Peu de centres en France disposent de nos jours de l'équipement nécessaire pour ces explorations, du fait de leur coût, de leur taille nécessitant des locaux dédiés, des compétences médicales requises et de l'absence de valorisation tarifaire. Pourtant, ces explorations sont le plus souvent essentielles pour établir le bon diagnostic et proposer un traitement adapté. La rareté des données bibliographiques sur les explorations photobiologiques confirment le faible investissement des dermatologues dans un domaine clinique pourtant majeur. OBJECTIFS : Notre étude rétrospective a recueilli les résultats des explorations photobiologiques d'une série de 100 cas grenoblois. L'objectif principal de notre travail était d'évaluer la pertinence des explorations photobiologiques pour aider à poser un diagnostic de photodermatose. Les objectifs secondaires étaient d'étudier différentes données épidémiologiques et cliniques des photodermatoses et les caractéristiques des patients. MATERIELS et METHODES : Il s'agit d'une étude épidémiologique descriptive, rétrospective et mono centrique qui a inclus 100 patients ayant bénéficié d'explorations photobiologiques entre juillet 2014 et décembre 2018 dans le service de dermatologie, d'allergologie et de photobiologie du CHU Grenoble Alpes (centre de référence Rhône Alpes Auvergne en photobiologie). Les explorations photobiologiques ont été effectuées à l'aide d'une lampe Waldmann UV 802 L, et d'une lampe UVA1 haute pression Waldmann 700L. Elles ont comporté le calcul de la dose érythémateuse minimale UVA et UVB, les phototests itératifs en UVA et UVB, et des photopatchs tests (batterie photoallergènes et parfois batterie anti-inflammatoires non stéroïdiens), selon les données de l'interrogatoire. Pour des raisons d'éloignement géographique, les explorations photobiologiques étaient le plus souvent programmées sur les données du courrier, sans consultation préalable. RESULTATS : Sur les 100 patients, 98 dossiers étaient exploitables (2 interruptions de l'exploration). Les tests photobiologiques ont permis de poser le diagnostic de photodermatoses dans 59 cas (60%), et de l'éliminer formellement dans 13 cas (13%). Dans un quart des cas, le diagnostic restait indéterminé (26 cas). Ont été ainsi diagnostiqués 13 photosensibilisations de contact (12.7%), 10 lucites polymorphes (9,8%), 10 urticaires solaires (9.8%), 9 lucites estivales bénignes (8.8%), 8 eczémas atopiques photoaggravés (7.8%), 5 dermatites actiniques chroniques (4.9%), et 2 photosensibilisations par voie systémique (1.9%). Parmi les photosensibilisations de contact, 9 patients avaient une photoallergie aux anti-inflammatoires non stéroïdiens (9,1% de toutes les explorations); 3 patients avaient une photoallergie aux filtres solaires, 2 patients avaient une photoallergie au fragrance mix. Lorsque le diagnostic de photodermatose a été éliminé, les diagnostics évoqués alors étaient eczéma atopique, eczéma irritatif, eczéma de contact allergique, rosacée, dyshidrose, urticaire non solaire. Les tests photodermatologiques permettant le diagnostic étaient effectués en moyenne 7,5 ans après l'apparition des premiers symptômes. Les patients explorés étaient le plus souvent des femmes (58.1%); tous les âges étaient représentés, le plus jeune patient étant âgé de 7 ans. Les patients étaient adressés par des confrères dermatologues, allergologues, médecins généralistes et exceptionnellement par d'autres spécialistes. DISCUSSION/ CONCLUSION : Les résultats de notre étude objectivent que les explorations photobiologiques se révèlent très utiles, permettant de poser un diagnostic dans la majorité des cas chez des patients bien souvent en errance diagnostique. Dans notre série, les photosensibilisations de contact sont les photodermatoses les plus fréquentes, et sont dominées par les photoallergies aux anti-inflammatoires non stéroïdiens, principalement au kétoprofène, ce qui rejoint les données de la littérature. La fréquence des lucites estivales bénignes est sous représentées du fait que les formes typiques, les plus fréquentes, ne nécessitent pas d'explorations photobiologiques. Les dermatites actiniques chroniques représentent un pourcentage non négligeable (4.9%), nuançant avec le caractère exceptionnel souvent rapporté de cette pathologie. Ainsi notre étude souligne l'importance de la réalisation de tests photobiologiques par une équipe experte pour préciser le diagnostic des dermatoses photo déclenchées et/ou photo aggravées. Ceci dans le but d'optimiser le traitement de ces dermatoses souvent très invalidantes au quotidien pour les patients.

One of the main goals of immunogenetic studies is finding susceptibility genes for complex diseases. This can provide an insight into the pathogenesis of the condition in a way that is not easily achievable through other kinds of studies. Thus they are a rational initial step for generating hypotheses about disease pathogenesis. This may especially benefit dermatology, a field notorious for having too many diseases with unknown etiologies. Immunogenetic investigations have made targeted treatment strategies possible for diseases such as psoriasis and pemphigus. Even though these strategies have revolutionized the management of chronic dermatological conditions such as psoriasis, still there are a lot of unanswered questions. For instance, psoriasis patients respond very differently to each of the commercially available biological agents. This diversity could be partially explained by the differences in the sets of genes responsible for disease induction in each individual. Thus whole genome sequencing strategies, if feasible at individual levels, might help in tailoring these targeted treatments based on specific genetic backgrounds. Our intention in preparing this book was to explore the broad spectrum of the genetic aspects of immunological processes involved in cutaneous diseases. We have tried to cover most areas of dermatology where enough studies were available to gather a chapter. Still, there is a substantial lack of knowledge on the immunogenetics of many dermatological conditions. We hope that this book would encourage the investigators to fill these gaps of knowledge.
