This volume therefore is not intended to be comprehensive - far from it. We have tried to be selective in the material presented and the references quoted. They should provide a quick guide and access to key works in the literature which can be expanded by library or computerised searching. We hope that our efforts will widen the appreciation of purines as biologically important substances and will herald the day when more purine related drugs than the handful currently available are used for the selective treatment of disease. These projects have focused on enhancing the efficiency of animal production through better use of locally available feed resources and those that do not compete with human food. As a part of these efforts, the Joint Division has been actively involved in developing and simplifying approaches for measuring microbial protein supply to ruminant livestock. The contribution of microbial protein in meeting the overall requirement for protein in ruminants is significant, but understanding the contribution of microbial protein to the nutrition of ruminant livestock has been hampered by the lack of simple and accurate methods for measuring microbial protein production in vivo. A Consultants’ Meeting was held in 1995 in Vienna to explore the feasibility of using nuclear and related techniques to develop and validate new approaches for measuring microbial protein supply in ruminant animals. Based on the available information, the consultants considered that the purine derivative excretion method, which is non-invasive and both simple to use and inexpensive compared with other available methods, had the potential for use in developing countries. However, development of models for various animal species and breeds and their validation were required before this technique could be advocated for use.

The first meeting of this series of symposia convened in Tel Aviv, Israel, and since then meetings have taken place every three years in various parts of the world. The second meeting was held in Baden, Austria, the third in Madrid, Spain, the fourth in Maastricht, the Netherlands, and fifth in San Diego, California. The Vlth meeting in Hakone marked the first such symposium held in Asia. On occasion of publishing these books, I would like to describe how research in this field has evolved in Japan. Early in the 1950s, I was engaged in clinical practice treating various rheumatic diseases as an orthopedicist, and found that a substantial percentage of our patients had symptoms apparently compatible with gout. During the 1960s, the number of these gouty patients increased, and in the 1970s, research on the pathogenesis of gout was performed on the basis of approximately 2,000 cases of this disease, together with precise epidemiological studies concerning gouty and hyperuricemic individuals. Data derived from the two kinds of study had greatly changed the notion that gout was a rare disease among Japanese. My clinical studies have been succeeded by research at the molecular level on various purine metabolic abnormalities, including not only gout but also other diseases with various symptoms of wide clinical spectra.

The interest of the chemist in gout began 1776 with the isolation of uric acid from a concretion of the urinary tract by the Swedish chemist SCHEELE. The same substance was subsequently extracted from a gouty tophus by the British chemist WOLLASTONE in 1797 and a half century later the cause of the deposits of sodium urate in such tophi was traced to a hyperuricemia in the serum of gouty patients by the British physician Alfred Baring GARROD who had also received training in the chemical laboratory and was therefore a fore-runner of many of today's clinician-investigators. The recent surge of progress in understanding of some of the causes of gout in terms of specific enzyme defects marks the entrance of the biochemist into this field of investigation. The identification of the first primary defect of purine metabolism associated with over-production of uric acid, a severe or partial deficiency of the enzyme hypoxanthine-guanine phospho ribosyltransferase was achieved less than a decade ago. The knowledge of the mechanism of purine over-production that it generated led shortly to the identification of families carrying a dominantly (possibly X-linked) inherited increase in the activity of the enzyme phosphoribosylpyrophosphate synthetase as a cause of purine over-production. Yet this is only a start as these two types of enzyme defects account for less than five per cent of gouty patients.
