To address this safety concern, scientists have often relied on accident data. Because of the rare nature of accidents, commercial aviation incident and near miss data may prove to be useful sources of safety information. In one such study, the National Transportation Safety Board interviewed GA pilots that were flying near a weather-related accident in pursuit of a different perspective than that of the accident pilot. Interviewing GA pilots about their own weather-related event may provide similar benefits. To understand factors leading GA pilots to encounter adverse weather conditions, pilots involved in an adverse weather encounter were interviewed using a one-hour structured interview. The interview was developed using surveys utilized by National Aeronautics and Space Administration and the Federal Aviation Administration (FAA). In total, 27 pilots who experienced an adverse weather encounter were interviewed, of which 25 were included in the final analysis.

Previous studies conducted by the FAA and others found many GA accidents involving flight into adverse weather were categorized as a willful disregard for the rules and regulations of safety; violations as defined by the Human Factors Analysis and Classification System. Contrary to what the accident record seems to suggest, flight into adverse weather may also be influenced by the lack of appreciation/understanding of the hazards associated with adverse weather. Perhaps some encounters with adverse weather were motivated by outside influences or exacerbated by some manner of mechanical failure that may have led to the willful acceptance of unnecessary hazards. Conclusions. These data suggest that current beliefs surrounding flight into adverse weather by GA pilots may be incomplete.

Most conventional health promotion interventions for older adults take very reactive approaches, typically attempting to minimise specific age-related functional losses. This implies an underlining assumption that such age-related losses are inevitable. However, we know that it is possible to take proactive action to prevent or mitigate negative health events in later life before they occur. Research suggests that proactive coping and future investment strategies may work harmoniously with adaptive definitions of healthy ageing. However, this concept has not been tested as part of a proactive behavioural intervention for a broad perspective of healthy ageing. This research explored the implementation and controlled evaluation of a behavioural health promotion intervention that incorporated proactive coping strategies to facilitate healthier ageing amongst older adults. The intervention was built on a theoretical foundation combining constructs from the Health Belief Model, the Theories of Planned Behaviour and Reasoned action, and the Transtheoretical Model. A field pilot intervention was conducted to help shape the intervention content and to identify process limitations. The pilot intervention consisted of nine two-hour sessions held over consecutive weeks. Each session combined proactive coping workshops and facilitated goal setting with motivational presentations on topics pertinent to healthy ageing. Qualitative feedback was very positive, with 98% of post-intervention survey respondents (n = 43) indicating that their health behaviours had positively improved. The participants identified group interaction, a motivating facilitator, and goal setting as the key factors that influenced the efficacy of the intervention. The intervention and evaluation methodology was improved, and the main intervention was then delivered in three formats, based on the key influencing factors identified in the pilot evaluation. Each intervention used the same information base for five sessions held over consecutive weeks. A 2.5-hour workshop-based group intervention reflected the pilot intervention, featuring facilitated (group) interaction, motivating facilitator, and goal setting. A 1.5-hour lecture-based (group) intervention featured the motivating facilitator and goal setting, while a workbook-based (individual) intervention featured goal setting only. A spectrum of dependant variables related to healthy ageing was measured via surveys at pre-intervention, intervention conclusion, and post-intervention stages, using an explanatory mixed methodological approach. A focus group interview was conducted with each group eight weeks after the conclusion of the intervention. Results indicate that both group-based interventions achieved self-reported improvements (approaching significance) to health activities. The lecture-based interventions achieved significant improvements to memory and health knowledge, while the workshop-based intervention facilitated improvements approaching significance to overall health and health knowledge. Additionally, the workshop-based intervention facilitated a significant improvement to self-efficacy through to the post-intervention stage (eight weeks after the intervention conclusion). Qualitative feedback revealed examples of changes made by individuals that were significant within personally relevant domains. Promoting proactive coping amongst groups of older adults appears to be an effective strategy for facilitating efforts towards healthier ageing. A broad, proactive approach is a necessary counterpart to the specific, reactive approaches of most health promotion interventions for older adults.

Unlike reactive and proactive safety, predictive safety can predict the next accident and enable prevention before an actual occurrence. The study outlined here promotes predictive safety management through machine learning technologies using large amounts of data to facilitate predictive modeling. The study addresses efforts to reduce General Aviation accidents, an effort that was renewed in earnest with the Federal Aviation Administration’s 1998 Safer Skies Initiative. Over the past 22 years, the General Aviation fatality rate has decreased. However, accidents still happen, and there is some evidence showing the number of accidents, representing hazard exposure, is increasing. The accident data suggest that the aviation community still has more to learn about the variables involved in an accident sequence. The purpose of the study was to conduct an exploratory data-driven examination of General Aviation accidents in the United States from January 1, 1998, to December 31, 2018, using machine learning and data mining techniques. The goal was to determine what model best predicts fatal and severe injury aviation accidents and further, what variables were most important in the prediction model. The study sample comprised 26,387 fixed-wing general aviation accidents accessed through the publicly accessible National Transportation Safety Board Aviation Accident Database and Synopses archive. Using a mixed-methods approach, the study employed both unstructured narrative text and structured tabular data within the predictive modeling. First, the accident narratives were culled using text mining algorithms to develop text-based quantitative variables. Next, data mining algorithms were used to develop models based on both text- and data-based variables derived from the accident reports. Five types of machine learning models were created using SAS® Enterprise MinerTM, including the Decision Tree, Gradient Boosting, Logistic Regression, Neural Network, and Random Forest. Additionally, three broad sets of variables were used in modeling, including text-only, data-only, and a combination of text and data variables. Three models, Logistic Regression (text-only variables), Random Forest (text-only variables), and Gradient Boosting (text and data variables), emerged with a similar prediction capability. The top six variables within the models were all text-based covering Medical, Slow-flight and stalls, Flight control, IMC flight, Weather factors, and Flight hours topics. The Logistic Regression (Text) model was selected as the champion model: Misclassification Rate = 0.098, ROC Index = 0.945, and Cumulative Lift = 3.46. The results of the study provide insights to the entire General Aviation community, including government, industry, flight training, and the operational pilot. Specific recommendations include the following areas: 1) improve the quality and usefulness of accident reports for machine learning applications, 2) investigate ways to capture and publish more open-source flight data for use in safety modeling, 3) invest in additional medical education and find ways to address impairing medications and high risk medical conditions, 4) renew efforts on improving flight skills and combatting decision-based errors, 5) emphasize the importance of weather briefings, pre-flight planning, and weather-based risk management, and 6) create an aviation-specific corpus for text mining to improve text analysis and transformation."--Abstract.
