Book excerpt: Stoughton Lake Dam is an earthfill dam with a maximum height of 23 feet and a crest length of 335 feet. The 90 feet crest length ogee spillway is located at the right abutment of the dam. The outlets works consist of an intake, a riser, a 36 inch corrugated metal pipe outlet conduit, and an outlet head wall. The riser is divided into two chambers by stop logs which can be removed to drawdown the reservoir. Hydraulic/hydrologic evaluations revealed that the spillway will pass the spillway design flood (SDF) on 53 percent of the Probable Maximum Flood (PMF) before overtopping will occur. Therefore, the spillway is assessed as being adequate. The dam was found to be in fair overall condition with the exception of the spillway training walls and the spillway discharge apron. The inspection revealed certain items of remedial work necessary for the dam which should be completed without delay by the owner.

The dam is constructed with a masonry spillway located about 35 feet left of the right abutment. The crest of the spillway is 100 feet long and about 11 feet below the top of its wingwalls. Based on a visual inspection, operational history, and available engineering data, the dam is consisered to be in poor condition. The defunct facility has been essentially abandoned since 1952 and its reservoir is sediment filled. Results of the hydrologic and hydraulic analysis indicate that the facility is capable of passing and/or storing only 36 percent of the PMF without embankment overtopping. Structural deficiencies noted by the inspection team included substantial embankment overgrowth, an access road cut through the embankment near the left abutment, cracking of the masonry spillway wingwalls, and a sediment filled reservoir. Due to the seriously inadequate spillway and overall poor condition, the facility is considered unsafe but non-emergency as failure is not considered imminent.

Results of the hydrologic and hydraulic analysis indicate that the facility can accommodate approximately 32 percent of the PMF and/or 64 percent of SDF (1/2 PMF). Thus, the spillway system is considered inadequate, but not seriously inadequate.

Observations made during the visual inspection suggest that the embankment may be structurally unsafe as extensive areas of seepage were noted on the downstream embankment slope and immediately below the toe area to the right of the outlet conduit. Seepage conditions became evident shortly after reservoir filling and have persisted in spite of the installation of a remedial sand drain system near the toe. Failure of the embankment does not appear imminent and the condition is presently considered non-emergency. It is recommended that the owner immediately develop and implement a warning system to notify downstream inhabitants should hazardous conditions develop. Included in the plan should be provisions for around-the-clock surveillance of the facility during periods of unusually heavy precipitation.

Based on the evaluation of available design information and visual observations of conditions as they existed on the dates of the field reconnaissances, the general condition of Laurel Hill Lake Dam is considered to be good. However, the cause and origin of a seepage zone located downstream of the dam embankment could not be conclusively established by visual observation and review of the construction drawings. Therefore, it is recommended that periodic monitoring of the seep be made by the dam owner. The presence of eroded footpaths, animal burrows, cracking and spalling on concrete surfaces, and shallow depression located on the dam crest, are considered minor deficiencies in need of maintenance. Guideline criteria recommend a PMF spillway design flood for intermediate size, high hazard dams. Spillway discharge capacity was found to be seriously inadequate based on the following data: (1) Maximum non-overtopping spillway discharge capacity is 34 percent PMF, (2) Failure of the dam resulting from 37 percent PMF overtopping significantly increases the downstream loss of life and damage potential compared to that which would exist prior to dam failure. Laurel Hill Lake Dam is categorized as unsafe, non-emergency in accordance with recommended criteria.

Book excerpt: Star Junction No. 2 Dam consists of an earthfill embankment and masonry spillway on the left abutment. The dam has a crest length of 341 feet, a maximum height of 30 feet and a storage volume of 31 acre-feet at the spillway crest level. The dam is classified by Corps of Engineers guidelines to be a small size, high hazard structure. Star Junction No. 2 Dam's embankment is assessed to be in poor condition. This is based on visual observations of the non-uniform slope, wet zones and state inspection reports. The downstream control was observed to be damaged and probably is not operable. No control or mechanism was observed to permit upstream flow control. The principal spillway is assessed to be in very poor condition. This is based on visual observations of the weir wall and training dike retaining wall. Both structures are partially collapsed and badly deteriorated and safe performance is questionable in the event of long-term, high discharge conditions.

Book excerpt: Lake Jo-Ann Dam, formerly known as Bracco Dam, is classified as a 'High' hazard - 'Small' size dam. The dam and reservoir, owned by Mr. Daniel Carapellucci, Sr., are used for recreational purposes. The dam was found to be in fair overall condition at the time of the inspection. Hydraulic/hydrologic evaluations, performed in accordance with procedures established by the Baltimore District, Corps of Engineers, for Phase I Inspection Reports, revealed that the spillway will pass 27.5 percent of the Probable Maximum Flood (PMF) before overtopping of the dam occurs. A spillway design flood (SDF) in the range of the 1/2 Probable Maximum Flood (1/2 PMF) to the PMF is required for Lake Jo-Ann Dam. Because the dam is on the low end of the 'Small' size category, the 1/2 PMF was selected as the SDF. It was determined that during the 1/2 PMF the embankment would be overtopped by a maximum depth of 0.89 foot and a total duration of 5.00 hours. Because a limiting criteria of one foot or greater of overtopping and duration in excess of 2 hours was estimated for this dam, it was concluded that a dam failure during the 1/2 PMF is unlikely to occur.

Book excerpt: The general condition of the Refuse Settling Basin is considered to be fair. This assessment is based on visual observations that indicate a possible inadequate margin of safety against slope failure of a high ground water level exists in Embankment A. The condition is not considered to represent an immediate threat to the structure because of the relatively low impoundment pool elevation. It should, however, be evaluated. If a high ground water does not exist in Embankment A, the facility can be considered to be in good condition. The structure is classified as an 'intermediate' size, 'high' hazard dam. Corps of Engineers guidelines recommend the Probable Maximum Flood (PMF) as the Spillway Design Flood for an 'intermediate' size, 'high' hazard dam. The visual inspection indicated several minor deficiencies in addition to the possible high ground water condition. The deficiencies can be corrected or improved through implementation of the following recommended evaluation, remedial, monitoring and/or maintenance efforts.

Butler's Lake Dam is classified as a 'high' hazard dam. Butler's Lake Dam is owned by Butler's Golf Course, Elizabeth, Pennsylvania 15037. The field inspection of the Butler's Lake Dam was performed on 11 December 1980, and consisted of: Visual observations of the embankment crest and slopes, groins and abutments; Visual observations of the principal (and emergency) spillway including approach and discharge channels and overflow crest; Visual observations of portions of the outlet works intake structure and conduit; Visual observations of the embankment's downstream toe area including drainage channels and surficial conditions; Visual observations of the reservoir shoreline, inlet stream channel, and watershed; Visual observations of downstream conditions and evaluation of the downstream hazard potential; Transit stadia surveys of relative elevations along the embankment crest centerline, spillway, and across the embankment slopes. The visual observations were made during periods when the reservoir was at normal operating level. There was no defined tailwater.
