Inflammation in the dorsal root ganglia (DRG) in response to SCI may contribute to nociceptor dysfunction and pain development. Little is known of the immune signaling events that transpire in the DRG after SCI that could induce pathological changes in the nociceptor and drive pain. This body of work examined the role of macrophages in the DRG in the development of neuropathic pain after SCI. Macrophages were found to infiltrate the DRG early after SCI, in response to the rapid but transient elevation of a chemokine and macrophage chemoattractant, CCL2. The magnitude of increase in CCL2 differed in rats with similar injuries, and affected the inflammatory nature of the macrophage response. After pain establishment, macrophage presence was exacerbated in rats with SCI that develop pain, and DRG immune environment was found to be pro-inflammatory. Inhibiting the CCL2-mediated macrophage response prior to pain development induced pain rather than preventing it, and this was found to be due to a decrease in pro- as well as anti-inflammatory cytokines and macrophages. These studies serve as a basis for ongoing work exploring the effect of macrophage phenotype and secretome on pathological nociceptor dysfunction. Understanding the neuroimmune interactions between macrophages and nociceptors in the DRG may lead to the development of pain preventative and pain modulatory therapies.

Substances released from activated immune cells (cytokines) stimulate peripheral nerves, thereby signaling the brain and spinal cord that infection/inflammation has occurred. Additionally, peripheral infection/inflammation leads to de novo synthesis and release of cytokines within the brain and spinal cord. Thus, cytokines effect neural activation both peripherally and centrally. Through this communication pathway, cytokines such as interleukin-1, interleukin-6 and tumor necrosis factor markedly alter brain function, physiology and behavior. One important but underrecognized aspect of this communication is the dramatic impact that immune activation has on pain modulation. The purpose of this book is to examine, for the first time, immune-to-brain communication from the viewpoint of its effect on pain processing. It is aimed both at the basic scientist and health care providers, in order to clarify the major role that substances released by immune cells play in pain modulation. This book contains chapters contributed by all of the major laboratories focused on understanding how cytokines modulate pain. These chapters provide a unique vantage point from which to examine this question, as the summarized work ranges from evolutionary approaches across diverse species, to the basics of the immune response, to the effect of cytokines on peripheral and central nervous system sites, to therapeutic potential in humans.

The ENS is often referred to as the “little brain” in the gut because the ENS is more similar in size, complexity and autonomy to the central nervous system (CNS) than other components of the autonomic nervous system. Like the brain, the ENS is composed of neurons that are surrounded by glial cells. Enteric glia are a unique type of peripheral glia that are similar to astrocytes of the CNS. Yet enteric glial cells also differ from astrocytes in many important ways. The roles of enteric glial cell populations in the gut are beginning to come to light and recent evidence implicates enteric glia in almost every aspect of gastrointestinal physiology and pathophysiology. However, elucidating the exact mechanisms by which enteric glia influence gastrointestinal physiology and identifying how those roles are altered during gastrointestinal pathophysiology remain areas of intense research. This work cuts across studies of neurophysiology, somatic anthropology, contemplative practice, and mind-body medicine. Key questions include: How is body awareness cultivated? What role does interoception play for emotion and cognition in healthy adults and children as well as in different psychopathologies? What are the neurophysiological effects of this cultivation in practices such as Yoga, mindfulness meditation, Tai Chi and other embodied contemplative practices? What categories from other traditions might be useful as we explore embodiment? Does the cultivation of body awareness within contemplative practice offer a tool for coping with suffering from conditions, such as pain, addiction, and dysregulated emotion? This emergent field of research into somatic awareness and associated interoceptive processes, however, faces many obstacles. The principle obstacle lies in our 400-year Cartesian tradition that views sensory perception as epiphenomenal to cognition. The segregation of perception and cognition has enabled a broad program of cognitive science research, but may have also prevented researchers from developing paradigms for understanding how interoceptive awareness of sensations from inside the body influences cognition. The cognitive representation of interoceptive signals may play an active role in facilitating therapeutic transformation, e.g. by altering context in which cognitive appraisals of well-being occur. This topic has ramifications into disparate research fields: What is the role of interoceptive awareness in conscious presence? How do we distinguish between adaptive and maladaptive somatic awareness? How do we best measure somatic awareness? What are the consequences of dysregulated somatic/interoceptive awareness on cognition, emotion, and behavior? The complexity of these questions calls for the creative integration of perspectives and findings from related but often disparate research areas including clinical research, neuroscience, cognitive psychology, anthropology, religious/contemplative studies and philosophy.

The ongoing opioid crisis lies at the intersection of two public health challenges: reducing the burden of suffering from pain and containing the rising toll of the harms that can arise from the use of opioid medications. Chronic pain and opioid use disorder both represent complex human conditions affecting millions of Americans and causing untold disability and loss of function. In the context of the growing opioid problem, the U.S. Food and Drug Administration (FDA) launched an Opioids Action Plan in early 2016. As part of this plan, the FDA asked the National Academies of Sciences, Engineering, and Medicine to convene a committee to update the state of the science on pain research, care, and education and to identify actions the FDA and others can take to respond to the opioid epidemic, with a particular focus on informing FDA's development of a formal method for incorporating individual and societal considerations into its risk-benefit framework for opioid approval and monitoring.
