Book excerpt: This book offers an updated review of malignant mesothelioma, including the latest advances in our understanding of its genetic control and molecular biology, as well as pre-clinical and clinical research. It also presents state-of-the-art diagnostic approaches and therapeutic options, and an open discussion on the future prospects for patient management. Malignant mesothelioma is an enormous global health problem related to asbestos exposure. Despite the best efforts of scientists and oncologists, the prognosis for those affected remains poor. Due to anatomical characteristics and non-specific symptoms, the diagnosis of mesothelioma at an early stage is often difficult, while surgery and radiotherapy are only of limited use, even if some multimodality approaches seem promising. In turn, medical treatments are sometimes successful in tumor control, but have little impact on overall survival. However, advances in our understanding of the disease’s biology, together with the availability of new drugs and combinations, make mesothelioma an essential and highly topical field for pre-clinical and clinical studies. This book is subdivided into four parts: epidemiology and preclinical data, diagnosis, therapy, and extrathoracic mesothelioma. It highlights the progress made in a variety of areas – e.g. in vitro and in vivo experimental models, genetics, environment, biomarkers, targeting agents, immunotherapy, metabolic imaging and ongoing clinical trials – and describes the standard clinical management of mesothelioma patients, including those with extra-thoracic localizations. Given its scope, the book offers an invaluable tool for researchers, oncologists and clinicians alike.

Available in PDF, EPUB and Kindle. Book excerpt: One reason for failure to cure solid tumors by surgery appears to be the impossibility of controlling metastases that are present but latent at the time of operation. This failure is a common clinical experience with aggressive neoplasms. but it is not always appreciated in tumors with longer survival times. e. g .• breast and colon cancer. In addition. recent evidence indicates that after resection of a primary tumor micrometas tases from it might be enhanced by suppression of immune and reticu loendothelial functions of the host. Other factors, such as increase of coagulability and stress in the perioperative period, can also promote tumor growth. The development of new metastases might be facilitated by cells forced into the circulation during operative manipulations. Such events could be important for the outcome of treatment and it is suggested that preventive measures should be directed to this systemic component of solid tumors. Radical surgery can reduce the number of tumor cells to a subclinical 3 6 stage (10 to 10 cells) in which chemotherapy might be more effective than in advanced stages. Chemotherapy, on the other hand, might aggravate the surgical morbidity by influencing the wound healing pro cess, by decreasing the immune response, and/or by toxicity to the bone marrow and to the gastrointestinal tract, for example.

Book excerpt: "Mesothelioma is a type of cancer in which tumors form in the tissue that lines the lungs, heart, stomach, and other organs. While mesothelioma is relatively rare, it represents a serious health crisis as the disease is aggressive and often fatal. This book includes five chapters that detail the treatment and prognosis of the disease as well as the factors that put a person at risk for developing mesothelioma. Chapter One describes the most promising emerging immunotherapy combinations for the treatment of malignant pleural mesothelioma, discussing the biological rational underlying their development as well as the issues surrounding their clinical development. Chapter Two provides an overview of malignant mesothelioma, describing the history of its diagnosis and treatment as well as the role that asbestos exposure and other factors play in its development. Chapter Three revises the role of the main inflammatory and immunological markers as prognostic indicators in malignant pleural mesothelioma and discusses the surgical effects on immune response against cancer. Chapter Four explores the main cancer markers utilized for mesothelioma diagnosis as well as the new developments in the field. Lastly, Chapter Five examines the various surgical options available for removing pleural tumors"--

Book excerpt: This book ventures into a new and exciting area of discovery that directly ties our current knowledge of cancer to the discovery of microorganisms associated with different types of cancers. Recent studies demonstrate that microorganisms are directly linked to the establishment of cancers and that they can also contribute to the initiation, as well as persistence of, the cancers. Microbiome and Cancer covers the current knowledge of microbiome and its association with human cancers. It provides important reading for novices, senior undergraduates in cancer and microbiology, graduate students, junior investigators, residents, fellows and established investigators in the fields of cancer and microbiology. We cover areas related to known, broad concepts in microbiology and how they can relate to the ongoing discoveries of the micro-environment and the changes in the metabolic and physiologic states in that micro-environment, which are important for the ongoing nurturing and survival of the poly-microbial content that dictates activities in that micro-environment. We cover the interactions of microorganisms associated with gastric carcinomas, which are important for driving this particular cancer. Additional areas include oral cancers, skin cancers, ovarian cancers, breast cancers, nasopharyngeal cancers, lung cancers, mesotheliomas, Hodgkin’s and non-Hodgkin’s lymphomas, glioblastoma multiforme, hepatocellular carcinomas, as well as the inflammatory response related to the infectious agents in cancers. This book covers the metabolic changes that occur because of infection and their support for development of cancers, chronic infection and development of therapeutic strategies for detection and control of the infection. The field of microbiome research has exploded over the last five years, and we are now understanding more and more about the context in which microorganisms can contribute to the onset of cancers in humans. The field of microbiome research has demonstrated that the human body has specific biomes for tissues and that changes in these biomes at the specific organ sites can result in disease. These changes can result in dramatic differences in metabolic shifts that, together with genetic mutations, will produce the perfect niche for establishment of the particular infection programmes in that organ site. We are just beginning to understand what those changes are and how they influence the disease state. Overall, we hope to bring together the varying degrees of fluctuations in the microbiome at the major organ sites and how these changes affect the normal cellular processes because of dysregulation, leading to proliferation of the associated tissues.

Book excerpt: Pathology of Asbestos-Associated Diseases integrates the newest research and advances in its discussion of asbestos-induced diseases. Organized in 13 chapters, the book begins with background information on the mineralogy of asbestos, occupation and environmental exposure to asbestos, and asbestosis. A detailed discussion on mesothelioma includes a historical background, addresses etiologic considerations and epidemiology, pathologic features, differential diagnosis and treatment and prognosis. Guidelines for assessing and identifying asbestos-associated carcinomas of the lung, including histopathology and differential diagnosis, are discussed. The text also addresses the explosion of research in the area of molecular biology and the abundance of information bearing on the mechanisms by which asbestos causes disease. Chapters also cover cytopathology and experimental models of asbestos-related diseases. The diseases associated with asbestos have significant medico-legal implications, and there are two chapters dedicated to addressing the issue. More than 190 illustrations complement the text. An invaluable reference for the pathologist, pulmonologist, radiologist, and occupational medical practitioner interested in asbestos-related disease as well as malpractice attorneys needing an understanding of asbestos, the diseases it induces, prognosis and clinical implications.
