Book excerpt: The Institute of Medicine's (IOM's) Roundtable on Research and Development of Drugs, Biologics, and Medical Devices evolved from the Forum on Drug Development, which was established in 1986. Sponsor representatives and IOM determined the importance of maintaining a neutral setting for discussions regarding long-term and politically sensitive issues justified the need to revise and enhance past efforts. The new Roundtable is intended to be a mechanism by which a broad group of experts from the public* and private sectors can be convened to conduct a dialogue and exchange information related to the development of drugs, biologics, and medical devices. Members have expertise in clinical medicine, pediatrics, clinical pharmacology, health policy, health insurance, industrial management, and product development; and they represent interests that address all facets of public policy issues. From time to time, the Roundtable requests that a workshop be conducted for the purpose of exploring a specific topic in detail and obtaining the views of additional experts. The first workshop for the Roundtable was held on April 14 and 15, 1998, and was entitled Assuring Data Quality and Validity in Clinical Trials for Regulatory Decision Making. The summary on that workshop is available from IOM. This workshop summary covers the second workshop, which was held on May 24 and 25, 1999, and which was aimed at facilitating the development and proper use of drugs, biologics, and medical devices for infants and children. It explores the scientific underpinnings and clinical needs, as well as the regulatory, legal, and ethical issues, raised by this area of research and development.

Available in PDF, EPUB and Kindle. Book excerpt: The book provides an exhaustive, authoritative and updated review on the interindividual variability in drug metabolism in humans. Four chapters address the general background: genetic factors causing variability, interethnic variability, environmental factors and developing and ageing as sources of variability. Six chapters address variability of drug metabolism in vivo: variability of psychotropic drugs, antiepileptic drugs, the dopamine precursor levodopa, cardiovascular active drugs and anti HIV drugs. Seven chapters address the interindividual variability of the main drug metabolizing enzymes: CYP-450s, acetyltransferases, glucuronosyl transferase, methyl transferases, sulfotransferases and glutathione transferases in human liver and extrahepatic tissues. A separate chapter discusses the prediction of drug interaction. Comprehensive in coverage, and with contributions from the leading international experts, this book is essential reading for researchers from both academia and the pharmaceutical industry. Advanced undergraduates and postgraduates in pharmacology, clinical pharmacology, toxicology, biochemistry and epidemiology who are interested in drug metabolism will also find this an indispensable resource.

Book excerpt: The EPA commissioned The National Academies to provide advice on the vexing question of whether and, if so, under what circumstances EPA should accept and consider intentional human dosing studies conducted by companies or other sources outside the agency (so-called third parties) to gather evidence relating to the risks of a chemical or the conditions under which exposure to it could be judged safe. This report recommends that such studies be conducted and used for regulatory purposes only if all of several strict conditions are met, including the following: The study is necessary and scientifically valid, meaning that it addresses an important regulatory question that can't be answered with animal studies or nondosing human studies; The societal benefits of the study outweigh any anticipated risks to participants. At no time, even when benefits beyond improved regulation exist, can a human dosing study be justified that is anticipated to cause lasting harm to study participants; and All recognized ethical standards and procedures for protecting the interests of study participants are observed. In addition, EPA should establish a Human Studies Review Board (HSRB) to evaluate all human dosing studiesâ€"both at the beginning and upon completion of the experimentsâ€"if they are carried out with the intent of affecting the agency's policy-making.

Book excerpt: The Best Pharmaceuticals for Children Act (BPCA) and the Pediatric Research Equity Act (PREA) were designed to encourage more pediatric studies of drugs used for children. The FDA asked the IOM to review aspects of pediatric studies and changes in product labeling that resulted from BPCA and PREA and their predecessor policies, as well as assess the incentives for pediatric studies of biologics and the extent to which biologics have been studied in children. The IOM committee concludes that these policies have helped provide clinicians who care for children with better information about the efficacy, safety, and appropriate prescribing of drugs. The IOM suggests that more can be done to increase knowledge about drugs used by children and thereby improve the clinical care, health, and well-being of the nation's children.

Book excerpt: Handbook of Mammalian Metabolism of Plant Compounds provides an extensive survey of how mammals metabolize compounds found in higher plants. Information about these compounds is important to researchers in pharmacology, toxicology, medical and natural products chemistry, and food sciences. Although there is plenty of literature about mammalian metabolism of plant compounds, it is scattered, and summaries of specific plant compounds are often difficult to obtain. This book contains 11 chapters discussing the metabolic fate of individual plant compounds grouped according to chemical class. It also features structural formulas of the compounds and their metabolites, including probable pathways of metabolism. This book provides a convenient single-volume reference source that will benefit all researchers requiring this type of information.

Book excerpt: Volume 31 of "Progress in Drug Research" contains 13 articles, a sub ject index, an index for all articles that have been published so far in this series of monographs, as well as an author and subject index for all 31 volumes. The reviews in this volume are particularly concerned with the therapy of helminth diseases, with pyramidinones as biody namic agents, and with quinolones which are of interest in the treat ment of infections. For a deeper understanding of the pharmacokinet ic actions of modem drugs, the articles on cooperative binding of drug molecules to DNA, on in vitro models for the study of antibiotic activi ties, on inhibitors of the renin-angiotensin system, on GAB A-drug in teractions, and on the mechanism of action of anxiolytic drugs provide a wealth offacts and new findings. The pharmacology of caffeine is re viewed from the viewpoint of its role in combination with other phy siologically active substances, and the chapter on high resolution nuc lear magnetic resonance spectroscopy demonstrates the importance of this method in the development of new drugs. Finally, the article on light and dark touches border problems of the therapy of psychic dis orders. With these contributions, the authors aim to summarize latest achieve ments in important and actual fields of drug research. Researchers who are actively engaged in the same or in similar fields of research are sure to benefit from these efforts.
