They are also used to monitor and maintain adequate tissue perfusion, infusions and transfusions(CDC, 2011). These catheters provide immense benefit and as so, they are being utilized in the settings of acute care more and more. Yet, with these benefits come complications and one that is the most common is infections. These are infections are commonly known as (CLABSI) or Central Line Associated Bloodstream Infections. The primary reason for the complications is due to lack of proper insertion techniques, cleanliness and lack of management after insertion (CDC, 2011). The issues that are related to this complication of infections are morbidity, mortality, longer hospital stays, and financial costs to the patient and the hospital (Jessica and Robin, 2010). These infections are estimated to range in the levels of about of 41,000 on annual basis (CDC, 2013). These infections can cause longer stays in the hospital and at least an added $30,000 per happening. CLABSI costs healthcare organization in the USA over $2 billion on a yearly basis (Guerin and others,). The situation is so extreme that several respected institutions have labeled and added CLABSI to their never event list (Miller-Hoover, 2011). It is up to providers of health care to be able to assess the situation and finding ways to prevent these infections. This increases the potential of higher quality in the outcomes of patients and decreased costs overall. The review of literature leads the author to believe that one of the best techniques in preventing CLABSI is through the use of a suggestion by the IHI. This suggestion is about the use of the (CLB) or the Central Line Bundle. (CLB) is comprised of specific suggestions and tips that target any issues that might cause the infections. These tips cover aspects of where exactly the catheter must go, the selection of the site, protocols for hygiene, utilization of (chlorhexidine) to cleanse the dermis, high barrier safety measures, and constant management of the line (Royer, 2010). The aim of this project is to prevent CLABSI through the utilization, implementation and maintenance of EBP of the CLB in the (ICU) at Baylor University Medical Center in Dallas (BUMC). Favorable outcomes are to be ensured through the proper implementation of the CBL through rigorous planning, execution, attainment of resources and the education of staff involved.

Book excerpt: In the year 2000, when the Institute of Medicine (IOM) published their findings on avoidable medical errors, Central Line-Associated Bloodstream Infection (CLABSI) rates began being viewed in a far different light (Kohn, IOM 2000). With CLABSI being identified as an avoidable medical error, hospitals around the country began working on solutions to reduce or eliminate CLABSI infections. CLABSI is a potentially life threatening condition which increases a patient's mortality and morbidity by 12% to 25%, with an average health care cost of $16,550 per case (HICP 2011). The Comprehensive Unit-based Safety Program (CUSP) was developed at Johns Hopkins Hospital and successfully implemented in 2003 during the Michigan Keystone project (Provonost 2006). One aspect of the Keystone utilized a program of interventions to prevent CLABSI infections called Maximum Barrier Precautions (MBP). Utilizing the CUSP process to implement MBP was demonstrated to Decrease CLABSI rates by 66% with sustained results beyond 18 months. This capstone project proposes assembling a CUSP team using the same process developed at Johns Hopkins Hospital. Once assembled, the team would choose appropriate evidence-based materials for educating staff, physicians, and hospital leadership. The team would also create a proposal that would outline the necessary costs associated with a hospital wide implementation program that accounted for all necessary education hours, equipment and supplies purchases, education materials and dissemination of information handouts and posters. A hospital wide policy and a checklist that contained all the necessary MBP elements would also be created for use with every central venous catheter (CVC) insertion. The team would establish a system of tracking checklist compliance and a method of surveillance to insure MBP compliance. The infection control nurse would be responsible for investigating and reporting monthly CLABSI rates to the CUSP team and to hospital leadership. Information would be disseminated by the CUSP team to the staff and physicians of the hospital to demonstrate the ongoing importance of their efforts and to give needed feedback so that front line staff is aware of their successes or need for increased efforts. This author did not utilize the proposal outlined in this paper, but was a participating member of an actual MBP implementation project at O'ahu Medical Center starting in January of 2009. When the actual project was launched in June of 2009, O'ahu Medical Center was able to demonstrate a zero percent CLABSI rate for 980 days, followed by a sustained decrease in CLABSI cases over the following three years (Appendix E and G).

Central intravenous lines on patients admitted to this critical care setting provide intravenous access for medications, total parenteral nutrition, and for hemodynamic monitoring. Although CVCs aid hospital staff in providing efficient care for these critically ill patients, these tools also place the patients at risk for central line-associated bloodstream infections (CLABSI). CLABSI, a nosocomial infection, is a major source of morbidity and mortality. It is also associated with increased cost of care for healthcare facilities across the country, averaging $45,000 per single infected patient. (Hakko, Guvenc, Karaman, Erdem, and Cakmakci, 2015) This project proposal calls for an implementation of a six-month pilot study to be conducted in the facility's medical intensive care unit to adopt a new updated central line management bundle. The new bundle will incorporate the utilization of disinfectant port protectors or alcohol-impregnated port protectors as a measure to prevent bloodstream infections. The utilization of disinfectant port protectors, which are saturated with alcohol, has been proven by several published research studies in preventing central line-associated bloodstream infections. In addition to the pilot program, facility-wide education will be conducted to educate nurses and other allied healthcare personnel about central line-associated bloodstream, central line management bundle, and prevention strategies. Educating healthcare workers about CLABSI and proper maintenance of central intravenous lines is primary prevention strategy, which can aid decrease the facility's CLABSI rate, and also decrease the facility's unnecessary expenditures of treating these infections. It is also important to disseminate these possible results, so that nurses know the value of compliance and strict adherence to protocols of central line management care in preventing CLABSI.

Based on the evidences from the 15 selected articles author could prove that bundle care is very helpful to prevent CLABSI in health care facility. At a minimal cost, each steps in central line bundle care and maintenance log was created as a checklist as per the Centers for Disease Control and Prevention (CDC) guidelines. Through this program it is expected to have zero CLABSI rate in the unit and decrease in secondary complications. The purpose of this program would be positively impacted by the dissemination of implementation strategies and evidences. It is proved that by providing education and reinforcing medical professionals about care and maintenance of central line has drastically reduced the rates of CLABSI occurrence, related complications, cost of health care and thereby increasing the satisfaction rates of the patients.
