charge préventive de la carie dentaire et de la prescription des thérapeutiques fluorées par les généralistes. Méthode : Une étude qualitative a été menée, permettant la réalisation de 15 entretiens semi-dirigés auprès de médecins généralistes dans plusieurs communes des Alpes-Maritimes et du Var. Après retranscription des entretiens, le codage des verbatims a permis d'aboutir à une analyse thématique, et à l'extraction des données recherchées. Résultats : L'étude révèle qu'aucun des praticiens interrogés n'assure la prise en charge de la prévention de la carie dentaire recommandée par l'HAS et l'AFSSAPS. Ils expriment de nombreuses difficultés pour justifier ce manquement, parmi lesquelles le manque de temps est omniprésent, imposant aux praticiens de ne répondre qu'aux motifs de consultation et de délaisser la prévention dentaire, qu'ils jugent souvent du ressort d'autres professionnels, tels le dentiste ou le pédiatre. Le manque d'information des patients est également délétère. Ils ne semblent pas être au fait des mesures préventives, et n'en parlent pas à leurs médecins. Les praticiens, non sollicités et souffrant d'un manque de temps, se forment peu au domaine dentaire. Les programmes de DPC sur le sujet sont quasi inconnus. La prescription fluorée s'en trouve inadaptée, voire souvent inexistante, d'autant que le calcul de posologie s'avère complexe, notamment par la nécessité de calculer les apports alimentaires en fluor. Conclusion : Cette étude pose les bases de futurs travaux. Une prochaine étude pourra confirmer les tendances retrouvées, et une réflexion pourra être menée sur les perspectives d'amélioration des pratiques des généralistes, notamment en terme de formation et d'aménagement de consultations à visée dentaire.

Les inégalités territoriales d'accès aux soins dentaires, les disparités de moyens pour y accéder ainsi que le coût important pour la société font de la carie dentaire de l'enfant un vrai enjeu de santé publique. Pourtant, des moyens de prévention existent, notamment le recours au dentifrice fluoré. L'objectif de cette étude est de synthétiser les preuves sur l'impact du fluor topique par dentifrice dans la prévention primaire de la carie dentaire chez les enfants de moins de 6 ans. METHODE : Revue parapluie portant sur les enfants de moins 6 ans, exposés au dentifrice fluoré vs non exposés au fluor par dentifrice, avec pour critère de jugement l'augmentation moyenne du dmfs/ DMFS ou dmft/DMFT (PICO). RESULTATS : 4 revues systématiques ont été incluses sur les 910 identifiées. Il existe une différence statistiquement significative en faveur de l'utilisation des dentifrices fluorés de plus de 1000ppm, pour les 3 revues ayant effectué des méta-analyses, les comparant à un placebo ou l'absence d'intervention : PF groupé 30% IC à 95% (18 à 43%), DMS -0,24 IC à 95% (-0,36 à -0,13%), DM -1,86 IC à 95% (-2,52 à -1,21%). Bien que plus efficace que l'absence d'intervention ou un placebo, aucun des résultats portant sur les dentifrices de moins de 1000ppm de fluor n'est significatif. CONCLUSION : Le dentifrice au fluor est efficace chez les enfants de moins de 6 ans en prévention primaire de la carie dentaire. Une relation dose-réponse semble exister dans l'efficacité du dentifrice au fluor. Des études complémentaires sont nécessaires pour affirmer ce propos. Le médecin généraliste devrait être partie prenante de la prévention de la carie dentaire précoce par une information fiable et systématique auprès des parents et de leurs enfants, lors du suivi habituel pédiatrique.
