Book excerpt: Les cardiopathies congénitales et acquises sont les pathologies les plus fréquentes chez l'enfant (6500 à 8000 enfants par an en France) et la première cause de mortalité infantile. Grâce aux progrès de la chirurgie cardiaque et du cathétérisme interventionnel, le nombre d'enfants vivants avec une cardiopathie augmente. Cela nécessite alors des interventions dès le plus jeune âge et un suivi médical régulier et adapté, notamment sur le plan dentaire (vis à vis du risque infectieux, hémorragique et syncopal majoré). Objectif : (1) décrire l'état de santé bucco-dentaire des enfants présentant une cardiopathie. (2) Comparer la santé bucco-dentaire des enfants porteurs de cardiopathie congénitale ou acquise avec celle des enfants indemnes de cardiopathie. Matériels et méthodes : une revue systématique de la littérature a été réalisée selon les recommandations PRISMA. Les bases de données MEDLINE (via PubMed) et Embase ont été consultées. Une sélection a été faite tenant compte de questions de recherches bien définies et de critères d'inclusions et d'exclusions bien établis. Une évaluation des risques de biais a été réalisée sur l'ensemble des études. Résultats et discussion : 22 articles ont été retenues. La majorité des études ont mis en évidence un réel manque d'hygiène chez ces enfants non compatibles avec leur pathologie au vu du risque infectieux. Conclusion : les enfants porteurs de cardiopathie ne sont finalement pas plus vulnérables par rapport aux enfants indemnes de cardiopathie face aux atteintes bucco-dentaire. Cependant, ils ne sont pas suffisamment sensibilisés sur l'importance d'une hygiène bucco-dentaire irréprochable. Ce manque d'information est directement en lien avec un réel manque de prise en charge et d'information des professionnels de santé. D'autres études devront être menées sur la place des professionnels de santé dans la prise en charge de ces patients.

Le contrôle de la plaque dentaire et une bonne santé buccodentaire permettent de réduire le risque d'endocardite infectieuse. Les objectifs du présent projet de recherche visent à évaluer les connaissances des parents d'enfants atteints de cardiopathies congénitales sur l'endocardite infectieuse et son lien avec la santé buccodentaire et de connaître les habitudes d'hygiène orale personnelles et professionnelles adoptées par les enfants atteints de cardiopathies congénitales. Le projet de recherche vise également à évaluer l'efficacité de deux méthodes d'enseignement d'hygiène orale chez les enfants atteints de cardiopathies congénitales. La procédure expérimentale implique que tous les parents ou gardiens légaux d'enfants atteints de cardiopathies congénitales, âgés entre 6 et 12 ans qui visitent le service de cardiologie du CHU Sainte-Justine sont sollicités à participer au projet de recherche. Un formulaire d'information et de consentement ainsi qu'un questionnaire sont remis aux parents. Le questionnaire vise à évaluer la connaissance des parents d'enfants atteints de cardiopathies congénitales sur ce qu'est l'endocardite infectieuse et son lien avec la santé buccodentaire ainsi que de connaître les habitudes d'hygiène orale personnelles et professionnelles des enfants atteints de cardiopathies congénitales. L'examen clinique nécessaire au projet de recherche implique le prélèvement d'un indice de plaque Quigley & Hein, Turesky modifié avant et après que l'enfant participant au projet de recherche ait appliqué les instructions d'hygiène orale reçues. L'enfant est assigné à l'une des deux méthodes d'instructions d'hygiène orale avec l'aide d'une table de randomisation. La méthode d'instructions d'hygiène orale du groupe 1 correspond à des instructions transmises par le cardiologue tandis que la méthode d'instructions d'hygiène orale du groupe 2 correspond aux instructions transmises par l'intermédiaire d'un document audio visuel. Des analyses chi-carré et des tests de T pairé ainsi que des analyses de variance univariée (one-way ANOVA) et des analyses de corrélation de Pearson entre le questionnaire et les données cliniques ont été effectuées pour analyser les données recueillies. Les résultats démontrent que les parents d'enfants « à risque élevé » d'effet adverse d'une endocardite infectieuse ne connaissent pas davantage le risque d'endocardite infectieuse d'origine buccodentaire que les parents d'enfants « de moindre risque » (p=0,104). Les résultats démontrent toutefois que les parents d'enfants atteints de cardiopathies congénitales qui connaissent le risque d'endocardite infectieuse et son lien avec la santé buccodentaire adhèrent à des comportements dans le but de maintenir une bonne santé buccodentaire chez leur enfant. Les résultats qui proviennent de l'examen clinique démontrent que l'application des instructions d'hygiène orale faites par le cardiologue et par l'intermédiaire d'un document audio visuel permettent d'observer une différence statistiquement significative (p=0,000) au niveau du contrôle de la plaque dans chacun de ces groupes. Toutefois, aucune différence statistiquement significative (p=0,668) n'a pu être démontrée entre les deux méthodes d'instructions d'hygiène orale. Les parents qui connaissent le lien entre la santé buccodentaire et le risque d'endocardite infectieuse pour leur enfant atteint de cardiopathie congénitale adoptent un comportement pour optimiser la santé buccodentaire de leur enfant. Les instructions d'hygiène orale par l'intermédiaire d'un document audio visuel sont équivalentes aux instructions d'hygiène orale prodiguées par le cardiologue.

The report makes extensive use of the Iraq Household Socio-Economic Survey, the first nationwide income and expenditure survey since 1988. IHSES data is complemented income and expenditure data from a wide range of other measures of living standards, allowing us to analyze living standards in a holistic way. The analysis presented here was performed with two main goals first, to inform the Government s Poverty Reduction Strategy; and second, to serve as a baseline for future assessments of changes in living standards and the identification of critical issues for deeper examination. Iraqi living standards have two unusual characteristics. First, they have fallen over the past generation. Second, they feature surprisingly little inequality. These characteristics are both rooted in Iraq s recent history of authoritarian government, war, military occupation, insurgency, and civil strife leading to infrastructure destruction and population displacement. There have been few opportunities for individuals to prosper from professional or entrepreneurial activities. Decades of neglected investment have resulted in deterioration of social services and economic infrastructure. Consequently, individuals have lacked capabilities to prosper and an investment climate conducive to prosperity. School enrollment and life expectancy have declined. Extremely low returns to education reflect the combination of poor educational quality and lack of employment opportunities. In terms of economic infrastructure, access to reliable electricity and water, and even access to paved roads are low, are further reflections of decades of neglect. While the upper end of the distribution has been pulled down by a lack of opportunities, the lower end has been supported by direct government provision of food. The Public Distribution System (PDS) provides 85 percent of food needs. While PDS has been useful as a safety net for the poor and the vulnerable, the system is expensive, inefficient, and fiscally risky. Indeed, PDS food rations account for a far greater share of public spending than does education or health. Going forward, Iraq faces two main challenges. First, although Iraq does not have to develop from scratch, it faces a formidable challenge in re-development. Second, a shift by the Government is required from direct provision of basic subsistence toward investment in human capacities. The Government can provide an enabling environment through investments in economic infrastructure and services to business and citizens, thus allowing the population to make productive use of education and their own labor. Both challenges are now being taken up by the Poverty Reduction Strategy, which articulates a detailed set of required actions and outlines priorities for government spending.
