Book excerpt: In Britain, between 1980 and 1998, the number of people classified as obese tripled to 21per cent of women and 17 per cent of men. It is estimated that this costs the economy, as a whole, �2 billion and the NHS �0.5 billion in treatment. However the response of the NHS is patchy, with no national guidelines and only 28% of health authorities taking action to address the problem. There is little activity related to the management of obesity outside of general practice but only a small proportion of GPs follow a protocol. This report recommends that there should be strategies to reduce obesity and that the Department of Health should build on the plan in the National Service Framework on coronary heart disease and work with partners and professional bodies to clarify responsibilities. It should also work with the National Institute of Clinical excellence to disseminate information. The Department of Health should also lead a cross government strategy to promote the benefits of physical activity and there should be local targets to encourage cycling and walking. It should also work with the food industry to improve the balance of diet. Much of this work needs to be aimed at school children to promote a healthy lifestyle throughout life and guidance to schools on commercial sponsorship should be strengthened so that there is no conflict with messages on healthy eating.

Book excerpt: The Charity Commission is not regulating charities effectively and there is a gap between what the public expects of the Commission and what it actually does. The NAO has concluded that the Commission does not do enough to identify and tackle abuse of charitable status. Between 2007-08 and 2013-14, the Commission's annual budget fell 40 per cent in real terms to £22.7 million but the number of main registered charities has remained fairly constant at around 160,000. In response to budget cuts, the Commission has reviewed how it works and successfully reduced demand for its services, but it has not identified what budget it would need to regulate effectively. The Commission makes little use of its enforcement powers, for example suspending only two trustees and removing none in 2012-13. And it can be slow to act when investigating regulatory concerns. The NAO found cases where periods of several months passed during which the Commission took no action. Furthermore, the Charity Commission does not take tough enough action in some of the most serious regulatory cases. It is also reactive rather than proactive, making insufficient use of the information it holds to identify risk. The Charity Commission needs to think radically about alternative ways of meeting its objectives with constrained resources. It also needs to make greater use of its statutory powers in line with its objective of maintaining confidence in the sector; and develop an approach to identify and deal with those few trustees who deliberately abuse charitable status. This report publishes alongside another NAO report, the Cup Trust.

Better price certainty and on-time delivery of good quality assets have been obtained by using PFI contracts. Only 22 per cent of public building projects had exceeded the cost initially expected by the public sector. Under previous contracts up to 73 per cent had overshot the original estimate. Only eight per cent of projects were delayed by more than two months. Public sector project managers were generally satisfied with the design, construction and performance of their PFI buildings. However, NAO did not try to judge whether PFI was the best procurement method for all public sector construction projects.

Book excerpt: The Department for Environment, Food and Rural Affairs and its Animal Health agency successfully contained limited outbreaks of Avian Influenza and Foot and Mouth Disease in 2007. The estimated £33 million expenditure by Animal Health in 2007-08 on dealing with these exotic disease outbreaks has represented good value for money when compared to the economic costs of these diseases becoming more widespread. The control of some of the more serious endemic diseases has been managed less successfully. Good progress has been made with the control of Bovine Spongiform Encephalopathy (BSE), Scrapie and Salmonella, but Bovine Tuberculosis has continued to spread. In 2007-08, tackling Bovine Tuberculosis accounted for 39 per cent of Animal Health's total expenditure. Herd restrictions are applied immediately when disease is identified, but compliance with the requirements for routine testing to detect disease is not rigorously enforced. There are no national standards on farm biosecurity to minimise the risk of diseases spreading. The Department, Animal Health and other inspection bodies, such as local authorities, do not systematically collect and share information about biosecurity risks. Beekeepers have reported unusually high losses of honeybees in recent years and, now that the Varroa parasite is endemic, honeybee colonies are more vulnerable to other diseases. Controlling Varroa and monitoring of other diseases is hampered by the limited inspections of colonies carried out by the Department's National Bee Unit. An estimated 20,000 beekeepers are not known to the Unit's inspectors and are less likely to notify the Department of any diseases.

