Dans un contexte d’accroissement de l’espérance de vie et des maladies chroniques, notre objectif principal était de connaitre les difficultés des infirmiers et médecins généralistes libéraux dans leur prise en charge des patients palliatifs au domicile.Matériel et méthode : Un questionnaire électronique a été soumis à des médecins généralistes et infirmiers libéraux afin de connaitre leur pratique des soins palliatifs, leurs difficultés et motivations. Les résultats ont été analysés et comparé statistiquement entre les groupes ruraux et urbains. Résultats : Nous avons recueilli 278 réponses, dont 165 IDE et 113 MG. Ils montraient un très fort intérêt pour cette pratique malgré des difficultés souvent partagées par les soignants : en premier le manque d’implication de la famille ou des proches du patient, puis l’impact sur la vie professionnelle et personnelle des soignants. La quatrième difficulté exprimée est le manque d’une rémunération adaptée, puis en cinquième les connaissances personnelles en soins palliatifs. En fin de classement se trouvent le manque de collaboration avec d’autres professionnels de santé et les soins en rapport avec une pathologie spécifique. Ces résultats sont globalement comparables entre soignants du secteur urbain ou rural. Conclusion : L’hospitalo centrisme de la prise en charge palliative des derniers instants de la vie est une réalité que la plupart des français voudraient éviter. Le manque d’implication de la famille ou des proches est la première cause de difficulté des intervenants libéraux, qui par leur connaissance du patient et de sa famille, sont des acteurs importants de cette prise en charge

Dans une société où le maintien à domicile est un enjeu primordial, il paraît intéressant de trouver des solutions pour le faciliter. L'objectif de cette recherche est d'étudier le vécu et les besoins des soignants libéraux face aux situations palliatives à domicile puis d'évaluer leur collaboration avec l'équipe mobile de soins palliatifs. Méthodes : étude qualitative à partir d'entretiens individuels semi-dirigés, réalisés auprès de médecins et infirmiers libéraux exerçant dans le secteur d'activité de l'EMSP du CHICAS de Gap, puis analyses par la méthode de théorisation ancrée. Résultats : concernant les médecins, ils décrivent une volonté de rester au cœur de la prise en charge de leur patient, avec qui ils ont un lien affectif souvent marqué. Ils évoquent des difficultés pour aborder la question du soin palliatif. La plupart sollicite l'EMSP pour un avis expert, mais la contrainte de temps et le manque de connaissances nuisent à l'anticipation de son intervention. De plus, la coordination entre tous les acteurs et l'augmentation du nombre d'interlocuteurs est une crainte pour eux. Pour les IDEL, les situations palliatives nécessitent une forte implication émotionnelle. Leur rôle paraît central, mais présente de multiples facettes source de questionnement. Ils expriment certaines frustrations et un manque de reconnaissance. Les IDEL se sentent régulièrement confrontés à l'entourage qui représente parfois l'unique difficulté. Le sentiment de solitude, la communication avec l'hôpital et le fait d'aborder les soins palliatifs sont vécus comme difficile. L'EMSP permet de les soutenir mais les IDEL font parfois face à une réticence de la part du médecin traitant. Discussion : anticiper les soins palliatifs et se former à la démarche palliative s'avèrent être des clefs pour éviter la survenue de difficultés en libéral et ainsi optimiser le maintien à domicile des patients. Cependant, la coordination, l'interdisciplinarité et la non-substitution sont des enjeux majeurs pour mettre en confiance les médecins généralistes les plus réticents. Des perspectives d'amélioration concrètes et facilement applicables sur le terrain sont à exploiter.
