Book excerpt: This report from the Health Committee examines Lord Darzi's final report of the NHS Next Stage Review (NSR), "High quality care for all" (2008, Cm. 7432, ISBN 9780101743228). The Committee warns that primary care trusts (PCTs) are not yet capable of carrying out the reforms, and highlights insufficient analytical and planning skills and variable management quality within PCTs. Poor PCT commissioning and the failure of successive reforms to enhance it mean that implementation of the reforms may be slower and more uneven than expected. The Committee calls on the Government to publish and rigorously monitor milestones for each stage of implementation and says greater clarification is required on the role of practice based commissioning, which at present remains opaque. More emphasis needs to be placed on the importance of recruiting and developing better managers and the issue of weak management skills in this area should be tackled. Not enough detail about cost is given in the NSR and the Committee is concerned that neither Strategic Health Authorities (SHAs) nor the Department have been clear on where and how much money will be saved. Figures on implementation costs for individual SHAs and PCTs should be published as soon as possible. While the focus in the NSR on improving quality of care is welcome, variations in quality have been known about for a long time and have continued despite increased funding. The Committee is not convinced by the Department's argument that all PCTs should have a GP-led health centre and recommends that their creation should be decided locally on a case-by-case basis.

Available in PDF, EPUB and Kindle. Book excerpt: This Review sets out to propose a structure for the funding arrangements for the whole spectrum of health research, with the objective of obtaining the maximum benefit from research success and, where possible, eliminating duplication of effort. The Review found, however, that the UK is at risk of failing to reap the full economic, health and social benefits that the UK's public investment in health research should generate. There is no overarching UK health research strategy to ensure UK health priorities are considered through all types of research and there are two key gaps in the translation of health research: (i) translating ideas from basic and clinical research into the development of new products and approaches to treatment of disease and illness; (ii) implementing those new products and approaches into clinical practice. The Review also found that the wider funding arrangements for supporting translation of ideas from conception to practice could be more coherent or comprehensive and, where arrangements exist, they do not function well. The Review identified cultural, institutional and financial barriers to translating research into practice in the publicly funded research arena. But it also found that, in the private sector, the pharmaceuticals industry is facing increasing challenges in translating research into health and economic benefit. The Review has sought to make recommendations that will increase the translation of R&D into health and economic benefit for the UK, both in the public and private sectors. The Review recommends that the Government should seek to achieve better coordination of health research and more coherent funding arrangements to support translation by establishing an Office for Strategic Coordination of Health Research (OSCHR).

Available in PDF, EPUB and Kindle. Book excerpt: Mental health-substance use is a complex interrelation, with equally complex implications for individuals and their families, health professionals and society. Although its international recognition as an issue of critical importance is growing, clear guidance for professionals on mental health-substance use and its treatment is hard to find. The Mental Health-Substance Use series addresses this need, concentrating on concerns, dilemmas and concepts that impact on the life and well-being of affected individuals and those close to them, and the future direction of practice, education, research, services, interventions, and treatment. Having set the scene with the first book's Introduction, this second book in the series tackles service development: how to evaluate the current state from a firm knowledge base, plan and manage change to provide better services, and continue monitoring and evaluating them on an ongoing basis. The volumes in this series are designed to challenge concepts and stimulate debate, exploring all aspects of the development in treatment, intervention and care responses, and the adoption of research-led best practice. They are essential reading for mental health and substance use professionals, students and educators

Available in PDF, EPUB and Kindle. Book excerpt: This Handbook provides an authoritative overview of current issues and debates in the field of health care management. It contains over twenty chapters from well-known and eminent academic authors, who were carefully selected for their expertise and asked to provide a broad and critical overview of developments in their particular topic area. The development of an international perspective and body of knowledge is a key feature of the book. The Handbook secondly makes a case for bringing back a social science perspective into the study of the field of health care management. It therefore contains a number of contrasting and theoretically orientated chapters (e.g. on institutionalism; critical management studies). This social science based approach is a refreshing alternative to much existing work in this domain and offers a good way into current academic debates in this field. The Handbook thirdly explores a variety of important policy and organizational developments apparent within the current health care field (e.g. new organizational forms; growth of management consulting in health care organizations). It therefore explores and comments on major contemporary trends apparent in the practice field.
