This change from the older model of inpatient care has important implications for overall quality of care for oncology patients and nutritional care in particular. Amidst growing concern about access to oncology nutrition services, combined with growing recognition of the importance of providing nutritional care to optimize oncology treatment outcomes and maximize quality of life among both patients and survivors of cancer, the National Academies of Sciences, Engineering, and Medicine convened a public workshop in March 2016 to explore evolving interactions between nutritional care, cancer, and health outcomes. Participants explored how health outcomes and survival of cancer patients in outpatient cancer centers are affected by current standards for nutritional services, nutritional interventions, and benefits associated with oncology patient access to medical nutrition therapy. They also studied the cost of outpatient nutritional care and assessed costâ€"benefit relationships between oncology nutrition services and health outcomes and survival. This publication summarizes the presentations and discussions from the workshop.

The use of nutritional assessment tools for both diagnostic and outcome measure purposes has been widely explored. However, understanding of their responsiveness to nutritional interventions is still in its incipient stages. There has been increasing awareness of the importance of nutritional screening and assessment, especially the integration of skeletal muscle mass evaluation into clinical practice. In this context, a standardized definition of low muscle mass, myosteatosis, and muscle strength is still under debate. Since there is no clear definition of proper cut-off points for each assessment tool, the understanding and synthesis of the literature results are hindered. Moreover, the interplay among nutritional disorders has been under-explored in literature research. Efforts should be made to assess the association between comprehensive nutritional diagnosis and the prognosis of chronic diseases and surgical outcomes, as most studies provide information on one feature of nutritional status and do not explore standardized nutritional assessment widely.

Chapter 3 elegantly and extensively reviews the nutritional implications; its biochemistry and the role of one of the most studied amino acids in clinical practiceNglutamine. This is followed by two chapters of nutrition support of patients with head and neck cancer and nutrition support of patients with gastrointestinal cancer. Chapters 6 and 7 review the role of total parenteral nutrition on perioperative nutritional support and cell cycle kinetics. While the plasma amino acids profile in cancer patients and the role of L-methionine is addressed in great details in Chapters 8 and 9, the role of ornithine alpha-ketoglutarate administration on surgical, trauma and cancer-bearing patients is reviewed on Chapter 10. Part Two of this book starts with the review of nutritional support in small bowel transplantation. This Section elegantly describes the process of recovery of small bowel from the ischemia and preservation, weaning from parenteral nutrition support and establishment of normal diets. In addition monitoring techniques and the nutritional complications of surgical intervention is described. Chapter 12 on liver failure and liver transplant patients addresses hepatic encephalopathy and the role of certain amino acids, nutrition assessment techniques and metabolic changes following liver transplantation. Furthermore, it offers some practical advice on how to establish nutrition support routes in these very ill patients. Nutrition support in renal transplantation, including metabolic abnormalities in renal failure, are described on Chapter 13. This monograph ends with a Chapter on total parenteral nutrition in bone marrow transplant patients.
