Book excerpt: The European School of Oncology came into existence to respond to a need for information, education and training in the field of the diagnosis and treatment of cancer. There are two main reasons why such an initiative was considered necessary. Firstly, the teaching of oncology requires a rigorously multidiscipli nary approach which is difficult for the Universities to put into practice since their system is mainly disciplinary orientated. Secondly, the rate of technological development that impinges on the diagnosis and treatment of cancer has been so rapid that it is not an easy task for medical faculties to adapt their curricula flexibly. With its residential courses for organ pathologies and the seminars on new techniques (laser, monoclonal antibodies, imaging techniques etc.) or on the principal therapeutic controversies (conservative or mutilating surgery, primary or adjuvant chemotherapy, radiotherapy alone or integrated), it is the ambition of the European School of Oncology to fill a cultural and scientific gap and, thereby, create a bridge between the University and Industry and between these two and daily medical practice. One of the more recent initiatives of ESO has been the institution of permanent study groups, also called task forces, where a limited number of leading experts are invited to meet once a year with the aim of defining the state of the art and possibly reaching a consensus on future developments in specific fields of on cology.

Book excerpt: Although transgender persons have been present in various societies throughout human history, it is only during the last several years that they have become widely acknowledged in our society and their right to quality medical care has been established. In the United States, endocrinologists have been providing hormonal therapy for transgender individuals for decades; however, until recently, there has been only limited literature on this subject, and non-endocrine aspects of medical care for transgender individual have not been well addressed in the endocrine literature. The goal of this volume is not only to address the latest in hormonal therapy for transgender individuals (including pediatric and geriatric age groups), but also to familiarize the reader with other aspects of transgender care, including primary and surgical care, fertility preservation, and the management of HIV infection. In addition to medical issues, psychological, social, ethical and legal issues pertinent to transgender individuals add to the complexities of successful treatment of these patients. A final chapter includes extensive additional resources for both transgender patients and providers. Thus, an endocrinologist providing care to a transgender person will be able to use this single resource to address most of the patient’s needs. While Transgender Medicine is intended primarily for endocrinologists, this book will be also useful to primary care physicians, surgeons providing gender-confirming procedures, mental health professionals participating in the care of transgender persons, and medical residents and students.

Available in PDF, EPUB and Kindle. Book excerpt: Endocrine Replacement Therapy in Clinical Practice, an update of Hormone Repla- ment Therapy published by Humana Press in 1999, aims to assist the endocrinologist, gynecologist, pediatrician, urologist, general surgeon, neurologist, neurosurgeon, p- chiatrist, generalist, and trainee in management of their patients with hormonal defici- cies or altered hormonal synthesis or responses. Many new authors have added several new chapters, and all of the previous chapters have been updated. Endocrine testing used to diagnose endocrine disorders and monitor hormone replacement therapy is reviewed. However, detailed discussion of physiology and pathophysiology is not an aim of this book, and these topics are covered in other volumes on endocrinology. Although no one is considered an expert in all areas of endocrine replacement therapy, each of the authors here have extensive knowledge and experience in the management of patients with s- cific endocrine disorders requiring hormone replacement treatment. Although some differences of opinion exist among experts in replacement therapy, each writer has attempted to give a balanced, unbiased recommendation. When com- rable regimens exist, the authors have made this apparent to the reader who can then choose the best treatment for the individual patient. Cost, therapeutic effectiveness, and route of administration are all important considerations in making the final selection of replacement therapy.

Available in PDF, EPUB and Kindle. Book excerpt: The International Handbook on the Demography of Sexuality is the first book to specifically address the study of sexuality from a demographic perspective. Demographic research has largely paid little attention to sexuality as a whole, or sexual orientation in particular, other than in studies examining the “consequences” of sex – sexually transmitted infections or fertility. Instead, the content of this handbook explores population sexuality in order to describe the prevalence of sexual behaviors, desires, and identities, as well as their connections with other demographic outcomes. The focus is on analyzing sexuality as a demographic topic in its own right, rather than solely as a variable in studies of sexually transmitted infection or other health-related topics. In this book, both researchers with traditional demographic backgrounds, as well as those with training in other disciplines, provide an overview of the state of current research on population sexuality. These chapters provide a foundation for the development of research in the burgeoning field of the demography of sexuality.