Book excerpt: Transport aircraft are vital for both strategic and tactical tasks to support national security objectives, military exercises and training, and humanitarian aid. Despite a decreasing fleet size, and an increasing need for maintenance and upgrades, the Ministry of Defence is meeting the requirement for Hercules aircraft to transport military personnel and freight in Iraq and Afghanistan. Hercules aircraft are available to fly on planned missions 85 per cent of the time. This report found increased stress on the aircraft has been caused by landing on unpaved airstrips in Iraq and Afghanistan, additional use of air drops, as well as a change from transporting people and equipment over long distances to making short flights in theatre. More wear and tear' has resulted, and increased maintenance costs. Fatigue, which decreases the life span of the wings, is accumulating more rapidly than in the past. The Department has had to retire four aircraft during 2006 and plans to retire a further five C-130Ks during this year, ahead of their planned retirement date of 2010. The Department will also need to address shortened wing life on the newer C-130J. The initial estimate for the number and type of spares for the C-130J was inaccurate, leading to some spares shortages. Whilst the shortages have not been allowed to affect aircraft deployed in Afghanistan or Iraq, on average Hercules C-130J have been unable to fly for 24 days a year for lack of parts. The Department has a shortage of engineering staff, and the RAF estimates that additional personnel would give increased aircraft availability. The focus on operations has meant there are fewer aircraft available for training. Aircrew on operations in Iraq and Afghanistan fly on average 60 hours per month, as opposed to 13 hours for aircrew based in the UK. Simulators are not able to replicate fully flying the Hercules C-130, as they have not kept pace with modifications and upgrades to the aircraft. As a result, there is a danger that the broader skills base of crews could be eroded in the future. There are also significant risks to the future ability of the Department to provide sufficient airlift to the armed forces. These include the withdrawal from service of the Hercules C-130K, the late delivery of the planned A400M transport aircraft, and a reduction in the anticipated service life of the C-130Js wings from flying in more challenging environments during operations. These issues could leave the RAF with a shortage of fixed wing aircraft.

Book excerpt: The 2001 outbreak of Foot and Mouth was set out in a report published in 2002, providing an analysis of what happened with recommendations (HCP 888, session 2001-02, ISBN 9780102976243). Another FMD outbreak occurred in the Summer of 2007 and the Government again asked Dr Iain Anderson to conduct a review and find out if the lessons of 2001 had been learned and whether new recommendations are needed. This report is built around the findings of 2001, with a particular look at the Pirbright facility, the source of the virus incriminated in this recent outbreak. Dr Anderson's states that he found much to applaud in the handling of the 2007 outbreak along with some deficiencies, but states the positive outweighs the negative. The report commends the strong leadership of the Prime Minister, as well as the contingency planning and better integration of scientific advice and the capabilities at the centre of disease control strategies. External communication was also greatly improved. Less encouraging were the inadequate infrastructure and governance of the Pirbright facility where the Institute for Animal Health is located and the poor regulatory regime in place to licence work with dangerous animal pathogens. Defra's information systems too were found wanting. The report also recommends that the Institute of Animal Health should become the new National Institute of Infectious Diseases, supported by multiple sources of funding from government and elsewhere. Also recommended is the creation of an Independent Advisory Committee on Animal and Emerging Infectious Diseases, combining a high level group of experts along with other stakeholders with a mission to provide independent advice to government and link closely with the Government's and departmental Chief Scientific Advisors and include the Chief Veterinary Office and Chief Medical Officer amongst its members. The publication is divided into five sections: Section 1: Foreword; Section 2: Summary and recommendations; Section 3: Lessons learned, the lessons are - Lesson 1: Maintain vigilance; Lesson 2: Be prepared; Lesson 3: React with speed and certainty; Lesson 4: Explain policies, plans and practices; Lesson 5: Respect local knowledge; Lesson 6: Apply risk assessment and cost benefit analysis; Lesson 7: Use data and information management systems; Lesson 8: Have a legislative framework; Lesson 9: Base policy decisions on the best available science; Section 4: Pirbright; Section 5: Appendices.
