La population infectée aspire à une vie et à une sexualité normales. En 2008, les experts suisses affirmaient que toute personne séropositive pour le VIH et ayant une charge virale plasmatique (CVp) indétectable depuis au moins 6 mois, sous traitement antirétroviral (TARV) efficace, n’ayant pas d’infection sexuellement transmissible (IST), ne transmettait pas le VIH par voie sexuelle. Cet avis s’appuyait sur des études auprès de couples hétérosexuels séro-différents. Est-il extrapolable à toutes les populations ? La littérature montre qu’il existe une corrélation évidente entre la CVp et la CV spermatique (CVsp), et que la transmission sexuelle dépend du niveau de CVp. Néanmoins, il existe dans 3 à 7% des cas, une discordance entre le compartiment sanguin et génital. Il est difficile aujourd’hui, de connaître le degré d’infectivité d’une CVsp détectable et si cette excrétion peut aboutir à une transmission du VIH. La question du risque résiduel à l’échelle de l’individu se pose. Notre enquête, réalisée auprès de médecins généralistes et de leurs patients vivant avec le VIH sur la perception de ce risque, montre que 19% des médecins interrogés véhiculent le message de l’avis suisse, uniquement lors d’un projet de parentalité. 71% des patients pensent être contaminants malgré une CVp indétectable. 41% ont des rapports sexuels non protégés et 17% avec une CVp détectables. Cette persistance des comportements à risque doit faire du TARV, un outil de prévention combinée, ce d’autant que les autres IST et hépatites virales sont en recrudescence
