Book excerpt: This research investigated the effects of static lower extremity posture on hip strength, and then examined their collective influence on knee and hip joint kinematics during a single leg squat in males and females. Thirty one healthy males and 31 healthy females, predominantly college students, between the ages of 18 and 35 participated in a single data collection session during which six lower extremity posture characteristics were measured, followed by measurement of hip abduction and extension strength and concluded with neuromuscular and kinematic analysis of the hip and knee during a single leg squat. Hip torque was normalized to body mass and electromyographic data were normalized to maximum voluntary isometric contractions. Five single leg squats were performed on the dominant stance limb to a depth of 60° of knee flexion. Path analysis, implemented by structural equation modeling, was used to examine whether greater lower extremity posture characteristics predicted decreased hip torque and whether greater lower extremity posture characteristics and decreased hip torque collectively predicted greater dynamic valgus knee motion (increased hip adduction and internal rotation, and knee external rotation and valgus). Separate multivariate analyses of variance determined whether females and males differed on measures of lower extremity posture, hip strength, and total hip and knee motion during the single leg squat. The findings were that greater hip anteversion predicted decreased hip abduction torque, and greater tibiofemoral angle predicted decreased hip extension torques. Direct relationships were noted between greater hip anteversion and genu recurvatum with greater knee external rotation, and between greater navicular drop and hip anteversion with greater hip internal rotation during the single leg squat. Furthermore, decreased hip abduction torque predicted greater knee external rotation while decreased hip extension torque predicted greater knee valgus during a single leg squat. Hence, it was concluded that greater lower extremity posture characteristics predicted decreased posterio-lateral hip strength, and collectively, greater lower extremity posture characteristics and decrease posterio-lateral hip strength predicted greater functional valgus collapse during the single leg squat.

Book excerpt: Patellofemoral pain syndrome (PFPS) remains a prevalent athletic injury, especially among females. Hip strength differences between females with and without PFPS has led to speculation that individuals with PFPS are prone to abnormal lower extremity alignment during weightbearing activities. This is thought to be due, in part, to an inability to adequately resist external moments at the hip. This tendency for malalignment may be especially evident if weightbearing activities are performed in a state of fatigue. The first aim of this study was to analyze lower extremity mechanics in females with and without patellofemoral pain (PFPS) during three different activities. We hypothesized that differences in lower extremity mechanics between groups would increase during activities with greater loading. 20 females with PFPS and 20 healthy female controls performed single leg (SL) squats, running, and repetitive SL jumps. The results suggest the PFPS group consistently demonstrates altered lower extremity kinematics during each activity, regardless of the magnitude of the external load. For the second aim, we examined the effect of exertion on hip strength and lower extremity mechanics between groups during SL jumps. We hypothesized that differences between groups would be greater following the exertion protocol. However, PFPS subjects demonstrated hip mechanics that differed from the control group throughout the protocol, and both groups demonstrated a similar response to exertion. The third aim of this study was to examine the association between trunk, hip, and knee joint strength and lower extremity mechanics between groups. Subjects with PFPS exhibited decreased hip and trunk strength compared with healthy subjects. However, these strength measurements were weakly to moderately correlated with mechanics suspected to contribute to the etiology or exacerbation of PFPS. The final aim of this study was to evaluate the construct validity and clinical utility of a 2D measure of lower extremity alignment (FPPA). The results suggest that the FPPA may be a useful clinical measure. The FPPA was significantly associated with LE kinematics known to increase retropatellar stress during dynamic activities. Additionally, PFPS subjects demonstrated greater FPPA values than the control group. The results of these four aims highlight kinematic and kinetic differences between females with and without PFPS which may contribute to the exacerbation and etiology of this condition. Conservative treatment programs focused on minimizing these differences in hip strength and lower extremity mechanics may improve long term patient outcomes.

