Book excerpt: Introduction : L'évolution de la pathologie VIH vers une affection chronique dont la morbi-mortalité se diversifie, justifie un suivi partagé des patients vivant avec le VIH (PvVIH) entre la ville et l'hôpital avec comme acteur clef de cette collaboration le médecin traitant. Nous nous sommes interrogés sur les perceptions du PvVIH, du médecin généraliste (MG) et du médecin spécialiste (MS) afin d'explorer les freins et les attentes de chacun concernant cette prise en charge partagée du PvVIH entre la ville et l'hôpital. Méthodologie : Nous avons réalisé une enquête par entretiens semi-dirigés auprès de 30 PvVIH rencontrés au sein des services hospitaliers de Nice et Antibes, de 30 MG tirés au sort en région PACA-Est en fonction du nombre de PvVIH suivis et de 16 MS des hôpitaux de Nice, Antibes, Grasse et Fréjus. Les entretiens ont été réalisés sur une période de 6 mois. Résultats : Les PvVIH étaient suivis par un MG à 96.7% et il était leur médecin référent auprès de la Sécurité Sociale dans 96.6% des cas. Les rôles attribués au MG par les PvVIH étaient principalement la prise en charge des maladies bénignes (96.7%) et les actes de prévention (66.7%). Ils étaient plus attachés à leur consultation chez le MS (93%) tous les 1à 6mois qu'à celle chez leur MG. 40% des MG n'avaient pas envie de s'impliquer dans la prise en charge des PvVIH car ils se sentaient peu à l'aise dans leur suivi.Le désir d'implication et le niveau d'aisance étaient corrélés au nombre de PvVIH suivis. 87.5% des MS se déclaraient prêts à une prise en charge partagée de leurs PvVIH entre la ville et l'hôpital. Cette collaboration nécessite un outil de communication : les MG privilégiaient l'association d'un courrier à une synthèse annuelle sous forme de « check-list » (63.3%) et l'outil internet sécurisé « Apycript » (43.3%). Les MS étaient favorables avant tout au dossier patient partagé informatisé (43.8%) et citaient volontiers l'outil Nadis®. Les patients restaient attachés au courrier papier à 96.7%. Conclusion : Le « Programme personnalisé de soins » issu d'une synthèse annuelle hospitalière semble être un élément de réponse capital. Il permettrait d'espacer les consultations auprès du MS en instaurant une « trame de suivi » qui accompagnerait le MG dans la prise en charge du PvVIH et renforcerait son sentiment de confiance. Le support privilégié pour cet outil serait un échange de données par courrier doublé d'une connexion internet sécurisée.

La prise en charge de l'infection par le VIH est donc un enjeu important de santé publique. Les dernières recommandations incitent le médecin traitant à assurer le suivi régulier des personnes vivant avec le VIH (PVVIH). Seulement, il semblerait que le niveau d'implication des médecins généralistes reste limité et que les PVVIH soient généralement suivis en milieu hospitalier. Objectif : A travers l'étude du suivi des patients infectés par le VIH au sein de la maison dispersée de santé Lille Moulin, comment appréhender le rôle du médecin généraliste dans leur prise en charge et mieux comprendre la faisabilité du suivi d'un traitement antirétroviral par le médecin généraliste ? Matériels et Méthodes : Etude descriptive au cours de l'année 2012 (réalisée au sein de la maison dispersée de santé, et au CH DRON) par analyse de dossier. Revue de la littérature actuelle sur le suivi des PVVIH par le médecin généraliste. Recueil par un questionnaire de l'expérience des praticiens de la maison dispersée de santé. Résultats : L'étude porte sur le suivi des patients séropositifs VIH sous traitement antirétroviral (ARV) au cours de l'année 2012, permettant d'inclure 159 patients. Le suivi des PVVIH est réalisable en médecine générale avec l'aide du centre de référence, seulement, l'implication des omnipraticiens semble ne pas se généraliser. Conclusion : Si l'implication de tous les médecins généralistes est peu réaliste à l'échelle nationale, il est intéressant de voir que les praticiens impliqués dans la prise en charge du VIH peuvent offrir une qualité de soin indéniable et plus flexible. Il est donc nécessaire d'informer les médecins généralistes sur la possibilité de suivre leurs patients sous antirétroviraux.

For more than 40years theories of human action have assumed that cogni tion and action are merely two sides of the same coin. Approaches as different as S-O-R behaviorism,social learning theory, consistency theories,and expectancy value theories of motivation and decision making have one thing in common: they all assume that "thought (or any other type of cognition) can move any thing," that there is a direct path from cognition to behavior. In recent years, we have become more and more aware of the complexities in volved in the relationship between cognition and behavior. People do not always do what they intend to do. Aside from several nonpsychological factors capable of reducing cognition-behavior consistency, there seems to be a set of complex psychological mechanisms which intervene between action-related cognitions, such as beliefs, expectancies, values, and intentions,and the enactment of the be havior suggested by those cognitions. In our recent research we have focused on volitional mechanismus which presumably enhance cognition-behavior consistency by supporting the main tenance of activated intentions and prevent them from being pushed aside by competing action tendencies.