Book excerpt: It has been known for over 40 years that GH-deficient-children benefit from replacement with the hormone. But GH, essential for longitudinal growth, also plays a role after completion of final height. With the introduction of biosynthetic human GH 20 years ago, the use of GH was no longer restricted to severe growth retardation in hypopituitary children. This book will take the reader behind the myths of GH and into the real world of clinical endocrinology. The contributions stem from recognized clinicians and scientists who have been working in the field for decades. The contents encompass traditional end points of GH therapy such as body composition, bone biology and physical performance. Attention is also devoted to diagnostic aspects and side effects. Additional features range from clinical epidemiology to quality of life, and novel areas such as the impact of traumatic brain injury on pituitary function are also covered. The present volume of Frontiers of Hormone Research is essential reading for health care professionals interested in clinical endocrinology and GH.

Book excerpt: The U.S. Food and Drug Administration (FDA) has approved dozens of hormone therapy products for men and women, including estrogen, progesterone, testosterone, and related compounds. These products have been reviewed for safety and efficacy and are indicated for treatment of symptoms resulting from hormonal changes associated with menopause or other endocrine-based disorders. In recent decades, an increasing number of health care providers and patients have turned to custom-formulated, or compounded, drug preparations as an alternative to FDA-approved drug products for hormone-related health concerns. These compounded hormone preparations are often marketed as "bioidentical" or "natural" and are commonly referred to as compounded bioidentical hormone therapy (cBHT). In light of the fast-growing popularity of cBHT preparations, the clinical utility of these compounded preparations is a substantial public health concern for various stakeholders, including medical practitioners, patients, health advocacy organizations, and federal and state public health agencies. This report examines the clinical utility and uses of cBHT drug preparations and reviews the available evidence that would support marketing claims of the safety and effectiveness of cBHT preparations. It also assesses whether the available evidence suggests that these preparations have clinical utility and safety profiles warranting their clinical use and identifies patient populations that might benefit from cBHT preparations in lieu of FDA-approved BHT.

Available in PDF, EPUB and Kindle. Book excerpt: This book describes recent advances in translational research in breast cancer and presents emerging applications of this research that promise to have meaningful impacts on diagnosis and treatment. It introduces ideas and materials derived from the clinic that have been brought to "the bench" for basic research, as well as findings that have been applied back to "the bedside". Detailed attention is devoted to breast cancer biology and cell signaling pathways and to cancer stem cell and tumor heterogeneity in breast cancer. Various patient-derived research models are discussed, and a further focus is the role of biomarkers in precision medicine for breast cancer patients. Next-generation clinical research receives detailed attention, addressing the increasingly important role of big data in breast cancer research and a wide range of other emerging developments. An entire section is also devoted to the management of women with high-risk breast cancer. Translational Research in Breast Cancer will help clinicians and scientists to optimize their collaboration in order to achieve the common goal of conquering breast cancer.

Available in PDF, EPUB and Kindle. Book excerpt: Several resources exist as reference guidelines for the initiation and maintenance of cross-sex hormone therapy. These include the Lyon Martin clinic, UCSF Center of Excellence for Transgender Health, American Endocrine Society, etc. However, no pocket guide exists for cross-sex hormone therapy, and the references listed above are often not all inclusive. As a result, primary care providers still commonly refer patients to endocrinology for something that can be initiated at a family practice clinic. The TransPocket educational guide created provides health care practitioners with an easy-to-use reference guide for both the initiation of transgender cross-sex hormone therapy as well as basic information that can be utilized during a patient encounter with a transgender patient. A prototype guide was evaluated by a panel of providers experienced in transgender care which was then revised into the final draft which was well-received by the panelists as a useful guide for primary care providers.
