Book excerpt: Excerpt from Experimental Study in Thoracic Surgery: Report of Committee on Surgery If we except a few isolated cases, recorded throughout medical literature, in which a herniated portion of lung tissue - the result of traumatism - was excised, and the removal of rare superficial tumors of the lung, pneumonectomy is, on the other hand, an essentially new operation. It is not the purpose of the writer to dwell upon the history of the first two operations mentioned, nor to present a statistical study of the results obtained up to the present time; such details may be found in the various surgical text-books and journal articles of recent years. The selection of a subject for discussion be fore this section at the present meeting was a matter of some difficulty. Having watched with great and increasing interest the labors of others in this and foreign countries, the writer decided to investigate the promising field of pneumonectomy for himself, and the present paper is the result of this decision. He has hoped to add his mite of information to the knowledge of this important subject, which he believes and predicts will, within the next ten years, become one of general inter est to the profession. Book excerpt: This revised and expanded second edition presents the most recent evidence-based facts on perioperative fluid management and discusses fluid management from basic sciences to clinical applications and the patients’ outcomes. Recent advances in understanding the Revised Starling principle with new concepts in tissue perfusion and the most recent techniques of perioperative goal directed fluid management are described. The endothelial glycocalyx functions and the influence of fluid management on its integrity are covered in detail; moreover, the techniques for its protection are also discussed. The dilemma of perioperative use of hydroxyethyl starch solutions and the resurgence of interest in using human albumin as an alternative colloid is explored. The problems of using unbuffered solutions during the perioperative period and comparison between restrictive versus liberal fluid management are discussed in full. Lastly, case scenarios for every possible clinical situation describe the most up-to-date fluid management for the corresponding clinical problem. Perioperative Fluid Management, Second Edition is of interest to anesthesiologists and also intensivists.

Book excerpt: Survival Analysis Using S: Analysis of Time-to-Event Data is designed as a text for a one-semester or one-quarter course in survival analysis for upper-level or graduate students in statistics, biostatistics, and epidemiology. Prerequisites are a standard pre-calculus first course in probability and statistics, and a course in applied linear regression models. No prior knowledge of S or R is assumed. A wide choice of exercises is included, some intended for more advanced students with a first course in mathematical statistics. The authors emphasize parametric log-linear models, while also detailing nonparametric procedures along with model building and data diagnostics. Medical and public health researchers will find the discussion of cut point analysis with bootstrap validation, competing risks and the cumulative incidence estimator, and the analysis of left-truncated and right-censored data invaluable. The bootstrap procedure checks robustness of cut point analysis and determines cut point(s). In a chapter written by Stephen Portnoy, censored regression quantiles - a new nonparametric regression methodology (2003) - is developed to identify important forms of population heterogeneity and to detect departures from traditional Cox models. By generalizing the Kaplan-Meier estimator to regression models for conditional quantiles, this methods provides a valuable complement to traditional Cox proportional hazards approaches.

Book excerpt: Excerpt from The Surgery of the Heart and Lungs: A History and Resume of Surgical, Conditions Found Therein, and Experimental and Clinical Research in Man and Lower Animals, With Reference to Pneumonotom, Pneumonectomy and Bronchotomy, and Cardiotomy and Cardiorrhaphy The author in his work as a surgeon has been brought in contact with several cases of pulmonary trouble requiring surgical intervention, and has thus become greatly interested in the subject. On taking up the study he found that there was no one work on surgery that gave more than a brief space to this subject. In order to obtain a knowledge of the operative surgery of the lung, or even a complete description of all the pulmonary lesions requiring surgical intervention, the student is forced to consult many works, and journals in many languages. For his own convenience the author collected a large mass of material, and compiled several bibliographies bearing on this subject. He also found that many questions regarding the technique of lung surgery were in doubt. In order to settle these questions to his own satisfaction, he made a series of original experiments on dogs. Considering the paucity of information contained in the standard treatises, and the inaccessible sources of the most valuable matter, the author thought that a work of this character might prove acceptable to students. Part of this work is an historical compilation, in an accessible form, of papers and reports scattered through the various journals of this and other countries, together with a bibliography of the subject. Book excerpt: Augmented reality (AR) is transforming how we work, learn, play and connect with the world, and is now being introduced to the field of medicine, where it is revolutionising healthcare as pioneering virtual elements are being added to real images to provide a more compelling and intuitive view during procedures. This book, which had its beginnings at the AE-CAI: Augmented Environments for Computer-Assisted Interventions MICCAI Workshop in Munich in 2015, is the first to review the area of mixed and augmented reality in medicine. Covering a range of examples of the use of AR in medicine, it explores its relevance to minimally-invasive interventions, how it can improve the accuracy of a procedure and reduce procedure time, and how it may be employed to reduce radiation risks. It also discusses how AR can be an effective tool in the education of physicians, medical students, nurses and other health professionals. Features: An ideal practical guide for medical professionals and students looking to understand the implementation, applications, and future of AR Contains the latest developments and technologies in this innovative field Edited by highly respected pioneers in the field, who have been immersed in AR as well as virtual reality and image-guided surgery since their inception, with chapter contributions from subject area specialists working with AR
