Book excerpt: First published in 1997, this volume examines how, in the last 15 years, HIV/AIDS has become a challenge for public health, public policy and research. Reducing further HIV transmissions as well reducing the personal and social impact of HIV/AIDS requires a wide range of activities developed by a wide range of organizations – the supply of which varies widely between countries, regions and social groups. The book describes the programmes which seem particularly effective in dealing with HIB/AIDS and sets out to explain the disparities in their distribution. It documents and tries to understand both similarities as well as the variety of national approaches taken to cope with HIV/AIDS in a number of European countries. On the basis of the welfare-mix model, six country studies and an introductory chapter draw particular attention to the different mixes of public policies and private non-profit, community-based activities; the functional mixes between different types of services in the areas of prevention, care, research, control and monitoring, interest representation, fund-raising. The mixes between specialized, so-called "exclusive" HIV/AIDS service organizations and services made available by general, comprehensive, or so-called "inclusive" institutions which provide AIDS-specific programmes among other activities will also be elaborated. The whole range of HIV/AIDS activities, from professional services to self-help, is analysed in a comparative perspective. The book is based on data from the European Centre / WHO Collaborative Study Managing AIDS. It is a comparative policy study focused on the role of non-profit organizations in public health and welfare policy, covering several thousands of organizations and HIV/AIDS programmes in six European countries. Unexpected similarities and divergence in AIDS service organizations across Europe were found. The sheer multitude of programmes offered called a surprise to experts in the field, as did remaining conspicuous blank spots or deficiencies in services. Degrees of AIDS policy coherence, prevention efforts, service density and quality, self-help and professionalization, medicalization vs. social integration of HIV/AIDS programmes, sectoral specialization and institutionalization all vary tremendously, as do the efficiency and effectiveness of organizational responses to HIV/AIDS. Interestingly, variations in the supply of activities can hardly be explained by epidemiological patterns and corresponding demand and needs. AIDS management requires long-term institutional strategies and information which cannot be provided by epidemiological or behavioural analysis alone. An effective struggle against HIV/AIDS also requires institution-building, inter-organizational development and policy-field analysis.

Book excerpt: More people in communities of color are contracting, living with, and being treated for HIV/AIDS than ever before. In 2005, 71% of new AIDS cases were diagnosed in people of color. The rate of HIV infection in the African-American community alone has increased from 25% of total cases diagnosed in 1985 to 50% in 2005. Latinos similarly comprise a disproportionate segment of the AIDS epidemic: though they make up only 14% of the U.S. population, 20% of AIDS cases diagnosed in 2004 were Latino/a. Though the number of racial and ethnic minority HIV/AIDS cases continues to grow, the health care community has been unable to adequately meet the unique medical needs of these populations. African-American, Latino/Latina, and other patients of color are less likely to seek medical care, have sufficient access to the health care system, or receive the drugs they need for as long as they need them. HIV/AIDS in Minority Communities acknowledges the prevalence of HIV/AIDS within minority communities in the U.S. and strives to educate physicians about the barriers to treatment that exist for minority patients. By analyzing the main causes of treatment failure and promoting respect for individual and cultural values, this book effectively teaches readers to provide responsive, patient-centered care and devise preventive strategies for minority communities. Book excerpt: This book, written decades into the HIV epidemic, reflects critically on the idea that the socially excluded populations often focused on in HIV research are in fact difficult to access and reach. The author broadly applies the concept ‘hard to reach’ to characterize populations that researchers find difficult to engage with. Social factors that produce marginalization and ultimately result in people choosing not to engage in research are not captured by the concept of ‘hard to reach’. Limited attention has focused on how researchers can address the social factors that result in decisions to not engage in research. Disrupting the ways in which people are conceptualized as ‘hard to reach’ so as to refocus on transforming social systems and personal values, beliefs and approaches is understudied. This book uses case examples based on HIV research with Indigenous youth, internally displaced women, LGBTQ communities in the Global North and Global South, and persons at the intersection of these identities, to identify successful approaches to working with marginalized and often vulnerable communities and groups. The chapters signal the need for attention to five key social factors when developing successful approaches: context and storytelling; cultural humility; critical hope; imagination and possibility; and love, intimate inquiry, and the beloved community, if nations, individuals and communities are to address the epidemic in a sustainable and impactful way.
