Book excerpt: The medical research landscape in the United States is supported by a variety of organizations that spend billions of dollars in government and private funds each year to seek answers to complex medical and public health problems. The largest government funder is the National Institutes of Health (NIH), followed by the Department of Defense (DoD). Almost half of DoD's medical research funding is administered by the Congressionally Directed Medical Research Programs (CDMRP). The mission of CDMRP is to foster innovative approaches to medical research in response to the needs of its stakeholdersâ€"the U.S. military, their families, the American public, and Congress. CDMRP funds medical research to be performed by other government and nongovernmental organizations, but it does not conduct research itself. The major focus of CDMRP funded research is the improved prevention, diagnosis, and treatment of diseases, injuries, or conditions that affect service members and their families, and the general public. The hallmarks of CDMRP include reviewing applications for research funding using a two-tiered review process, and involving consumers throughout the process. Evaluation of the Congressionally Directed Medical Research Programs Review Process evaluates the CDMRP two-tiered peer review process, its coordination of research priorities with NIH and the Department of Veterans Affairs, and provides recommendations on how the process for reviewing and selecting studies can be improved.

Then, the report describes strengths and weaknesses of major peer review components and issues, including the following: Objectives and Purposes of Peer Review; Quality of Peer Review; Impact of Peer Review Manager on Quality; Selection of Peer Reviewers; Selection of Evaluation Criteria; Secrecy (Reviewer and Performer Anonymity); Objectivity, Bias, and Fairness of the Peer Review; Normalization of Peer Review Panels; Repeatability and Reliability of the Peer Review; Effectiveness and Predictability of Peer Review; Global Data Awareness; Costs of Performing a Peer Review; Ethical Issues in Peer Review; and Alternatives to Peer Review. The report then presents different federal agency peer review practices as well as sample protocols and processes for conducting a successful research program peer review. Some peer review variants, such as the Science Court and Network-Centric Peer Review, are described, and research requirements to improve peer review are discussed. Book excerpt: The Office of Science and Technology (OST) of the U.S. Department of Energy's (DOE's) Office of Environmental Management (EM) recently has instituted a peer review program that uses the American Society of Mechanical Engineers (ASME), with administrative and technical support provided by the Institute for Regulatory Science (RSI), to conduct peer reviews of technologies (or groups of technologies) at various stages of development. OST asked the NRC to convene an expert committee to evaluate the effectiveness of its new peer review program and to make specific recommendations to improve the program, if appropriate. This is the first of two reports to be prepared by this committee on OST's new peer review program. OST requested this interim report to provide a preliminary assessment of OST's new peer review program. In the final report, the committee will provide a more detailed assessment of OST's peer review program after its first complete annual cycle.

Book excerpt: The Government Performance and Results Act (GPRA), passed by Congress in 1993, requires that federal agencies write five-year strategic plans with annual performance goals and produce an annual report that demonstrates whether the goals have been met. The first performance reports are due in March 2000. Measuring the performance of basic research is particularly challenging because major breakthroughs can be unpredictable and difficult to assess in the short term. This book recommends that federal agencies use an "expert review" method to examine the quality of research they support, the relevance of that research to their mission, and whether the research is at the international forefront of scientific and technological knowledge. It also addresses the issues of matching evaluation measurements to the character of the research performed, improving coordination among agencies when research is in the same field, and including a human resource development component in GPRA strategic and performance plans.

Directed research is commissioned or noncompetitively awarded research that is not competitively solicited because of specific reasons, such as time limitations or highly focused or constrained research topics. The scientific merit assessment processes have been developed by NASA to evaluate individual directed research tasks in order to ensure the scientific integrity of the HRP's directed research portfolio. The committee examined the HRP's current scientific merit assessment processes and conducted a public workshop to identify best practices among other federal agencies that use various assessment processes for similar types of directed research. Review of NASA Human Research Program's Scientific Merit Processes: Letter Report finds that the scientific merit assessment process used by the HRP for directed research is scientifically rigorous and is similar to the processes and merit criteria used by many other federal agencies and organizations - including the Department of Defense, National Institutes of Health, and the United States Department of Agriculture - for comparable types of research. This report also makes recommendations on ways to streamline and bolster the accountability and transparency of NASA's current processes.
