Book excerpt: Introduction : L’accident vasculaire cérébral (AVC) de l’enfant est rare et les études sur le devenir à long terme, en termes de scolarisation, autonomie et participation sociale, sont peu nombreuses. Le but de notre étude était d’étudier les caractéristiques cliniques de ces patients, le devenir moteur, fonctionnel, cognitif et scolaire à long terme, ainsi que les facteurs prédictifs de ce devenir. Méthodes : Nous avons repris rétrospectivement les dossiers des enfants consécutivement admis dans un service de médecine physique et réadaptation après un AVC (survenu après 28jours de vie) entre 1992 et 2010. L’âge de survenue, l’étiologie, l’atteinte sensitivo-motrice et fonctionnelle à l’entrée et à la sortie du service, les premiers et derniers bilans neuropsychologiques, la scolarisation ont été étudiés. Résultats : 128 enfants ont été hospitalisés après un AVC ischémique (n=46) ou hémorragique (n=82). A l’admission, 22 jours (8-538) après l’AVC, 53% étaient hémiplégiques et seulement 44% marchaient seuls. A la sortie 80% marchaient seuls et 57% pouvaient utiliser leur main. Le devenir fonctionnel était influencé par le type d’AVC, la présence d’un coma initial et d’une épilepsie. Les bilans neuropsychologiques à 3 (0,5-41) et 29 (2-143) mois post-AVC montraient, en moyenne, un QI total (QIT) une déviation standard en deçà des normes. De plus 25 à 50% des patients présentaient des troubles de la mémoire antérograde et 25 à 35% avaient des troubles des fonctions exécutives. Après un suivi médian de 43 mois, seuls 53% des enfants avaient une scolarité normale (dont 30% avaient une aide) et 41% suivent une scolarité spécialisée.