Book excerpt: Asymmetrical loading between lower limbs can theoretically be explained as an inequality of strength, neuromuscular control, or subconscious reliance on one leg more than the other. Asymmetries are often identified utilizing costly equipment such as force plates and 3D motion analysis cameras. It is important to establish less costly ways of identifying movement and loading asymmetries for clinicians to utilize. One qualitative assessment that may identify asymmetries is a 'lateral hip shift' during an overhead squat. Objective: To identify differences in lower extremity range of motion (ROM) and plantar foot pressures in individuals with and without a lateral hip shift during an overhead squat. Design: Cross-Sectional Observation. Setting: Lab. Participants: Twenty-nine (14 males and 15 females) physically active individuals participated in this study. Seventeen individuals with a lateral hip shift during an overhead squat (LAT; Age = 21.2 +/- 2.1 years, Height = 175.1 +/- 9.1 cm, Mass = 77.6 +/- 14.2 kg) and twelve without (CON; Age = 20.8 +/- 2.1 years, Height = 177.4 +/- 6.8cm, Mass = 77.8 +/- 11.1kg) were identified through screening. Interventions: Participants were screened for a lateral hip shift while performing five overhead squats to determine group allocation (LAT, CON). All ROM measures were performed in a randomized order followed by additional trials of the overhead squat while standing on the pressure sensor mat. Main Outcome Measures: Active, passive, and weight-bearing ROM for ankle dorsiflexion (DF) and passive hip abduction, internal, and external rotation were measured. Plantar pressure variables were captured via a pressure sensor mat during the overhead squat, including side-to-side weight bearing %, anterior-posterior excursion (cm), left-right excursion (cm), and center of force (COF) distance (cm). Results: A significant group-by-limb interaction for active DF ROM, such that the LAT group had less active DF ROM with the knee flexed on the limb they shifted away from in comparison to the assigned CON limb (mean difference = 6.87º +/- 0.2). The LAT group also had less active DF ROM with the knee extended (3.07° +/- 1.25) compared to the CON group (8.30° +/- 1.49). No differences were observed on other ROM variables. The LAT group demonstrated greater movement in their center of force on the plantar pressure mat for total distance (mean difference = 24.27 cm +/- 0.4) and LR excursion (mean difference = 2.96 cm +/- 3.2). Conclusion: Participants with a lateral shift during an overhead squat generally had decreased active ankle DF ROM and greater movement of their center of force. Clinically, addressing active DF ROM may promote more symmetrical loading during an overhead squat.

Female athletes are 4 to 8 times more at risk of suffering an anterior cruciate ligament injury (ACL). Strength is believed to be an important predictor of ACL injury in female athletes and has been a focus of improvement in injury prevention programs. However, there is a lack of consensus on the relationship between muscle strength and hip and knee kinematics during landing tasks. Only a few studies used a lower extremity relative strength measure and reported promising results, but none of them reported EMG activity. The goal of this study was to explore the relationship between back squat strength to body mass ratio and the lower limb muscle activity during single-leg landing tasks. Twenty-eight varsity athletes were recruited (13 males and 15 females) from various sports from the universities in the Montreal, Québec region. Participants were asked to perform a standardized 1-RM back squat test followed by single-leg drop landings from 28cm and 44cm plyometric boxes. EMG data was collected 150ms pre-initial and 250ms post-initial contact for the following muscles: biceps femoris (BF), semitendinosus (ST), vatus lateralis (VL), vastus medialis (VMO), rectus femoris (RF), gluteus medius (GMED) and gluteus maximus (GMAX). Significant differences were found between male and female athletes on the back squat one-repetition maximum and squat to body mass ratio (SQ:BW). Females had significantly more neuromuscular activity of the quadriceps and GMAX, and significantly less activity of the biceps femoris in both landing phases. After controlling for strength, only significant differences in RF, BF and GMAX activity remained between sexes in the deceleration phase. Prior to landing, moderate negative correlations between VL, VMO and GMAX activity and SQ:BW were observed at both drop heights. During the post-initial contact phase, moderate negative correlations for VL and VMO activity were found at both drop heights. Our findings suggest that researchers looking at sex-based differences in muscle activity should control for relative strength differences between groups as it may influence muscle activity during landing tasks.

However, there has been an increase in use of minimalist style shoes in activities other than running such as resistance training, hiking, cross training, etc. When comparing barefoot and minimalist to shod conditions there have been positive adaptations found. While there are adaptations in running gait it is unknown if adaptations occur in other activities when comparing barefoot and minimalist to shod conditions. The goal of this study was to investigate biomechanical differences in an overhead and single leg squat in barefoot, minimalist, and shod conditions. It was hypothesized that (a) barefoot, minimalist, and shod conditions will result in differences in joint excursion and total mechanical energy expenditure at the ankle, knee, and hip (b) minimalist conditions will be similar to barefoot conditions when compared to shod conditions, and (c) ankle range of motion will exacerbate these differences. A total of 32 able-bodied, Division I athletes that partook in strength and condition sessions with a certified strength and conditioning specialist (CSCS) participated in the current study. Participants had no history of lower leg injury in the previous six months. Participants were grouped based on their choice of daily footwear, minimalist style or traditional shod.. Participants completed ten repetitions of a single leg and overhead squat in barefoot and minimalist/shod conditions. Testing consisted of 3D motion analysis. The results of the study found that ankle range of motion did not differ between groups (P= .566). During an overhead squat it was found that there was a significant effect at the hip by condition (P= .004), all other interactions, joint excursions, and total mechanical energy expenditure were not significant. In the single leg squat it was found that there was a significant effect at knee by condition (P= .040), all other interactions, joint excursions, and total mechanical energy expenditure were not significant. Clinicians and researchers can use the results of the study to further investigate differences in barefoot, minimalist, and shod activities in activities other than running.
