Dans un contexte de difficultés d'accès aux soins pour la population dans certains bassins de vie, avec des médecins généralistes libéraux de moins en moins nombreux et inégalement répartis sur le territoire, l'objectif de notre étude était de créer une nouvelle cohorte de jeunes médecins généralistes, diplômés de l'UFR Santé de l'Université de Rouen, et d'analyser les déterminants de leur exercice professionnel au fil des années, afin de décrire leurs attentes, de comprendre ce qui motive leurs parcours professionnels et de proposer des solutions adaptées pour améliorer le système de soins ambulatoires. Méthode Il s'agit de la création d'une cohorte prospective, constituée de 68 jeunes médecins généralistes ayant débuté leur Troisième Cycle des Etudes Médicales en novembre 2014 à la faculté de médecine de Rouen. Les informations concernant leurs données socio-démographiques, leurs parcours scolaires dans l'enseignement secondaire et supérieur, leurs pratiques médicales actuelles et leurs projets professionnels envisagés dans l'avenir, ont été recueillies par un questionnaire électronique envoyé de janvier à juillet 2018. Résultats Le taux de réponses était de 73%. La population était âgée de 30 ans, avec 66% de femmes, de parents majoritairement cadres. La médecine générale était le premier choix pour 76% des répondants. 84% avaient fait un SASPAS. 16% avaient fait un DESC. 60% avaient remplacé pendant l'internat. 30% avaient passé leur thèse. En novembre 2017, 60% pratiquaient exclusivement la médecine générale, 20% étaient encore internes, 14% pratiquaient exclusivement une autre activité et 3% avaient une activité mixte. Les déterminants du type d'exercice étaient la réalisation d'un SASPAS, les remplacements au cours de l'internat et la soutenance précoce de la thèse. Les déterminants du lieu d'exercice étaient le choix de la médecine générale en priorité à l'ECN et le lieu de réalisation du SASPAS. Conclusion Notre travail a permis de faire émerger des déterminants des parcours de ces jeunes médecins généralistes. Il s'inscrit dans la continuité des travaux de création et de suivi de cohortes de jeunes médecins généralistes dans différentes régions françaises. Notre cohorte devra faire l'objet d'un suivi afin de confirmer de façon factuelle les déterminants mis en évidence et d'en dégager de nouveaux.

L’objectif était de construire une cohorte de jeunes médecins généralistes Hauts-Normands. Elle complètera les données issues d’autres études régionales. Méthode : Création d’une cohorte prospective. Inclusion de 70 jeunes médecins généralistes ayant débuté leur troisième cycle à la faculté de médecine de Rouen en novembre 2012. Un questionnaire électronique leur a été adressé. Données recueillies entre novembre 2015 et janvier 2016. Résultats : Taux de réponse 87,5%. Population âgée de 28,48 ans en moyenne, féminine à 67%, de parents essentiellement cadres. La médecine générale était leur premier choix aux ECN à 60%. 85% ont réalisé un SASPAS. 17% réalisaient un DESC. 63,3% ont remplacé pendant leur internat. 37% avaient soutenu leur thèse. En novembre 2015, 67% exerçaient exclusivement la médecine générale, 12% une autre activité, 7% une activité mixte, 15% étaient encore internes. Dans l’avenir, 50% souhaitaient pratiquer exclusivement la médecine générale, 5% une autre activité, 33% une activité mixte et 12% étaient indécis. Les déterminants liés au type d’exercice étaient la réalisation d’un SASPAS, d’un DESC et la soutenance précoce de la thèse. Les déterminants liés au lieu d’exercice étaient la zone d’habitation actuelle, le statut foncier et le lieu de réalisation du stage praticien. Conclusion : La cohorte a permis de faire émerger les parcours personnels, pédagogiques et professionnels actuels et envisagés de ces jeunes médecins. Des propositions visant à améliorer le recours aux soins primaires en ont découlé. Le suivi de la cohorte permettra d’objectiver le devenir des jeunes médecins généralistes hauts-normands.

L'objectif de ces études était de connaitre le profil des jeunes médecins généralistes français afin de proposer des solutions d'amélioration de la répartition des médecins généralistes et de réduire ces inégalités d'accès aux soins. L'objectif de notre étude était d'identifier les sous cohortes régionales de jeunes médecins généralistes afin de reconstituer une cohorte nationale et de mettre en évidence les déterminants de leur parcours professionnel. Méthode Un recensement des internes de médecine générale ayant débuté leur DES entre 2008 et 2014 à été fait. Une étude de cohorte multi centrique rétrospective constituée des sous cohortes régionales à été réalisée. Une analyse descriptive de la population et du parcours professionnel puis une analyse univariée ont été réalisées. Résultats Quinze sous cohortes ont été identifiées. En prenant compte des données manquantes et des non répondants, les données de 13 sous cohortes soit 1275 répondants ont été agrégées et analysées. Le taux de participation était de 55,7%. La population était composée à 67,2% de femmes et l'âge moyen était de 29,2 ans. La médecine générale exclusive représentait 48,7% de la pratique actuelle avec 92,2% de remplaçants. 73% des médecins généralistes exclusifs exerçaient en MSP ou cabinet de groupe. Les facteurs influençant la pratique médicale actuelle de la médecine générale étaient le choix d'avoir fait des études de médecine pour devenir médecin généraliste et d'avoir effectué un SASPAS durant le TCEM. Les facteurs influençant la zone géographique d'exercice actuelle étaient les lieux de vie dans l'enfance et actuel, les lieux de réalisation du stage niveau 1 et le SASPAS pendant le TCEM. Conclusion La démographie médicale alarmante et la modification du mode d'organisation du travail des jeunes médecins généralistes imposent la mise en place de nouvelles mesures par les pouvoirs publics. L'objectif de ce travail est de permettre dans un second temps la création d'une cohorte nationale prospective de jeunes médecins généralistes.

Ils travaillaient en groupe (cabinet de groupe ou en maison de santé pluridisciplinaire), majoritairement en zone urbaine et semi-rurale. en Haute Normandie. Ils sont 20% à s'être installer. Les déterminants à cette installation rapide sont la soutenance la thèse et les aides financières durant leurs études. Les médecins exerçaient préférentiellement dans leur département de stage ambulatoire. Ceux ayant une partie d'activité en zone rurale étaient plus nombreux qu'en 2016. Conclusion : La cohorte a permis de mettre en évidence des hypothèses d'évolution qu'il faudra confirmer en poursuivant le suivi.
