The seizure-free "latent period" that often follows the brain injury is of unknown mechanistic significance but is commonly considered as the "epileptogenic" period characterized by gradual pathogenic processes leading to the onset of clinically detectable epilepsy. Acute convulsive status epilepticus (SE) is often associated with an adverse developmental outcome characterized by learning disabilities related to the cumulative effects of seizures and development of TLE. The symptomatic manifestations of TLE appear only after a widespread irreversible damage of entorhinal cortex, and hippocampus, the brain area most affected by this disease. These pathological features of TLE reduce the possibility of successful therapeutic approaches, often rendering the disease refractory. The difficult clinical management of chronic TLE and the limited success rate of surgical approaches, increase the incapacitating nature of this specific epileptic disorder. Prevention of TLE with an appropriate intervention after a known inciting event (in the case of acquired epilepsy) might represent the most ambitious goal in the clinical treatment of this epileptic disorder, but has been largely unsuccessful to this point. Clinical trials aimed at prevention of chronic epilepsy have often produced negative, disappointing results. However, in most cases, these studies ultimately evaluated the downstream clinical manifestations, failing to monitor early, specific molecular epileptogenic events. Therefore, elucidation of the underlying mechanisms of epileptogenesis, and their time course(s) are essential. The primary purpose of this topic is to collect scientific contributions providing novel insights in the cellular and molecular mechanisms of epileptogenesis as potential targets for innovative therapeutic approaches aimed at preventing the chronic epileptic disorder.

These reviews, however, have focused almost exclusively on studies performed in models of partial (or focal) epilepsy. It was natural that scientists interested in the fundamental mechanism of epileptogenesis at the cellular level were attracted to work on models in which the epileptogenic dysfunction could be confined to ever-smaller populations of neurons. Many of the most illuminating studies done in this field in the recent past have been carried out in vitro on brain slices or on dissociated neuronal cultures. Much valuable insight into the mechanisms of epileptogenesis has been obtained in this manner. One cannot, however, automatically assume that mechanisms elucidated in such exquisitely "focal" models are applicable to forms of epilepsy in which seizures from their very start involve the entire brain. One of the most difficult questions that arises in this context is how epileptogenic dysfunction, which can be understood in terms of mechanisms involving a few contiguous cells, can lead to what appears to be a simultaneous onset of seizure discharge in virtually all areas of the brain. Most of the workers who have made excellent contributions to the understanding of the cellular mechanisms involved in epileptogenesis have paid virtually no attention to this important problem.

Chemical neurotransmission in the vertebrate nervous system represents a form of signaling of this type. The biology of chemical neurotransmission is complex, involving transmitter synthesis, transport, and release by the presynaptic neuron; signal generation in the target tissue; and mechanisms for termination of the response. The focus of this book is on one aspect of this scheme: the diverse electrophysiological effects induced by different neurotransmitters on targets cells. In recent years, astonishing progress has been made in elucidating the specific physiological signals mediated by neurotransmitters in the verte brate nervous system, yet, in our view, this has not been adequately recog nized, perhaps because the new concepts have yet to filter into neuroscience textbooks. Nevertheless, the principles of neurotransmitter action are critical to advances in many areas of neuroscience, including molecular neurobiol ogy, neurochemistry, neuropharmacology, physiological psychology, and clinical neuroscience. It was the need for a sourcebook that prompted us to engage a group of neurophysiologists to prepare the chapters in this volume. However, there was an additional reason for this book: more and more it seemed that the field, if not yet having reached maturity, at least was ap proaching adolescence, with strengths in some areas and healthy conflicts in others. At this stage of development a textbook can help to define a field, clarify problems to be resolved, and identify areas for future investigation.

Frequently repeated experimental seizures impair brain development in an age-dependent manner. At critical ages, they delay developmental milestones, permanently lower seizure thresholds, and can cause very specific cognitive and learning deficits, such as the permanent impairment of neuronal spatial maps. Some types of experimental status epilepticus cause neuronal necrosis and apoptosis, and are followed by chronic epilepsy with spontaneous recurrent seizures, others appear relatively benign, so that seizure-induced neuronal injury and epileptogenesis are highly age-, seizure model-, and species-dependent. Experimental febrile seizures can be epileptogenic, and hyperthermia aggravates both neuronal injury and epileptogenicity. Antiepileptic drugs, the mainstay of treatment, have major risks of their own, and can, at therapeutic or near-therapeutic doses, trigger neuronal apoptosis, which is also age-, drug-, cell type-, and species-dependent. The relevance of these experimental results to human disease is still uncertain, but while their brains are quite different, the basic biology of neurons in rodents and humans is strikingly similar. Further research is needed to elucidate the molecular mechanisms of epileptogenesis and of seizure- or drug-induced neuronal injury, in order to prevent their long-term consequences.
