In Remaking Policy, Carolyn Hughes Tuohy advances an ambitious new approach to understanding the relationship between political context and policy change. Focusing on health care policy, Tuohy argues for a more nuanced conception of the dynamics of policy change, one that makes two key distinctions regarding the opportunities for change and the magnitude of such changes. Four possible strategies emerge: large-scale and fast-paced ("big bang"), large-scale and slow-paced ("blueprint"), small-scale and rapid ("mosaic"), and small-scale and gradual ("incremental"). As Tuohy demonstrates, these strategies are determined not by political and institutional conditions themselves, but by the ways in which political actors, individually and collectively, read those conditions to assess their prospects for success in the present and over time. In labour market policy, this approach is reflected in the widespread emphasis on flexibility - a euphemism for the retrenchment of income support and social security, the relaxation of labour market regulations, and the enhanced power of private actors to determine the terms of the employment relationship. These strategies have had marked effects on labour market outcomes, leading to greater vulnerability and polarization - and not always in ways that enhance worker-centred flexibility. The authors offer a more balanced analysis of the functioning and effects of labour market regulation and deregulation. By questioning the underpinnings of the flexibility paradigm, and revealing its often damaging impacts (on different countries, sectors, and constituencies), they challenge the conclusion that unregulated market forces produce optimal labour market outcomes. The authors conclude with several suggestions for how labour policy could be reformulated to promote both efficiency and equity.

At the same time, the failure of provinces to correct health care problems acts a serious irritant in intergovernmental relations, creating a vicious cycle where deficiencies in intergovernmental fiscal relations make health care reform difficult while failures to effect health care reform increase conflict between the provinces and the federal government. This collection of essays analyses key issues in federal-provincial health care relations, particularly the fiscal component. The authors look at why there is a role for the federal government in health care and consider the critical issues in recent intergovernmental political battles over this role. The issues of whether the vertical federal-provincial fiscal imbalance is myth or reality, how much the federal government does and should contribute financially to provincial health care programs, and methods for settling disputes, such as those over user fees, are discussed.
